Dear Mr. Weaver:
We arc submitting for your review and approval revised drawings for the approved BMP at the above
mentioned facility. This revision is to repair the forebay berm in the BMP, and does not change the
overall BMP design and functionality.
During construction, it has been brought to our attention that the forebay berm has been breached due
to the occurrence of several large storm events.. The attached documents of the plan view and the cross
section of the BMP show how we are proposing to revise the approved design to provide reinforcement
to the forebay berm by the use of geogrid fabric and riprap.
Please notify us at your earliest convenience if these revisions meet your standards so we may notify
the contractor to begin this repair work.

Dear Commander Readal:
The Wilmington Regional Office received a complete Stormwater Management Permit
Application for FSRG Operations & Maintenance Facility, 2nd LAR Armory on March 10, 2006.
Staff review of the plans and specifications has determined that the project, as proposed, will
comply with the Stormwater Regulations set forth in Title 15A NCAC 2H.1000. We are
forwarding Permit No. SW8 051065 dated March 14, 2006, for the construction of the subject
project.
This permit shall be effective from the date of issuance until March 14, 2016, and shall be
subject to the conditions and limitations as specified therein. Please pay special attention to
the Operation and Maintenance requirements in this permit. Failure to establish an adequate
system for operation and maintenance of the stormwater management system will result in
future compliance problems.
If any parts, requirements, or limitations contained in this permit are unacceptable, you have
the right to request an adjudicatory hearing upon written request within thirty (30) days
following receipt of this permit. This request must be in the form of a written petition,
conforming to Chapter 150B of the North Carolina General Statutes, and filed with the Office of
Administrative Hearings, P.O. Drawer 27447, Raleigh, NC 27611-7447. Unless such demands
are made this permit shall be final and binding.

the provisions of 15A NCAC 2H A 000 (hereafter referred to as the "stormwater rules')
and the approved stormwater management plans and specifications and other
supporting data as attached and on file with and approved by the Division of Water
Quality and considered a part of this permit.
This permit shall be effective from the date of issuance until March 14, 2016, and shall
be subject to the following specified conditions and limitations:
I. DESIGN STANDARDS
1. This permit is effective only with respect to the nature and volume of stormwater
described in the application and other supporting data.
2. This stormwater system has been approved for the management of stormwater
runoff as described in Section 1.6 on page 3 of this permit. The stormwater
control has been designed to handle the runoff from 106,722 square feet of
impervious area.
3. The tract will be limited to the amount of built -upon area indicated on page 3 of
this permit, and per approved plans. There is no built -upon area available for
future development in this permit.
4. All stormwater collection and treatment systems must be located in either
dedicated common areas or recorded easements. The final plats for the project
will be recorded showing all such required easements, in accordance with the
approved plans.
5. The runoff from all built -upon area within the permitted drainage area of this
project must be directed into the permitted stormwater control system.

II. SCHEDULE OF COMPLIANCE
1. The stormwater management system shall be constructed in its entirety,
vegetated and operational for its intended use prior to the construction of any
built -upon surface.
2. During construction, erosion shall be kept to a minimum and any eroded areas of
the system will be repaired immediately.
3. The permittee shall at all times provide the operation and maintenance
necessary to assure the permitted stormwater system functions at optimum
efficiency. The approved Operation and Maintenance Plan must be followed in
its entirety and maintenance, must occur at the scheduled intervals including, but
not limited to:
a. Semiannual scheduled inspections (every 6 months).
b. Sediment removal.
C. Mowing and revegetation of slopes and the vegetated filter.
d. Immediate repair of eroded areas.
e. Maintenance of all slopes in accordance with approved plans and
specifications.
f. Debris removal and unclogging of outlet structure, orifice device, flow
spreader, catch basins and piping.
g. Access to the outlet structure must be available at all times.

