North Carolina Administrative Code(15A NCAC 211.0105(e))requires that an application for permit renewal be filed at least
180 days prior to expiration of the current permit, To satisfy this requirement,your renewal packages should have been sent
to the Division postmarked no later than April 3,2006.
The Division has not received a renewal request your company's CoCs. This is a violation of the permit at Part D.B.
10,which states"Any per ittee that has not requested renewal at least 180 days prior o expiration...will subject the
permittee to enforcement procedures as provided in NCGS 143-215.6 and 33 USC 1251 et.seq.".
In order to prevent an assessment of civil penalties you must submit a completed permit application for each
permitted site(see enclosed foams)no later than June 19,2006. If some systems are inactive due to lack of trust fund
payments,but the remediation equipment is still on site,those CoCs should be renewed.
If all remediation discharge from any of your facilities has ceased and you wish to rescind those CoCs[or if you have

The facility listed above is covered under NPDES permit NCG510000 for discharge of remediated
groundwater. NCG510000 expires on September 30, 2006. Federal(40 CFR 122.41) and North Carolina(15A
NCAC 2H.0105(e))regulations require that permit renewal applications must be filed at least 180 days prior
to expiration of the current permit. If you have already mailed your renewal request,you may
disregard this notice.
To satisfy this requirement,the Division must receive your renewal request postmarked no later than
April 3. 2006. Failure to request renewal by this date may result in a civil assessment of at least$500.00.
Larger penalties may be assessed depending upon the delinquency of the request.
If any discharge from the remediation system will occur after September 30, 2006, your CoC must be
renewed. Discharge of wastewater without a valid permit would violate North Carolina General Statute 143-
215.1;unpermitted discharges of wastewater may be assessed civil penalties of up to$25,000 per day.
If you have halted remediation activity due to lack of trust fund money,but your site ranking from the
Aquifer Protection Section requires future remediation&discharge,your CoC must be renewed.

documented in your current permit in Part II. B. 14. Your annual fees for the period March 2007-February 2008 and March 2009-
February 2010 has not been paid. The amount owed is$180.00;a copy of the invoice previously sent by the Division's Budget Office
is attached.
Failure to pay the annual fee is grounds for revocation of your permit,as documented in part I. B. 13 and II. B. 14. This matter must be
promptly resolved.
This letter serves as final notice that the Division will refer the fee noted above to the North Carolina Attorney General's Office for
collection through the courts unless payment in the amount of$ 180.00 is received by January 10,2010.

Annual Permit Fee Overdue
This annual fee is required by the North Carolina Administrative Cade. It covers the administrative costs associated with your
permit. It is required of any person holding a permit for any time during the annual fee period,regardless of the facility's operating
status. Failure to pay the fee by the due date will subject the perrait to revocation. Operating without a valid permit is a violation
and is subject to a$10,000 per day fine. If the permit is revoked and you later decide a permit is needed,you must reapply,with the

(COC). This COC is reissued pursuant to the requirements of North Carolina General Statute 143-215.1 and the
Memorandum of Agreement between the state of North. Carolina and the U.S. Environmental Protection Agency,

In accordance with your application for permit renewal, the Division is forwarding the subject NPDES general
permit for petroleum contaminated groundwater remediation systems. This General Permit is issued pursuant to the
requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement between North
Carolina and the US Environmental Protection Agency dated December 6,1983 and as subsequently amended.
The Certificate of Coverage previously issued to your facility is now valid under the revised General Permit for
discharges associated with groundwater remediation systems (NCG510000 - issued August 22,1996). The new General
Permit is more comprehensive, in that it covers remediation of groundwater contaminated with heavier fuels (such as
diesel, fuel oil, and kerosene) as well as gasoline contamination. Please note that the permit has three effluent
limitations pages: The type of contamination at your site (gasoline only, heavy fuels only, or gasoline and heavy
fuels) will. determine the effluent limitations for your discharge.
If any parts, measurement frequencies or sampling requirements contained in this permit are unacceptable to you, you
have the right to request an individual permit by submitting an individual permit .application. Please take notice
that the previously issued Certificate of Coverage is not transferable except after notice to the Division of Water
Quality. The Division of Water Quality may require modification or revocation and reissuance of the Certificate of
Coverage.
This permit does not affect the legal -requirements to obtain.other permits which may be required by the Division of
Water Quality or permits required by the Division of Land Resources, the Division of Coastal Management, or any
other Federal or Local governmental permit that may be required.

