E. Flow Tracking/Acceptance Form (Form: FTSE 04-16) (If Applicable):

The flow acceptance indicated in form FTSE must not expire prior to permit issuance and must be dated less than one

Submittal of this application and form FTSE indicates that owner has adequate capacity and will not violate G.S. 14 -
2l 5.67(a).
Intergovernmental agreements or other contracts will not be accepted in lieu of a project -specific FTSE.
F. Site Maps (All Application Packages):

area and closest surface waters.

with the station. The Division will accept a letter signed by the applicant (see 15A NCAC 02T .0106(b)) or proposed
contractor, stating that "the portable power generation unit or portable, independently -powered pumping units, associated
appurtenances and personnel are available for distribution and operation of this pump station."
If the portable power source or pump is dedicated to multiple pump stations, an evaluation of all the pump
stations' storage capacities and the rotation schedule of the portable power source or pump, including travel
timeframes, shall be provided in the case of a multiple station power outage. (Required at time of certification)

Division of Wamr Resources INSTRUCTIONS FOR FORNI: FTA 04-16 d SUPPORTING DOCUMENTATION
This application is for sewer extensions involving gravity sewers, pump stations and for;e mains, or any combination that has been
certified by a professional engineer and the applicant that the project meets the requirements of 15A NCAC 02T and the Division's
Minimum Design Criteria and that plans, specifications and supporting documents have been prepared in accordance with, 1AA
NC:%C n2•r, 15A NCAC 02T .03011, Di%ision policies and good cn lincering practicess.
While no upfront engineering design documents are required for submittal, in accordance with ISA NCAC 02T .0305(b), design
documents must be prepared prior to submittal of a fast track permit application to the Division. This would include plans, design
calculations, and project specifications referenced in I iA NCACs? t .030; and the applicable minimum design criteria. These
documents shall be available upon request by the Division.
Projects that are deemed permitted (do not require a permit from the Division) are explained in 1 iA `CAC 02T.0303.
Projects not eligible for review -,is the fast tract: process (must be submitted for full technical review):

General — When submitting an application, please use the following Instructions as a checklist to order to ensure all required items
are submitted. Adherence to these instructions and checking the provided boxes will help produce a quicker review time and
reduce the amount of requested additional inforniation. Failure to submit all required items will necessitate additional
processing and review time, and may result in return of the application. Unless otherwise noted, the Applicant shall submit
one original and one copy of the application and supporting documentation.

RE: U-6243 - Holly Springs Road Widening Phase 2
Fast -Track Sewer Application for Town of Holly Springs
Mr. Robinson:
The Town of Holly Springs has retained the services of Kimley-Horn and Associates, Inc. to design
and permit wet utility relocations related to the U-6243 project in Wake County, NC. The proposed
relocations in the attached permit application include 3,994 linear feet of 20-inch sewer force main
(upsize from 16-inch), 140 linear feet of 16-inch sewer force main, 1,001 linear feet of 8-inch gravity
sewer main, 139 linear feet of 20-inch gravity sewer main, and 65 linear feet of 21-inch gravity sewer
main along new alignments that will accommodate the proposed roadway and drainage
improvements.
The existing sewer force main is in direct conflict with proposed storm drainage in multiple locations
and cover on the pipe would be reduced to less than minimum allowable cover in other locations by
the proposed roadway widening. The relocated sewer force main will be located within the proposed
roadway pavement in the NCDOT right-of-way connecting to the existing sewer force main.
The existing sanitary sewer gravity mains are also in direct conflict with the proposed storm drainage
and roadway improvements and will be relocated as the same size, but as P-401 lined ductile iron
where required by NCDOT. The relocated gravity mains will lie outside the proposed roadway
pavement where possible with perpendicular crossings and connections to existing sewers as
exceptions.
This project will create no additional flow to the system; the proposed relocations are required to
accommodate the construction of roadway and drainage improvements related to the project.
Kendra Parrish, PE, Executive Director of Utilities and Infrastructure Services, and Nate Harvey, PE,
Kimley-Horn and Associates Project Engineer, have certified the attached Fast -Track Application. It
is our understanding that a completed Flow Tracking/Acceptance form is not required, as no
additional flow is to be allocated with this permit.

Thanks Nate and Rachel.
I'm thinking the easiest way would just to be to issue a new permit with the new lines. If we do that, we technically
should revise the old permit to remove the lines from the old permit that will be in the new permit.... However, since
it's an older permit (2000), and it's not even in our database, I don't really see the value in that.
So unless anyone objects, we just plan to issue a new permit with the new lines it.

Thanks for your emails, hopefully I can shed some light.
• All of the sanitary work is relocation of existing facilities without new flows.
• The force main work is replacement of 4,134 LF of the existing 16" FM with 20" with a little 16" at the tie-ins.
• All gravity sewer work is size -on -size replacement, with a few material changes to ductile iron due to being
under NCDOT pavement.
o Our survey picked up the large diameter interceptor that runs parallel to Middle Creek as 21", matching
the Town's GIS. It's not outside the realm of possibility that the existing line was 30" and survey missed
the pipe size. We will work to verify as soon as possible.
0 20" Ductile was selected where a material change is required in order to match inside diameters with
the 21" PVC and because 21" DI is not a readily available size.
o If the large gravity sewer being relocated is actually 30", we intend to replace with 30".
Since the gravity sanitary sewer is simply replacement in -kind within the same ROW/easement, does it fall under
Permitting by Regulation 15A NCAC 02T .0303? If so, perhaps the cleanest approach is for us to revise the permit,
removing the gravity sewer and permit the new upsized force main under a new permit?
We are happy to pursue whichever approach is preferred. If helpful, we can set up a meeting to discuss at your

project had gone in as a new permit, and I believe on January 61h there was an email thread regarding switching it to a
mod. I'm not sure how many lines are being upsized or replaced and will let Nate comment on that portion, but based

We've started reviewing the application for this project.
The resubmitted application shows that it is modifying WQ0017170. That permit is not in our current database (BIPAS),
but I was able to locate a copy of it, it was issued back in 2000 (I've included a snippet of it below). The sizes of the lines
in the old permit don't match the sizes of the proposed line in the new permit.
The 2000 permit has 30", 15", 12" and 8" gravity and 16", 6" and 4" FM (see below).

So I guess my question is: are a lot of the lines that are being replaced also being upsized? Or is it possible that some
(many?) of the lines that are described in WQ0017170 are not the same that are being replaced by this project?
When a permit is modified, it rescinds the earlier permit, and the new permit that is issued still needs to include
everything that was in the original permit that will stay in operation. But we're trying to determine if we should actually
modify the old permit, or if would just make more sense to create a new permit that only includes the items in the new
application.

I hope you were able to enjoy the holidays and that 2021 is starting off on the right foot for you!
I have attached a sewer fast track permit application package for sanitary sewer and force main relocations related to
the U-6543 Holly Springs Road Phase 2 project in Holly Springs. All of the gravity sewer relocations are size -for -size but
some material changes were necessary due to NCDOT requirements. The force main relocation is being upsized in the
same material from 16-inches to 20-inches in anticipation of future conditions and to be consistent with other
relocations on this particular force main. No new flows are being added with this work. I will be standing by for any
questions or comments you may have.
We have the review fee check in hand and can overnight it to whatever address will nest expedite the review
process. I believe we previously send the review fee to 3800 Barrett Dr.
