Dear Mr Melton
On February 3 2012 the North Carolina Division of Water Quality (Division) received a copy of your application for a
401 General Water Quality Certification install a fill in streams at the future Autism Classroom Building site in
Greensboro NC in Guilford County Additional information is required in order that we may determine your project s
compliance with 15A NCAC 2H 0500 The Division will require this information in order to process your application to
impact protected wetlands and /or streams on the subject property Therefore we will place this project on hold as
incomplete until we receive this additional information and if we do not receive the additional information requested
below we will have to move toward denial of your application as required by 15A NCAC 2H 0506 Please provide the
following information so that we may continue to review your project
1 During a site visit on February 22 2012 a possible wetland area was noted Follow up investigation is necessary
to determine if this area will be considered a jurisdictional wetland by the US Army Corp of Engineers Please
provide information regarding their determination
2 Please provide a more detailed analysis of avoidance and minimization for this project It appears that the
classroom could be accessed via existing driveways/bus lanes It also appears that the access road could be
relocated to cross only one stream downstream of both current locations
3 Please provide a plan sheet that indicates the location of proposed stormwater measures for this project
4 Please provide documentation that the Jordan Lake Riparian Buffer rules adopted by the City of Greensboro
Ordinance have been satisfied
5 Please provide a plan sheet of the impact area that clearly identifies the location of temporary impacts and
permanent impacts separately Please also provide a detail for dewatering techniques and stream restoration in the
areas of temporary impacts
6 Please provide a detail showing the proper installation (below the streambed elevation) of the riprap pad at the
culvert outlets

Certification Unit 1650 Mail Service Center Raleigh NC 27699 1650 If you will not be able to provide the requested
information within that timeframe please provide written confirmation that you intend to provide the requested
information and include a specific timetable delineating when the requested materials will be provided If we do not hear
from you in 30 calendar days we will assume that you no longer want to pursue this project and we will consider the
project as returned
This letter only addresses the application review and does not authorize any impacts to wetlands waters or protected
buffers Please be aware that any impacts requested within your application are not authorized (at this time) by the
