THE CONTRACTOR SHALL VISIT THE SITE TO INFORM AND SATISFY HIMSELF OF ALL EXISTING CONDITIONS PRIOR TO SUBMITTING HIS BID. ANY DISCREPANCY BETWEEN THE SITE AND THOSE CONDITIONS REPRESENTED ON THE DRAWINGS SHALL BE BROUGHT TO THE ATTENTION OF THE OWNER AND THE ENGINEER IMMEDIATELY. 2. ALL EXISTING UTILITIES AND/OR STRUCTURES INDICATED ON THE DRAWINGS ARE BASED ON A FIELD SURVEY. THE EXISTING IMPROVEMENTS SHOWN ON THESE DRAWINGS MAY NOT BE ALL-INCLUSIVE. THE CONTRACTOR SHALL TAKE PRECAUTIONARY MEASURES TO PROTECT THE UTILITY LINES AND OTHER IMPROVEMENTS SHOWN HEREON AND ANY OTHER EXISTING LINES NOT OF RECORD OR NOT SHOWN ON THESE PLANS, UNLESS THEY ARE SHOWN TO BE REMOVED OR RELOCATED. THE CONTRACTOR SHALL BE RESPONSIBLE FOR MAKING ARRANGEMENTS FOR THE PROTECTION, RELOCATION OR REMOVAL OF CONFLICTING EXISTING UTILITIES AND SHALL BE FULLY RESPONSIBLE FOR ANY AND ALL DAMAGES RESULTING FROM HIS FAILURE TO LOCATE AND PRESERVE ANY AND ALL UNDERGROUND UTILITIES. BEFORE BEGINNING WORK THE CONTRACTOR SHALL CONTACT THE N.C. ONE -CALL CENTER AT 811 TO HAVE ALL EXISTING UTILITIES LOCATED. 3. THE LOCATION AND ELEVATIONS OF IMPROVEMENTS, TO BE MET OR CROSSED BY WORK, SHALL BE CONFIRMED BY FIELD MEASUREMENT, SUFFICIENTLY AHEAD OF CONSTRUCTION TO PERMIT REVISIONS TO THE PLANS, IF REVISIONS ARE NECESSARY DUE TO ACTUAL LOCATION OF EXISTING UTILITIES. 4. THE CONTRACTOR SHALL ASSUME SOLE AND COMPLETE RESPONSIBILITY FOR JOB SITE CONDITIONS DURING THE COURSE OF CONSTRUCTION OF THIS PROJECT, INCLUDING SAFETY OF ALL PERSONS AND PROPERTY. THIS REQUIREMENT SHALL APPLY CONTINUOUSLY AND NOT BE LIMITED TO NORMAL WORKING HOURS. THE CONTRACTOR SHALL DEFEND, INDEMNIFY, AND HOLD THE OWNER AND THE ENGINEER HARMLESS FROM ANY AND ALL LIABILITY, REAL OR ALLEGED IN CONNECTION WITH PERFORMANCE OF WORK ON THIS PROJECT, EXCEPTING FOR LIABILITY ARISING FROM THE SOLE NEGLIGENCE OF THE OWNER OR THE ENGINEER. 5. THE CONTRACTOR SHALL VERIFY THAT ALL REQUIRED PERMITS AND APPROVALS HAVE BEEN OBTAINED FROM THE APPROPRIATE AUTHORITIES BEFORE BEGINNING WORK ON THE PROJECT. APPROVED PLANS, APPROVAL LETTERS OR OTHER CERTIFICATES OF APPROVAL SHALL BE MAINTAINED ON THE SITE AT ALL TIMES DURING CONSTRUCTION. 6. CLEARING AND GRUBBING DEBRIS SHALL BE DISPOSED OF OFFSITE IN A PERMITTED LANDFILL FACILITY. DISPOSAL ON THE PIPELINE EASEMENT OR IN OTHER LOCATIONS IS NOT ALLOWED. 7. GAS MAIN INSTALLATION SHALL BE BY OPEN CUT UNLESS SHOWN OTHERWISE ON THE DRAWINGS. THE ENTIRE WIDTH OF THE RIGHTS -OF -WAY, EASEMENTS AND WORK SPACES SHALL BE CLEARED AND GRUBBED UNLESS SHOWN OTHERWISE ON THE DRAWINGS. ALL CLEARING DEBRIS - STUMPS, TREES, TOPS, BRUSH, ETC. SHALL BE REMOVED FROM THE SITE AND DISPOSED OF IN A LEGAL MANNER. 8. AREAS SHOWN AS ' TREE CUTTING ONLY - NO GRUBBING' SHALL BE CUT AT NO MORE THAN 12 INCHES ABOVE THE GROUND. STONE SHALL BE PLACED ON NON -WOVEN GEOTEXTILE FABRIC. 11.IF FENCING REMOVAL AND REPLACEMENT IS REQUIRED FOR INSTALLATION OF THE GAS MAIN, THE CONTRACTOR SHALL PROTECT REMOVED FENCING MATERIAL DURING CONSTRUCTION. SILT FENCE OUTLET DEVICES ARE SHOWN AT THE LOW SPOTS BASED ON THE ELEVATION CONTOURS. SHIFT THESE OUTLET DEVICES AS NEEDED IN THE FIELD TO LOCATE THEM IN THE ACTUAL LOW SPOTS. 