Dear Mr. Wolf:
In accordance with your application for a discharge permit received February 5, 2003 we are
forwarding herewith the subject Certificate of Coverage to discharge under the subject state - NPDES
general permit for petroleum contaminated groundwater remediation systems and one set of final
approved plans and specifications. Authorization is hereby granted for the construction and operation
of an oil/water separator, product storage tank, air stripper, two bag filters in series, two carbon'filters
in series, and all associated pumps, piping and controls with discharge of 14,400 gallons per day of
treated wastewater into an unnamed tributary of Concord Lake, Class WS IV waters in the Yadkin
River Basin. This Certificate of Coverage is issued pursuant to the requirements of North Carolina
General Statute 143-215.1 and the Memorandum of Agreement between North Carolina and the US
Environmental Protection Agency .dated May 9, 1994 (or as subsequently amended).
Based on the source of your contamination, specifically gasoline and diesel, effluent page A.
(3) applies to your facility and lists required effluent limitations and monitoring frequencies.
If any parts, measurement frequencies or sampling requirements contained in this permit are
unacceptable to you, you have the right to request an individual permit by submitting an individual.
permit application and letter requesting coverage under,an individual permit. Unless such demand is
made, this certificate of coverage shall be final and binding.
Please take notice that this certificate of coverage is not transferable except after notice to the
Division of Water Quality. The Division of Water Quality may require modification or revocation and
reissuance of the certificate 'of coverage in case of change of ownership or control of this discharge.
In the event that the facilities fail to perform satisfactorily, including the creation of nuisance
conditions, the Permittee shall take immediate corrective action, including those as may be required by
this Division, such as the construction of additional or replacement wastewater treatment or disposal
facilities.
The Mooresville Regional Office, telephone number [phone], shall be notified at least
forty-eight (48) hours in advance of operation of the installed facilities so that an in -place inspection can

be made. Such notification to the regional supervisor shall be made during the normal office hours from
8:00 a.m. until 5:00 p.m. on Monday through Friday, excluding State Holidays.
A copy of the approved plans and specifications shall be maintained on file by the Permittee for
the.life of the facility.
Failure to abide by the requirements contained in this Authorization to Construct may subject the
Permittee to an enforcement action by the Division of Water Quality'in accordance with North Carolina
General Statute 143-215.6A to 143-215.6C.
This permit does not affect the legal requirements to obtain other permits which may be required
by the Division of Water Quality or permits required by the Division of Land, Resources, the Coastal
Area Management Act or any other Federal or Local governmental permit that may be required.

Further review of the above mentioned permit has shown that there are some additional pieces of
information that need to be received before the permit can be issued.
Additional Information Required;
• A letter approving the discharge of water into the proposed drainage ditch needs to be provided from
either the DOT or the City of Kannapolis, whichever has jurisdiction over the ditch. -
• The air stripper efficiency calculations provided were performed for a different air stripper under
different operating conditions than those specified in.the application and written system description.
Also, the pollutant concentrations used were not consistent with this application. Provide air stripper
efficiencies that'are consistent with the equipment and conditions specified.
e The carbon usage data provided was for a system other than that described in .the package. Provide
appropriate carbon calculations for the described system.
Please get the above -mentioned information to my attention by August 15, 2003. If I have not received
the required information by that time the package will be' returned.