4. Records of maintenance activities must be kept and made available upon
request to authorized personnel of DWQ. The records will indicate the date,
activity, name of person performing the work and what actions were taken.
5. Decorative spray fountains will be allowed in the stormwater treatment system,
subject to the following criteria:
a. The fountain must draw its water from less than 2' below the permanent
pool surface.
b. Separated units, where the nozzle, pump and intake are connected by
tubing, may be used only if they draw water from the surface in the
deepest part of the pond.
c. The falling water from the fountain must be centered in the pond, away
from the shoreline.
d. The maximum horsepower for a fountain in this pond is 118 horsepower.
6. The facilities shall be constructed as shown on the approved plans. This permit
shall become voidable unless the facilities are constructed in accordance with
the conditions of this permit, the approved plans and specifications, and other
supporting data.
7. Upon completion of construction, prior to issuance of a Certificate of Occupancy,
and prior to operation of this permitted facility, a certification must be received
from an appropriate designer for the system installed certifying that the permitted
facility has been installed in accordance with this permit, the approved plans and
specifications, and other supporting documentation. Any deviations from the
approved plans and specifications must be noted on the Certification. A
modification may be required for those deviations.
8. If the stormwater system was used as an Erosion Control device, it must be
restored to design condition prior to operation as a stormwater treatment device,
and prior to occupancy of the facility.
9. Access to the stormwater facilities shall be maintained via appropriate
easements at all times.
10. The permittee shall submit to the Director and shall have received approval for
revised plans, specifications, and calculations prior to construction, for any
modification to the approved plans, including, but not limited to, those listed
below:
a. Any revision to any item shown'on the approved plans, including the
stormwater management measures, built -upon area, details, etc.
b. Project name change.
C. Transfer of ownership.
d. Redesign or addition to the approved amount of built -upon area or to the
drainage area.
e. Further subdivision, acquisition, lease or sale of all or part of the project
area. The project area is defined as all property owned by the permittee,
for which Sedimentation and Erosion Control Plan approval or a CAMA
Major permit was sought.
f. Filling in, altering, or piping of any vegetative conveyance shown on the
approved plan.
11. The permittee shall submit final site layout and grading plans for any permitted
future areas shown on the approved plans, prior to construction.
12. A copy of the approved plans and specifications shall be maintained on file by
the Permittee for a minimum of ten years from the date of the completion of
construction.

13. The Director may notify the permittee when the permitted site does not meet one
or more of the minimum requirements of the permit. Within the time frame
specified in the notice, the permittee shall submit a written time schedule to the
Director for modifying the site to meet minimum requirements. The permittee
shall provide copies of revised plans and certification in writing to the Director
that the changes have been made.
III. GENERAL CONDITIONS
This permit is not transferable except after notice to and approval by the Director.
In the event of a change of ownership; or a name change, the permittee must
submit a.formal permit transfer request to the Division of Water Quality,
accompanied by a completed name/ownership change form, documentation
from the parties involved, and other supporting materials as may be appropriate.
The approval of this request will be considered on its merits and may or may not
be approved. The permittee is responsible for compliance with all permit
conditions until such time as the Division approves the transfer request.
2. Failure to abide by the conditions and limitations contained in this permit may
subject the Permittee to enforcement action by the Division of Water Quality, in
accordance With North Carolina General Statute 143-215.6A to 143-215.6C.
3. The issuance of this permit does not preclude the Permittee from complying with
any and all statutes, rules, regulations, or ordinances, which may be imposed by
other government agencies (local, state, and federal) having jurisdiction.
4. In the event that the facilities fail to perform satisfactorily, including the creation
of nuisance conditions, the Permittee shall take immediate corrective action,
including those as may be required by this Division, such as the construction of
additional or replacement stormwater management systems.
5. The permittee grants DENR Staff permission to enter the property during normal
business hours for the purpose of inspecting all components of the permitted
stormwater management facility.
6. The permit may be modified, revoked and reissued or terminated for cause. The
filing of a request for a permit modification, revocation and reissuance or
termination does not stay any permit condition.
7. Unless specified elsewhere, permanent seeding requirements for the stormwater
control must follow the guidelines established in the North Carolina Erosion and
Sediment Control Planning and Design Manual.
8. Approved plans and specifications for this project are incorporated by reference
and are enforceable parts of the permit.
9. The permittee shall notify the Division any name, ownership or mailing address
changes within 30 days.

