The NC Division of Water Quality has reviewed your submittal for a 401 Water Quality
Certification for the aforementioned project Review of your application revealed it lacking
necessary information required for making an informed permit decision The permit application
was deficient in the following areas
• No mitigation was proposed DWQ requires mitigation on impacts equal to or exceeding 150
feet per stream for both intermittent and perennial streams on linear transportation projects
Please refer to page 17 of the PCN help file found at
http / /www saw usace army mil /WETLANDS /pcn/
• Streams shown on the plans are not included in the permit application These include Sta
45 +78 91 and potentially Sta 40 +03 Please include all impacts to perennial and intermittent
streams in the PCN and clearly document the point of jurisdiction on the plans
• Based on the plans there are channel relocations at the pipe outlets for both permit sites
Please document on how the channels will be re routed and stabilized
• Please provide a more detailed stormwater management plan Please indicate where
stormwater will and won t be treated And if no treatment is possible please explain why it
is not possible Please also show construction details for all stormwater treatment BMPs and
document that these are designed in accordance with the most recent version of the NCDOT

Please note that the Division has scheduled a field visit on Nov 13 and additional concerns may
arise that need to be addressed
Therefore, pursuant to 15A NCAC 2H 0507(a)(5) we will have to place the permit application
on hold until we are supplied the necessary information You have 30 days to respond in writing
with the requested information or notification to this office that the information is forthcoming
If at the end of the 21 days this office has not received this information in writing we will
assume you are withdrawing your application and it will be returned Furthermore until the
