Dear Mr. Green:
Following an inspection of a nearby facility, I noticed this site and observed its condition from
Center Street. Based on the fact it has fuel storage with a fueling station, parks a large amount of
equipment with potential for fuel/oil/fluid leaks, some maintenance is most likely performed, and
equipment is washed on -site; then this site qualifies for coverage under the North Carolina
Stormwater General NPDES permit (NCG080000).
By this letter, our office and the Division is requiring this facility's ownership to apply for coverage
under said permit. The application (NOI - Notice of Intent) -and other important documentation
about this permit can be found on the Division's NPbES Stormwater Permitting Unit's website at:

Should L&M Jacksonville Investments LLC fail to apply for coverage for this facility within 30 business
days following receipt of this letter it may result in an enforcement action whereby a civil penalty
may be assessed for operating without a permit. Enforcement action may be taken pursuant to
North Carolina General Statute 143-215.1 for operating without coverage under the North Carolina
Stormwater General NPDES permit (NCG080000) for this facility.