Certification Requirements:
1. The drainage area to the system contains approximately the permitted
acreage.
2. The drainage area to the system contains no more than the permitted
amount of built -upon area.
3. All the built -upon area associated with the project is graded such that the
runoff drains to the system.
4. All roof drains are located such that the runoff is directed into the system.
5. The outlet/bypass structure elevations are per the approved plan.
6. The outlet structure is located per the approved plans.
7. Trash rack is provided on the outlet/bypass structure.
8. All slopes are grassed with permanent vegetation.
9. Vegetated slopes are no steeper than 3:1.
.10. The inlets are located per the approved plans and do not cause short-
circuiting of the system.
11. The permitted amounts of surface area and/or volume have been
provided.
.12. Required drawdown devices are correctly sized per the approved plans.
_13. All required design depths are provided.
.14. All required parts of the system are provided, such as a vegetated shelf,
and a forebay.

Per the request for additional information and clarification and our telephone conversation
with Mr. Rob Gordon resulting from his last review of the referenced project, we are
submitting revised sheets for slip sheeting into the Plans. Kindly remove and replace Sheets
C-130 and C-530 from the two sets of previously submitted Armory Plans and insert the
herein provided plans in their place. Kindly remove and replace the Application Form and the
Wet Detention Basin Form in the previous Application and Design packages for the site,
AFTER removing the original signature sheets for the. Application Form and the Wet
Detention Basin Supplement. We are resubmitting two sets of the Permit Application and
Design Package for the site. Please insert the original signature sheets (page 4 of the
Application and page 4 of the Wet Detention Basin Supplement) into the new packages.
Also please remove the BMP Design calculations and Pre and Post Drainage Maps in the
previously submitted sets and replace with the new sheets.
We have addressed and coordinated the review comments from Mr. Gordon in this submittal
based on our telephone conversation with him. The following responses are our
understanding of the results of that conversation:
a. The 1" rain event rise is elevation 14.90 and is used only to calculate the orifice drawdown
as required by the regulations. The 15.40 elevation is the top of the outfall structure,
determined by physical structure dimensions and is the elevation used in relation to the
temporary pool volume in all calculations.
b. The overall site is 5.28 acres. 4.36 acres of the project drain to the BMP while the
remaining 0.92 acres bypasses the BMP and continues on downstream. The additional
information regarding the bypass area has been added to the application form and the
drainage area numbers have been revised accordingly.
c. The square footage for streets draining into the BMP was initially shown as part of the
parking square footage. It has been shown separately for clarification and all numbers
verified accordingly.

The number has been revised without the BMP, showing a new percentage of 48.1%.
e. The impervious areas between the application and the supplement have been verified and
revised as needed.
f. The 1" temporary pool volume in the supplement and calculation has been revised based
on the structure elevation of 15.40.
g. The permanent pool volume has been revised and verified on the supplement and
calculations with the stage -storage chart based on the structure elevation.
h. The forebay volume has been revised and verified with the stage -storage chart.
i. The numbers for Section 111 (4) of the supplement have been revised as directed. Round -
off problems have been corrected
j. The orifice diameter and design rainfall have been revised in the supplement.
We anticipate that these responses are sufficient for your prompt approval of this project.
The additional Resubmittal Fee of $1,000 is submitted herewith.

The Wilmington Regional Office received a Stormwater Management Permit Application for
FSRG Operations & Maintenance Facility P-1148 — 2nd LAR Armory on March 1, 2006. A
preliminary review of that information has determined that the application is not complete. The
following information is needed to continue the stormwater review:
There are still discrepancies/inconsistencies in figures listed between the application,
narrative, plans and calculations. Several have been Wed below; but please verify all
elevations, volumes, surface areas and other figure'§Xtsted in the application package.

Additional fees, not to exceed 50% of the original permit application fee, may be
charged for subsequent reviews due to the insufficiency of a permit application, as
specified by Section 11.4A of S.L. 2003-284.

=> This application is not consistent with the expectations and standards of the
express review program. The last Request for Additional Information dated
February 21, 2006 required all items to be complete upon resubmittal or.the
project would be removed from the express program. Because the application
still has several inconsistencies/ deficiencies, a resubmittal fee of $1,000 will be
required in order for this project to remain in the express program.
Please note that this request for additional information is in response to a preliminary review.
The requested information should be received by this Office prior to April 6, 2006, or the
application will be returned as incomplete. The return of a project will necessitate resubmittal of