9. ALL DISTURBED AREAS SHALL BE SEEDED AND MULCHED IN ACCORDANCE WITH THE SEEDING SCHEDULE WITH LIMITATIONS AS FOLLOWS. GIVEN THE NATURE OF THE PROJECT, TEMPORARY AND PERMANENT SEEDING MIXTURES HAVE BEEN COMBINED AND SHALL BE APPLIED IN A SINGLE OPERATION. 10. ALL EROSION AND SEDIMENT CONTROL DEVICES SHALL REMAIN IN PLACE UNTIL ALL DISTURBED AREAS ARE REVEGETATED AND THE EROSION CONTROL INSPECTOR APPROVES THE REMOVAL OF THE DEVICES. ALL TEMPORARY EROSION AND SEDIMENT CONTROL DEVICES SHALL BE REMOVED UPON APPROVAL BY THE EROSION CONTROL INSPECTOR. 11. THE CONTRACTOR IS RESPONSIBLE FOR INSTALLING ALL MEASURES AND DEVICES SHOWN ON THE PLANS, AND FOR COMPLYING WITH THE PROVISIONS AND CONDITIONS OF THE PROJECT SPECIFIC EROSION CONTROL PERMIT. GRADING NOTES 1. ALL GRADING ACTIVITIES SHALL BE CONTAINED WITHIN THE LIMITS OF DISTURBANCE SHOWN ON THE PLANS. 2. ALL AREAS TO BE GRADED SHALL BE STRIPPED TO REMOVE ALL TOPSOIL, STUMPS, ROOTS, ORGANICS AND OTHER UNSUITABLE MATERIAL. AFTER STRIPPING, EXPOSED SUBGRADE IN BUILDING AND PAVEMENT AREAS TO BE FILLED SHALL BE PROOF ROLLED IN THE PRESENCE OF THE GEOTECHNICAL ENGINEER. IF UNSTABLE AREAS ARE ENCOUNTERED THAT ARE THE RESULT OF WET SOIL CONDITIONS, REASONABLE EFFORT SHALL BE MADE TO MECHANICALLY DRY THOSE SOILS. IF UNSUITABLE SOILS ARE ENCOUNTERED THEY SHALL BE UNDERCUT TO FIRM MATERIALS AS DIRECTED BY THE GEOTECHNICAL ENGINEER, AND BACKFILLED WITH CONTROLLED FILL. REASONABLE DRYING EFFORT AND UNSUITABLE SOILS ARE DEFINED IN THE EARTHWORK SPECIFICATIONS. 3. CONTROLLED FILL SHALL BE BENCHED INTO THE SIDES OF SLOPES STEEPER THAN 4:1 (H:V) TO IMPROVE THE BOND BETWEEN THE FILL AND THE UNDISTURBED SOIL. EACH BENCH CUT INTO A SLOPE SHOULD BE AT LEAST 12 INCHES BUT NOT MORE THAN 60 INCHES HIGH. THE BENCHES SHALL BE WIDE ENOUGH TO ALLOW OPERATION OF THE COMPACTION EQUIPMENT WITHIN THE BENCHED AREA SO THAT FILL CAN BE PLACED ON THE BENCH AND OVER THE REST OF THE FILL IN CONTINUOUS LAYERS. 4. FILL SLOPES SHALL BE CONSTRUCTED BY PLACING CONTROLLED FILL IN HORIZONTAL LIFTS WHICH EXTEND SLIGHTLY BEYOND THE SLOPE FACE. EACH LIFT OF FILL SHALL BE COMPACTED TO THE SPECIFIED MINIMUM DENSITY BEFORE PLACING THE NEXT LIFT. THE COMPACTION EQUIPMENT SHOULD TRAVEL PARALLEL TO THE SLOPE FACE TO ENSURE PROPER COMPACTION. ONCE THE SLOPE IS COMPLETED IT SHALL BE TRACKED WITH A DOZER TO MINIMIZE THE POTENTIAL FOR EROSION. 5. CONTROLLED FILL FOR STRUCTURAL AREAS SHALL BE COMPACTED TO 95% OF THE STANDARD PROCTOR MAXIMUM DRY DENSITY (ASTM - D698), UNLESS OTHERWISE NOTED ON THE PLANS, IN THE SPECIFICATIONS, OR GEOTECHNICAL REPORT. FILL UNDER BUILDINGS AND PAVED AREA SHALL BE COMPACTED TO 98% STANDARD PROCTOR IN THE TOP 12" OF SUBGRADE. COMPACTION TESTS SHALL BE TAKEN TO SUBGRADE AND MINIMUM 15 FEET BEYOND LIMITS OF PAVED AREAS OR BUILDING FOOTPRINTS AREAS. FREQUENCY OF COMPACTION TESTING SHALL BE DONE IN ACCORDANCE WITH THE WRITTEN SPECIFICATIONS. 6. PERMANENT CUT AND FILL SLOPES SHOWN TO RECEIVE EROSION CONTROL MATTING SHALL BE TRACKED WITH A DOZER PRIOR TO INSTALLING THE MATTING. STORM DRAINAGE NOTES NOTES 1. ALL STORM DRAINAGE CONSTRUCTION SHALL CONFORM TO NCDOT STANDARDS AND SPECIFICATIONS. 