Inspection Summary:
A reconnaissance inspection of the high -rate infiltration basins was conducted on January 15, 2021. The basin, located
on -site of the facility, was found to be 6 inches to 1 foot from breaching the berm. The freeboard gauge was not visible. The
basin, located in the neighborhood, was found to be 3-6 inches from breaching the berm. The freeboard gauge was not
properly installed and a exact measurement could not be read.
A review of the reported freeboard measurements on the NDARs from January to November2020 shows that one or both
basins were non -compliant for freeboard for all but 2 of those months. According to the current permit, W00030088, Section
III. 9-10 states:
III. Operation and Maintenance
9. Freeboard in the infiltration basins and 5-day upset pond shall not be less than two feet at any time. [15A NCAC
02T.0705(c)]
10. Gauges to monitor waste levels in the infiltration basins and 5-day upset pond shall be provided.

Infiltraion and maintenance of these basins are a concern to the Wilmington Regional Office. A Notice of Violation and Intent
to Access will be issued. Please respond within 10 business days with plans to address these issues.
