For clarity, do you need two notarized originals of the form submission or will two copies suffice. Our client is having the form notarized this morning but the only way for me to get it is via email. Hoping a copy will suffice. Steven, 1 Based on what you showed me, it looks like this will qualify as a minor modification (A change to the plans that does not change the amount of BUA or SCM size/design). If that is the case, please complete the minor modification form (https://files.nc.gov/ncdeq/Energy%20Mineral%20and%20Land%20Resources/Stormwater/State%20Stormwater%20Pe rmits/SSW%20Forms/SPF7-Permit-Minor-Mod-Form-20171005-FINAL.docx) and submit to our office (Address in my email signature). Tips for filling out the form: Part A - Fairly straightforward, most of this information can be found in the permit package that was sent out when the project was approved. Let me know if you need any of this information. Part B 1) Make sure there are two signed/notarized hard copies of the minor mod form in the submission. 2) A redline of the plans (Basically what you emailed me) will suffice for this (don’t forget, 2x hard copies). 3) Does not appear to be needed based on what you provided (can just put “N/a”). 4) Not needed (“N/a”). Part C - Fairly straightforward, this is your contact info. Part D - Fairly straightforward. You should use the same permittee that is listed on the permit (SW3191101). If this person has changed, let me know. Electronic files should be uploaded to Laserfiche (https://edocs.deq.nc.gov/Forms/SW_Project_Submittal) using the permit number (SW3191101). The review/approval of minor mods is usually pretty quick, and there is no fee. Typically, I’m able to approve these within a week or two of us receiving the hard copy submission. Send all suspicious email as an attachment to report.spam@nc.gov Jim, Due to a guy wire conflict and the 4-6 week timeframe for Duke to coordinate, our client and contactor has decided it best to re-align our approved stormwater conveyance within the conflict area. See attached for permit and proposed modifications. I need to know if the state will require any type of formal submittal for this field modification. We are preparing a revision to the plan sheet now for distribution to the Lincoln County inspector and planning. Please advise. We are trying to get this update addressed today as it is impacting site work. I am managing the construction administration on this project in the absence of Steve Farmartino who recently moved and left Bohler.