2. ALL STORM SEWER PIPE SHALL BE REINFORCED CONCRETE PIPE (RCP) (15" MIN. DIAMETER) UNLESS OTHERWISE NOTED. RCP SHALL BE MINIMUM CLASS III WITH BUTYL RUBBER MASTIC JOINTS UNLESS NOTED OTHERWISE ON THE PLANS. 3. HIGH DENSITY POLYETHYLENE (HDPE): PIPE AND FITTINGS SHALL COMPLY WITH THE LATEST EDITIONS OF AASHTO M252 (FOR TUBING), AND M294 (FOR PIPE). FILTRATION TUBE \\// FIBER FILTRATION TUBE CHECK DAM NTS Maintenance of Fiber Filtration Tubes To assure long term performance, Fiber Filtration Tubes (FFTs) installations must be properly maintained. By definition, these devices are designed to provide filtration and it is intuitive that a properly functioning filter will eventually need to be replaced. This is the case with the water, oil and air filters that we all use on a daily basis. The first maintenance activity for FFTs is to inspect the installation every two weeks and/or after each significant precipitation event (typically > %" of rainfall) as mandated by the US EPA NPDES regulations. When sediment accumulations reach'/2 of the FFT height the sediment should be removed to insure storage capacity for the next event. This may be accomplished using hand labor or a small bucket loader. Care must be taken to prevent damage to the FFTs. Replace all FFTs that display torn netting and/or excessive deformation. Unlike many sediment retention devices that simply act as barriers to slow water flow velocities, FFTs are also designed to capture sediment within their 3-D matrix. As filtered material accumulates the matrix will eventually begin to blind or clog resulting in reduced flow rates and increased turbidity of water exiting the FFT. The functional longevity of an FFT is dependent upon a variety of site specific environmental conditions including duration of installation, sediment composition, the presence/type of a flocculant and amount of water that has passed through the device. The functional longevity of an FFT must be determined by regularly scheduled field inspections and on site experience will lead to predictive results in a given environment. Fiber Filtration Tubes are designed to photo -biodegrade within two years. Depending upon application and long term land use the devices may be left in place or removed. FFTs will support long term plant growth in areas conducive to establishment of dense vegetation. If an FFT is removed there are at least three options for disposal and re -use. 1. The FFT may be re -used for additional sediment retention or storm water treatment applications if there is not excessive sediment loading. 2. The FFT may be cut open and the matrix may be rolled out to be used as a blanket if there is not excessive sediment loading. 3. The FFT may be hauled to a landfill if there are potentially undesirable constituents trapped within the matrix. To replace, carefully pull out the anchoring staples and stakes. Take care not to damage erosion control devices that have been installed in conjunction with FFTs. The staples and stakes may be reused if they demonstrate no significant damage and can be driven back into the ground. Remove the "spent" FFT and reshape the existing trench, if applicable. Smooth the soil surface and install the new FFTs following the published installation guidelines.