Dear Mr. Edwards:
This letter serves as written notification for an exceedance of the effluent permit limit for
toluene at the referenced site. The exceedance was discovered -on -October 6, 1997 upon
review of laboratory analytical data from an effluent_.sample` collected on September 17, 1997.
The toluene level in the effluent sample was 36 parts per billion (ppb),~exceeding the permit
limit of 22 ppb. Shield notified -you on October 7, 1997 via telephone of the permit exceedance
and corrective action planned for this system. A copy of the laboratory analytical report is
attached.
Due to problems with the telemetry programming caused by a power outage, the system has
only discharged 119 gallons between 8/28/97 to 9/29/97. The activated carbon in the carbon
unit was changed out on October.2., .1997. Shield.,plans_.to..pressure..w.ash.th.e...shallow tray air -
stripper to increase the efficiency of the system.

Dear Mr. Edwards:
This letter serves as written notification for an exceedance of the effluent permit limit for
Toluene and Benzene at the referenced site. The exceedance was discovered on September 16,
1997 upon receipt of laboratory analytical data received on September 12, 1997 from an
effluent sample collected on August 28, 1997. The toluene level in the effluent sample was
220 ppb, exceeding the permit limit J 1 ppb.. The Benzene level was 180 ppb, exceeding the
permit limit 71.4 ppb. Shield notified Jim Reed at your office on September 16, 1997 via
telephone of the permit "exceedence and corrective action planned for this system. A copy of
the laboratory analytical report is attached.
A power outage to Remediation system caused a memory loss in the telemetry programming
which caused the system to shut down sometime between 8/28/97 to'9/11/97. Shield's
corrective action includes replacing the carbon in the carbon unit along with powerwashing the
shallow' tray airstripper. Shield will resample the effluent as soon as these corrective measures
are taken'.

.The�following'letter was written to request your -permission xb -amend the corrective action plan
(CAP) for the above referenced site. The amendments deal with the proposed air stripping unit
for groundwater remediation, an oil/water separator, and a change in the NPDES permit.
Proposed NPDES General Permit
Shield has contacted Mr. Charles Lowe of the North Carolina Division of Environmental
Management (NCDEM), Permits and Engineering Section regarding the proposed change from an
individual NPDES permit to a general permit. Currently, Shield is working toward the goal of
obtaining a general NPDES permit for the site.
Proposed New Effluent Discharge Point and Receiving Waters
Based on a review of the original CAP, the effluent discharge from the groundwater treatment
system was to be directed to an unnamed tributary of Bracketts Creek. Bracketts Creek is located "
south of the Sandy Mush facility and across Highway 221-A. In the original CAP,- the discharge
piping was. to be installed under Highway 221-A by trenching across the highway. Based on our
experiences at similar sites, it is unlikely that :the North Carolina Department of Transportation' '.
(NCDOT) will give their permission to trench across the highway. We have also considered an ,
alternative approach that would � allow us to bore horizontally under ' the * highway; however,. the ° :. =
cost of the boring program would be very expensive. Instead of directing the discharge to the
south, we propose directing the discharge to the north to an unnamed tributary of Morrow Creek.
Figure 1 is a topographic map of the area surrounding the ' Sandy Mush site and. shows the . .
intermittent drainage we are proposing for discharge. Figure-.2 shows the location 'of the

groundwater treatment compound, the proposed location of the underground discharge piping
and the proposed discharge point near the Sandy Mush site. The discharge point is located at an
intermittent stream channel that empties into Morrow Creek (the railroad track shown in the
figure was recently abandoned and is no longer is used). Shield requests that the NCDEM
consider this 'amendment to the original Sandy Mush CAP. We believe that substantial cost
savings will be gained by the proposed discharge arrangement.
Proposed Addition of an Oil/Water Separator
To accommodate the general NPDES conditions, an oil/water separator must be installed as part
of the groundwater remediation system. Shield recommends this change because of the general
permit conditions and because the free product gasoline at RW-2 needs to be removed from the
influent water prior to air stripping. A coalescing plate oil/water separator (Hydroflo Model DP-3
or equivalent) rated for a minimum of 10 gallons per minute is recommended.
Proposed Low Profile Air Stripping Unit
In the original Sandy Mush CAP, a counter current air stripping tower was proposed. Since that
time, a low profile air stripping unit has become available through advances made in the area of air
stripping technology. Shield recommends amending the CAP to allow a low profile stripper to be
installed instead of the originally proposed air stripping towers. The low profile stripper is easier
to maintain and more cost effective since there is no column packing to replace or clean.
System Schematic and Specifications
A schematic representation of the groundwater remedial system has been included as Figure 3.
The manufacturer's specifications for the oil/water separator and the low profile stripper are
presented in Appendix A. Shield plans to extract groundwater from recovery well RW-2 using a
Grundfos Model 5E5 pump or -equivalent, as indicated in the original CAP.

are the many industries whose processes
involve contact between oil and water. This
tremendous volume is an environmental
problem in itself (one gallon of oil contami-
nates one million gallons of ground water).
Adding to the problem is the difficulty in
treating oily wastewater.