preliminary review of that information has determined that the application is not complete. The
following information is needed to continue the stormwater review.
1. There are many discrepancies in figures between the application, supplement and
calculations. Please make sure figures on these three documents are consistent.
2. The temporary pool volume is the pond volume from the permanent pool to the spillway
outlet structure. It does not include the permanent pool volume. Please correct on the
supplement.
3. Please show an actual drawdown alc/drawdown
A 1" orifice appears to draw the 1"
storage volume down in >5 days. orifice should draw the 1" volume in 2-
5 days.
4. On the supplement, the sediment cleanout elevations are incorrect. The 75125 ration
appears to be reversed. Also note that the sediment cleanout elevation in the forebay
cannot be above the designated inlet pipe invert elevation.
5. The supplement AA`o'es not contain an original signature of the applicant.
6. The (immediate) dr�age area appears to be Cogdels Creek, not the New River.
Please correct.
7. Please convert section III (6) f e application to square feet, not acres. Please note
that the pond itself is not cons dered impervious.
8. Please dimension the entire permanent pool, not just the main bay on the drawings.
9. The permanent pool (NWL) should be drawn at the bottom of the invert pipe (on the
outlet structure detail), not the top.
10. It is not clear what the temporary pool elevation is. The 1" rise on the BMP cross-
section is 14.9, the spillway elevation on the outlet structure is 15.5 and the supplement
lists the temporary pool at 15.4. Please clarify.

Hi guys.
There appears to be some confusion about the Camp Lejeune projects. Fred received
Rob's add info request of January 18 and the subject refers to: * P-1151 Building
Addition to O&M Bldg.* with a* SW8 051065* permit number. This letter was your list
of 20 items and indicates that a resubmittal fee of $1000 is required because we held
the 1st meetings with Myra and then Fred has been down as well. I saw the copy of
this in the file here.
The r� cblem_ is that this /site name/ is listed as PAUL'S project in your SW
spreadsheet but with an *SW8 991204 * permit number and Paul has put down in the
spreadsheet that he issued this plan revision on January 18, presumably with,a copy
to Myra because Fred says P-1151 is not his project.
So, assuming that this is correct, the proper site name for the add info request Rob
sent to Fred should have been: *P1148 - 2nd LAR Armory, **FSRG Operations &
Maintenance Facilities, SW8 051065.
*As an aside, Fred indicated that he will be putting the other two P-1149 projects in
the mail to us soon for those reviews, as well.

1. It appears page 2 of the application was for a different Camp Lejuene project. All site
data appears incorrect. Please resubmit.
2. The top portion of the entire application (not the supplement) was cut off (bad copy?).
Please resubmit a complete application.
3. Please include the applicants title on section 1 (2) of the application.
4. Please use square feet on Section 111 (6) of the application, not acres.
5. Please submit 2 drawing sets, only one was received.
6. The calculations must be PE stamped and signed.
7. Please hand initial section 11 of the wet detention supplement.
8. The sediment removal elevations listed in the operation & maintenance section of the
wet detention supplement are incorrect.
9. The applicant must sign both the application and supplement. The supplement (page 4)
was not signed by the applicant.
10. A project area must be defined and dimensioned on the plans. This area must
correspond to the project area listed on the application. For an area (such as Camp
Lejuene) where there is no defined parcel, any area may be used as long as it contains
all proposed improvements. Please note this area will provide the distinction between
on -site and off -site drainage area in section III (6) of the application.
11. The proposed impervious area listed on the application, supplement and calculations
are not consistent.

12. Please show the dimensions of the permanent pool for the proposed pond.
13. The wet detention pond is required to have a 3:1 length:width ratio. Only about 2:1 is
provided. The flow path of the pond may be aincreased by reconfiguring the pond,
specifying baffles or providing a weir in the forebay (or any combination of the above).
14. The elevations listed in the project narrative are not consistent with the drawings,
calculations or the supplement.
15. The 1" required storage volume appears incorrect (the runoff coefficient appears off a
decimal). Please recalculation the runoff volume.
16. Please provide the following stage -storage information for the pond: forebay (from
bottom to permanent pool) & the entire pond (from the bottom to the 1" storage
elevation).
17. Please show the forebay berm elevation on the detail.
18. Please add a note specifying all roof drains from the building will be directed to the
permitted wet detention pond.
19. Please provide detail of the proposed trash rack. It must have a max opening size of 4".
20. The bottom elevations listed on the two pond details are not consistent.

