PO Box 929 Monore,NC 28111 Subject: Preconstruction Conference Minutes Dear Mr. Timmons: A preconstruction conference was held at the project site located on SR 1203 in Northampton County, on Wednesday,January 27,2021 at 1:00 P.M. A list of the attendees is attached. Environmental Issues: Kyle Barnes reviewed the Army Corps of Engineers Permit. Garcy Ward reviewed the 401 Water Quality Certification.Andy Blankenship and Paul Williams discussed erosion control issues. The contractor will use a commercial site as the borrow source and a commercial site as the waste site if needed. The contractor should notify the Army Corps of Engineers of any endangered species or historical properties that may be encountered. All equipment shall be kept maintained and care should be taken to avoid contamination of stream waters from fluids, fuels, etc. No staging of equipment of any kind is permitted in waters of the U.S. or protected riparian buffers. The contractor will maintain NPDES inspection records and immediately report any violations to the Engineer,who will notify DWR within 24 hrs. of becoming aware of any violations. The completion date for this project ICT 1 is October 15,2021. Direct contact between curing concrete and stream water shall be avoided. Highly visible fencing shall be placed at the outer wetland boundaries before clearing begins, and silt fence should be installed at the toe of slopes. Only coir fiber matting is allowed in riparian areas. The use of matting which contains a nylon mesh grid is prohibited. A turbidity curtain will be required if driving or drilling activities occur within the stream channel, on the stream bank, or within 5 feet of the top of bank. Concrete washout structures will be required on this project. Brooks Braswell discussed contract and plan issues. Contract Issues: The date of availability for this project is January 25, 2021 and completion date is April 13,2022. Liquidated damages are $200.00 per calendar day. ICT1 except for work under Project Special Provisions entitled Planting,Reforestation and Permanent Vegetation Establishment the contract completion date for ICT1 is October 15, 2021. The Liquidated Damages for intermediate contract time are $750.00 per calendar day. Brooks discussed the Major and Specialty items in the contract and the base index price for fuel. Final surface testing is not required on this project. Clearing on this project shall be performed to the limits established by Method "II" shown on Standard Drawing No. 200.02 of the 2018 Roadway Standard Drawings. The monthly estimate has been set on the 15`h of each month. Subcontract approval forms, along with copies of the subcontracts were requested. Brooks requested additional area need for construction of slopes on Detour Bridge for permit modification of additional temporary fill in wetlands. Brooks discussed the relocation of Century Links utilities and advised Century Link of constructability issues until such time relocation occurs. This completes the minutes of the Preconstruction Conference for Contract C204530 held on Wednesday, January 27, 2021. Any discrepancies with the contents of these minutes should be submitted in writing to this office.Failure to do so will be an indication of agreement. Sincerely, Edwin W.
