Dear Ms Hair
The North Carolina Department of Transportation requests authorization to extend an existing
box culvert on US 74 Business (Marion Street) which is being done in conjunction with a turn
lane construction/road shoulder widening project in Cleveland County NCDOT proposes to
extend an 83 foot long 4'x 4' concrete box culvert 10 feet The extension will be constructed on
a skew to better align with the stream and to prevent downstream bank destabilization The
existing culvert outlet is at streambed grade therefore the extension will also be at streambed
grade and will not meet the permit depth specification Due to this condition, NCDOT
requests a waiver from the Regional Conditions 2(b) (e) permit depth specifications A
temporary cofferdam pumping system will be used to divert the stream around the construction
site as needed Area affected by temporary cofferdam system will be restored back to pre project
conditions The scope of the work involves an unnamed tributary to Buffalo Creek which is
classified as Class C Waters by the Division of Water Quality
This office has conducted a GIS and field survey for the project for historical sites and
threatened /endangered species sites No sites listed on the National Register of Historic Places
are located in the vicinity of the project nor did there appear to be any eligible structures The
federally listed species for Cleveland County (USFSW listed date September 22 2010) include
the Dwarf Flowered Heartleaf (Hexastylis naniora) National Heritage Program records
document the nearest Dwarf flowered heartleaf location approximately 14 miles east of the
proposed project area The heartleaf is usually found on dry to moist ravine slopes (usually north
facing and of the oak hickory pine forest type) and in boggy areas near springheads There is no
suitable habitat within the proposed project area and no species were observed No impact on
the species is anticipated
Hwy 74 Business is an urban arterial however the project is not located within '/z mile of a
critical watershed area Hazardous spill basins are not required Enclosed is the hazardous spill
basin checklist

The purpose of the proposed work is to extend a concrete box culvert to increase shoulder width to provide a turn lane
along section of Hwy 74 business for the safety of the travelling public

The extension will be places on a 10 degree angle to better align with the stream The existing culvert outlet is at
streambed elevation therefore the extension will also be at streambed elevation and will not meet the depth
specification A temporary cofferdam pumping system will be used to divert the stream around the construction site as
needed Area affected by temporary cofferdam system will be graded and restored back to pre project conditions All
Best Management Practices will be utilized during the course of the project The Project will be constructed using typical
heavy equipment (trackhoe backhoe etc )