BASIC CONCEPTS
The basic concept used by most
manufacturers of gravity type oil water
separators is that materials of different
specific gravities will rise and settle at
predictable rates. By using the basic
formulas, as outlined in Stokes Law, we
can predict the retention time necessary to
separate oil of any specific gravity and
droplet size, from any ,given waste stream at
the desired flow rate. The problem is that a
separator based solely on retention time
cannot effectively separate oil particles
smaller than 150 microns, and if they could
they would be prohibitively large.
By placing certain materials in the waste
stream, the efficiencies of oil/water separa-
tion can be greatly improved due mainly to

impingement on .their surfaces. As fine oil
droplets impinge upon the plate surfaces
they coalesce, or merge, with previous drop-
lets to produce much larger droplets. There
they continue to grow in size until they are
large enough to break free and rapidly rise
to the surface where they are skimmed or
decanted. This coalescing action allows
removal of smaller droplets than is possible
with a standard gravity separator.
The effectiveness of any coalescing media
is governed by several critical variables,
shape, density, available surface area,
velocity and dire

hanalino, capabilities. At Hydro-
Fo we use a proprietary Dyna-Pak
cross corrugated coalescing media.
T ne Dyna-Pak media was designed with
consideration of all these factors. The
specific shape and spacing of the Dyna-Pak
plates provides greater coalescing
pe:,onnance than any othermedia available,
while also assuring maximum protection
from blinding. The Dyna-Pak media is
moided of ahighly oleophilic (oil attracting)
material which greatly enhances our oil -
water separation capabilities.
OPERATION -
INFLUENT DIFFUSION
CHAMBER:
Fow enters the inlet chamber where it is
evenly dispersed through a non -clog dif-
fuser across the width and depth of the
Dvna-Pak. Large solids fall out of suspen-
sion here before entering the separation
chamber.
SEPARATION CHAMBER:
The separation chamber is filled with the
Dyna-Pak coalescing media. The pack cre-
ates a uniform cross -sectional resistance
which further helps disperse the flow
throughout the separation chamber. The
sinuosidial flow of wastewater through the
pack serves to promote intimate contact
between the oil droplets and the plate sur-
faces. The coalesced oil has the least re-
stricted path to exit the waste stream, and
slides to the top of the pack on the surfaces
of the

the sludge collection chamber.
SLUDGE COLLECTION
CHAMBER:
The sludge collection chamber is located
direcly beneath the Dyna-Pak and pro-
vides ample volume for the settled sludge.
The sides of the chamber are sloped at 45
degrees to insure easy and complete re-
moval of the sludee. '
OIL REMOVAL:
The separated oil accumulates at the sur-
face of the separation chamber where it
displaces the water, as the oil layer in-
creases, oil automatically spills over an
adjustable oil skimming weir into an inte-
gral oil reservoir where it can either flow by
gravity or be pumped automatically to
remote storage tanks.
EFFLUENT CHAMBER:
The clean waterleaving the Dyna-Pakflows
under the oil reservoir and up over the ad-
justable effluent weir, which maintains
liquid level throughout the separator

chamber where it can either flow by gravity

CONSTRUCTION:
S tandard materials of construction include,
but are not limited to, carbon steel, stainless
steel, PVC, neoprene, schedule 40 pipe,
15N NSI flanges, and grade 5 stainless
steel fasteners, all water tight weld joints
are double welded and tested either hydro-
statically, magnafluxed, dye -penetrant or
other NDE methods in accordance with
current military specifications.
COATINGS:
Above grade carbon steel tanks have an
exterior surface that is prepared to a SSPC-
SP6, commercial blast, and given one (1)
coat of self priming epoxy paint (6DMT).
All steet tanks have an interior surface that
is prepared to a SSPC-SP10, near white
metal blast, and given two (2) coats of coal
tar epoxy (16 DMT). As a standard on all
buried equipment, Hydro -Flo will supply
dielectric insulators and anodes to, insure
tate-of-the-art corrosion protection for

Obtain the anticipated discharge volumes in writing from the
applicant. None was supplied .in materials available in the
Regional Office. The figures given in Part II 1. a. were obtained
during a telephone,conversation with Mr. Don Ceccarelli of Shield
Environmental.
Please run the,wasteload allocation at the three points indicated
on the attached map and provide for,two effluent pages in the
permit, (page #1) with a volume limitation of 0.0064 mgd and (page
#2) which becomes effective upon instal lation, and operation of
additional recovery wells,.with a volume limitation of 0.0120 mgd.
Issue permit after addressing above two points and any other
concerns which you may have concerning the application.
