Interim inspections have been conducted in conjunction with monthly progress meetings scheduled for the 2nd Tuesdays each month,at 2:00 p.m. throughout the project. Location of the Progress meetings have been at the Conover Public Works Building at 939 4`h St. SW. It is anticipated that the Final inspection will be conducted on July 13. Sewer Rehab & Replacement Interim Inspection of 2W 370 436-05 June 8, 2010 GENERAL CONCERNS / RECOMMENDATIONS / ACTION ITEMS Please note UPDATES on Comments and Concerns noted in previous progress meetings (for reference) are italicized. 5..:In April Atlantic, Coast lContractors, (AC) requested final, inspectionon work:done,,Engineer stated this might occur week ending April 23 or shortly thereafter. As of the May meeting that didn't occur, but today June 8 all concurred that a final inspection can be scheduled for July 13. 8. Remaining work as of June 8 consists of a pipe burst at/near Post Office which will be done the last week in June, dig up of 3 services to install saddles- to be done week of June 14 and miscellaneous cleanup. 9. CG&L will provide closeout checklist via email. CG&L at Final inspection will go over applicable items on that checklist and then do spot observations of SS improvements after the meeting. CG&L will then provide Final inspection report with items needed to closeout the project. At the Final, please have all Buy American documentation, Davis Bacon interviews and Certified payroll records- available for review. LOAN DISBURSEMENTS 2W370 436-05 The last reimbursement for $208,858 (#9) was June 1. Total construction contract amount is $1,390,562.25. Project is — 99 %+ complete in CG&L's opinion. DESCRIPTION of ELIGIBLE WORK by Atlantic Coast Contractors (AC) : Cleaning and TV investigation of approximately 18,500 linear feet of sewer pipe and service laterals, numerous point repairs, some line replacement, and manhole rehabilitation. There were additions and deletions to original Scope via 4 change orders. Inspectors logs/Work Orders were observed earlier and at today's meeting and deemed adequate. Engineer emphasized to Atlantic Coast Constructors, Inc. (AC) need to dress up areas where work is completed. 2. Engineer noted there were 14 rain days last month. AC should include request for such time in next change order if needed. AC has stated they feel they will be complete in less than 2 months thus before the contract date of July 13, 2010. 3. Engineer, AC, and Owner stated they need to evaluate NE butfall MH#57 to MH#13 during a rain event to determine source of silt and apparent high inflows. 4. MH #42 observed to have evidence of overflow. All went to site and evaluated this MH and environs to determine what should be done.' 5. There exists — 2000 lin. ft. on the outfall run of MH#204 through #209 where the line changes -- size- from -15"-to-18"--to 12 CG&Urequests this matterr and possib- e resolution as well as MH##42 be discussed as to resolution at next meeting. CG&L recognizes that remaining funds in this contract may not be adequate to address all outstanding issues. 6. AC is requesting final inspection on work done. 7. Testing still needs to be done on line segments. 8. The cured in place pipe (CIPP) installation resumed. 9. Remaining work: a 2 MHs near City Hall b . — 10 point repairs c . 90 lineal feet SS to relay d . — 1000 lin. ft. pipe burst, crew is due last week in March 10. The efficacy of the injection grouting in manholes was discussed. CG&L was concerned about long term efficacy and several of these manholes should be inspected at final. CMT used an injectable grout. Conover WWTP Operator mentioned at this meeting that most recent 2-inch rain event (which is significant) showed a noticeably reduced wastewater flow spike than before this project. 2. Leah Martin, WP COG, conducted Davis -Bacon interviews in October and CG&L observed those forms on this date. It appeared contractor was in compliance with Davis -Bacon labor requirements. ' 3. Question arose if tree removal in Right -of -Way would be eligible. Upon checking at CG&L office it was determined such work would not be eligible for funding. 4. Dec. Update: this work now completed. It was decided to pipe burst the segment of sewer at the police department. 5. Dec. Update: SUO and Sewer Rates were recently provided CG&L. Conover shall submit Sewer Use Ordinance (SUO) and Sewer rates schedule to CG&L Attention: Pam Whitley if it has not already done so. CONSTRUCTION INSPECTION Observations of Atlantic Coast Contractors construction and progress (AC) 1. Work Oorder #1 is nearing completion, with — 1000 LF remaining but should finish Aug. 14. 2. Work Order #2 was delivered to Conover today Aug. 11. It calls for addional closed circuit TV of sewers. 3. There are no outstanding issues but 2 property concerns trees on ROW on 6th St. NE and on NC 16 sewer is tied into basement. City will resolve these issues. CHANGE ORDER UPDATE — the nature of the work is discovering problems that are then identified for work repairs. Change orders after approval by Owner and Contractor should be submitted (1 original with original signatures) to Gerald Horton, CG&L. The inspection was conducted in conjunction with the monthly progress meetings normally scheduled for the first Thursday each month. However there has been some confusion on when. The meetings will be held at 9:00 a.m. (State Utilities) and 10:30 a.m. (Wharton-Smith)'for the 2 projects. Meeting place is in the Wharton Smith, Inc. construction office on site. The next meeting is scheduled for Thursday September 3, 2009. FM,SS & PS - State Utilities (SU) project: 1. Revised route of SS on sheet 15, for the record the work is done. 2. It was discussed at meeting the Erosion Control inspector had visited site and so it was emphasized that contractor should check erosion. control structures and apparatus after each rain event. 3. Owner advised SU, it may take 3 months, to get power into PS site. 4. Engineer is still reviewing sizing of generator set. Owner wants answer soon on the issue and resolution thereof. 5. SU requested alternate means to test the 36 inch sewer stating air test would be dangerous. After discussion, Engineer and Owner allowed infiltration testing as acceptable. 6. CG&L had earlier requested a schedule of values for this contract. Please provide SOV in electronic format for both contracts. WWTP — Wharton -Smith (WS) project: 1. Duke Power took responsibility and will resolve height/clearance issues of the power lines at entrance. 2. WS proposed use of a Volclay water stop.
