The site map is not required to be signed or sealed by a Professional Engineer. However, the map must indicate the source of
information. For example, parcel Iines taken from a county GIS map should reference the GIS website.

knowledge. I understand that any unauthorized discharge of wastewater fi•orn this non -discharge system to surface waters or the land
may result in an enforcement action that may include civil penalties, injunctive relief, and/or criminal prosecution. I will make no claim
against the Division of Water Resources should a condition of this permit be violated. I also understand that if all required parts of this
application are not completed, this application may be returned to me as incomplete. I further certify that the Applicant or any parent,
subsidiary, or other affiliate of the Applicant has not been convicted of an environmental crime, has not abandoned a wastewater facility
without proper closure, does not have an outstanding civil penalty where all appeals have been exhausted or abandoned, are compliant
with any active compliance schedule, and does not have any overdue annual fees.
NOTE — In accordance with General Statutes 143-215.6A and 143-215.6B, any person who knowingly makes any false statement,
representation, or certification in any application package shall be guilty of a Class 2 misdemeanor, which may include a fine not to
exceed $10,000 as well as civil penalties up to $25,000 per violation. The Applicant's Certification shall be signed in accordance with
15A NCAC 02T .0106(b). Per 15A NCAC 02T .0106(c), an alternate person may be designated as the signing official if a delegation
letter is provided from a person who meets the criteria in 15A NCAC 02T .0106(12)

6. Does the facility treat and dispose of industrial wastewater? Yes.
2 industries are permitted by the Town of Apex pretreatment program, both operated by Tipper Tie, Inc.
Both industries are EPA categorical industries designated as 40 CFR 465 aluminum forming
manufacturers. These industries take large diameter aluminum cable and draw it down to appropriately
sized diameters for use as electric fencing wire and aluminum staples on food packaging such as sausage
patties, orange bag netting, etc.
Both facilities have industrial wastewater releases are controlled by Apex pretreatment permit numbers
002 and 010. They are required to report daily flows, monthly sampling and analysis, and monthly
production reports to the Town of Apex. The permit applications included cleaning, production chemical
use, wastewater chemical releases, spill contingency plans, production hours and rates, etc.
Each facility is judged monthly for compliance with flow and pollutant limits in their permits. Production
wastewater flows are only 15,000 and 5,000 gallons per day from these facilities. A site inspection is
conducted annually per pretreatment regulations. Each facility maintains and utilizes wastewater
treatment equipment that is required by each of the permits. Required equipment includes a flow meter,
oil and grease separator, and an ultrafiltration system.

Pursuant to the North Carolina Administrative Code 15A NCAC o2T .oio6, the Town of Apex is
writing to designate our Water Resources Director as the signatory authority for all NC Division
of Water Resources permit applications. Our current Director of Water Resources is Michael
Deaton, P.E.
This designation will provide Mr. Deaton with the authority to sign all required permit
