Dear Ms Sullins
This letter is in response to your letter of December 19 2011 in which you requested necessary
information regarding the permit application for the subject pipe replacement
Bridge Maintenance Engineer Jeremy Warren and Division Environmental Officer Jerry Parker
met with Ms Amy Euliss on site and the following was discussed and agreed upon
1 A floodplain bench with a seal will be placed covering half of the width of the
structure on inlet end
2 A seal will be placed covering half of the width of the structure with no floodplain
bench on the outlet end due to the fire intake
3 A small rock ladder will be placed at the outlet end of the structure to prevent further
sediment loss
4 The fire intake will be noted on the plan
