On January 27 2012 the Division of Water Quality (DWQ) received your application dated
January 27 2012 to impact 0 07 acres of jurisdictional ditch to construct the proposed Umhealth
Post Acute Care facility The DWQ has determined that your application was incomplete and/or
provided inaccurate information as discussed below The DWQ will require additional
information in order to process your application to impact protected wetlands and/or streams on
the subject property Therefore unless we receive the additional information requested below
we will have to move toward denial of your application as required by 15A NCAC 2H 0506 and
will place this project on hold as incomplete until we receive this additional information Please
provide the following information so that we may continue to review your project
Additional Information Requested
1 Please relocate the two inlets to Proposed Wetland #1 so that they discharge directly to
the forebay rather than to the adjacent side slope This will mimiruze the risk of erosion
2 The DWQ strongly recommends removing the stilling basin device located between the
two wetlands and instead suggests that stormwater be piped through this area to minimize
erosion and maintenance issues

For the planting plan
a Please remove the plants that are specified in the deep pools where the outlet
devices are located This will help to avoid clogging issues
b Many of the instructions provided on Sheets L 01 and L 02 do not pertain to
stormwater wetlands Per the BMP Manual only a one time fertilizer application
to establish the plants is permitted Please provide appropriate instructions for
establishing the wetland plants (including watering specifications)
c Please include the grass species and instructions for preparation establishment
and management you wish to specify for the side slopes around the perimeters of
the stormwater wetlands
Please respond in writing within 30 calendar days of the date of this letter by sending a copy of
this information to Ms Annette Lucas of the DWQ Central Office Wetlands Buffers and
Stormwater Compliance and Permitting Unit at 1650 Mail Service Center Raleigh NC 27699
1650 If you will not be able to provide the requested information within that timeframe please
provide written confirmation that you intend to provide the requested information and include a
specific timetable delineating when the requested materials will be provided If we do not hear
from you in 30 calendar days we will assume that you no longer want to pursue this project and
we will consider the project as returned
This letter only addresses the application review and does not authorize any impacts to wetlands
waters or protected buffers Please be aware that any impacts requested within your application
are not authorized (at this time) by the DWQ Please call Ms Annette Lucas at (919) 807 6381 or