Good morning! I apologize that it has taken this long to get this information to you. I have been trying to
find out about the wetland information and who would be the applicant for the Marines. I have filled out the
forms as per my phone conversation with Bobby Canady of Camp Lejuene's ROICC office this morning. I
have attached 4 different PDF's for the different projects.
I have been in contact with Dean Hunkley regarding the DENR Sewer extension permits. He stated that I
needed to contact Marie Doklovic in the Raleigh office. I have left a message with her yesterday. Have
not heard back as of this time.
Dean also stated that I should contact Debra Benoy regarding the DENR Water extension permits. I have
left a message with her yesterday. Have not heard back as of this time.
Please let me know what time you have set the meeting for gesday, Septemb 7 My flight gets in

Good afternoon! I have spoken Paul Bartlett on September 15, 2005, regarding the NCDENR process and
requirements in brief. We are interested in doing the express permitting for our projects. Please forward
the forms required for me to start that process. I will be in Jacksonville, NC Tuesday, September 27, 2005
and would like to schedule the pre -application meeting with you early afternoon. Please let me know what
time will be acceptable with you.
I have attached projectlocations. pdf with the project location topo maps, aerials, and proposed plans.
Even though we are under one contract, we are required to submit each site as a separate permitting
package regarding stormwater. Below is a brief description of each project...
P-1148
P-1148 will be 2nd LAR Armory building with associated parking located in a partially cleared area. The
cleared area contains tennis courts and a shelter, both to be removed. A small portion of the area is
treed. There are preliminary plans for a BMP, which will discharge into an existing swale to the south of
the project area.
P-1149 ;,
P-1149 contains two different buildings and two different building locations.
The first building will be 1BN/9 Armory building with associated parking located in a grassed area between
existing buildings. This area has existing stormwater inlets, which we had planned to discharge directly
into the inlets. I'm not sure is there is an existing permit for this area or not.
The second building will be a VMF building with associated parking located in a grassed area that is
currently a ballpark and obstacle course. There are preliminary plans for a BMP, which will discharge into
the existing roadway swale to the west of the project area.
P-1150
P-1150 will be a small addition to building A66. This building addition will be within an impervious area.
Looking at the record drawings, I believe this area has been permitted but I'm not for certain.
Please review the attached, forward me the required forms, and a time to meet with you on the 27th. I do
greatly appreciate your time and consideration in the matter and look forward to meeting/working with you

toward 'C' Street. Existing features to be demolished under this contract include a
ballfield with pole -mounted lighting, a judo pit, and an obstacle course. The
Contractor shall coordinate with the Camp Lejeune Facilities Planning Department to
determine disposition of existing judo pit and obstacle course equipment. The site i5
bordered closely to the rear by a natural, wooded area leading to a deep ravine and

The 1" BN/9 Armory shall be constructed on a clear, near -flat site located in th,
middle of an enlisted personnel barracks complex off of 'C' Street in the Hadnot Poin
area, as shown on the RFP drawings. This site is surrounded by four existing three
story barracks. There are few significant features of site demolition; however, th,
Contractor must create a vehicle entrance to the site at an existing 90-degree turn it
the existing driveway. This area is encumbered by a large number of POV's ant
pedestrians. Existing underground steamlines also cross this site. It shall

This office has received a soil erosion and sediment control plan for the project listed above which
was submitted as required by the North Carolina Sedimentation Pollution Control Act (G.S. 113A-57 (4)). The
Act requires that all persons disturbing an area of one or more acres of land must obtain approval of a soil
erosion control plan prior to the commencement of the land -disturbing activity (G.S. 113A-54(d) (4)).
The review period for an express review is three days of receipt of a complete plan. If additional
information is requested, it must be submitted within five working days of receipt.
Commencement or continuation of a land -disturbing activity under the jurisdiction of this Act prior to
the approval of an erosion and sediment control plan is a violation of the Act.
We have' performed a preliminary review of your submitted plan and have determined that additional
information will be necessary before a final review can be completed. Enclosed is a checklist of items that
must be submitted_ before we can complete the review process. Failure to provide the additional information
may result in disapproval of your plan.
The approval of an erosion and sediment control plan is conditioned on the applicant's compliance
