6. 'Specific Condition and Limitation No. A. 8. as required by 15A NCAC 21 .0524'NNew Source Performance
Standards" states that NSPS affected sources commence construction, reconstruction, or, modification after August 31,
1983, mustcomply with notification, testing, reporting, record keeping, and monitoring requirerrients as promulgated
in 40 CFR, Part 60, Subpart 006, including Subpart A, "General Provisions".
The NSPS notification requirements state that the Permittee must notify the DAQ in writing of the date construction
or reconstruction of an affected facility commenced, postmarked no later than thirty (30) days after such date;'and the
actual date of initial start-up of an affected facility, postmarked within fifteen (15) days after, such date.
The facility has submitted the above referenced notifications within the required time frame; therefore,
compliance is indicated
The NSPS visible :emissionslmiitations;are as follows: 15%. opacity oil criisliers; 10% opacity for conveyor belts and
screens; and 0%.opacity for process saturated materials from conveyor belts and other affected facilities such as
screens.
During theInspection, there were, no visible emissions above. the opacity limits listed under [his condition.
Compliance is indicated
`Facilities that commenced construction, modification, or reconstruction on or after April 22, 2008 that use wet
suppression to .control emissions must conduct monthly inspections of the wet suppression systems. Each inspection
and any corrective action must be recorded in a logbook.
This facility was. constructed prior to 2008; therefore,.they are not required to conduct'monthly inspections.

Process Dust Emissions Sources" states that the Permittee. shall not cause or allow fugitive non -process dust emissions
to cause or contribute to substantive complaints. This company is utilizing'a water truck to wet the roads at this
facility.
There have not been any dust complaints for this facility since the last inspection was conducted The roads
appeared to be wet during the inspection, and there was no dust observed beyond the property line. therefore,
compliance is indicated

As was -indicated in the previous inspection the facility has replaced their diesel -fired pit pump with an electric pump;
therefore, they do not appear to be subject to the requirements of the RICE MACT — Subpart ZZZZ.

7. Report:
On September 7, 2016, a.multimedia inspection at the Vulcan Construction Materials -Gold Hill quarry
was conducted. The facility is an active quarry. The facility is currently operating as a Conditionally
Exempt Small. Quantity Generator (CESQG).
A tour of facility operations was conducted during the visit. The facility generates used oil. Used oil is
accumulated in an above ground tank. The tank is located within a secondary containment structure. Tank
was properly labeled at the time !of the inspection. Used oil is pumped out as needed, oil was last
pumped on 7/19/2616 by Heritage Crystal Clean (EPA ID # ILR000130062).
Used florescent light bulbs are stored properly, within a closed box, for disposal.

Although the, facility primarily opexates as a conditionally exempt small quantity generator
(CESQG) of hazardous waste, it is a reminder that if the facility generates more than 220-pounds of
total hazardous waste in any calendar month, or accumulates more than 2,200-pounds of hazardous
waste onsite at any one time, the facility will be subject to small quantity generator (SQG)
regulations.. SQG hazardous waste regulations are listed in Title 40, Code -of Federal Regulations,
Part 262.34 (c) & (d). If the facility generates more than 2,200-pounds of hazardous waste in any
calendar month or accumulates more than 2.2-pounds of acutely toxic (P-Listed) hazardous waste

onsite at any one time the facility will.be subject to full regulation as a large quantity generator ,
(LQG). LQG hazardous waste regulations. are listed in Title 40, Code of Federal Regulations, Part
262. Additionally, LQGs and SQGs must obtain a site EPA Identification Number before offering
hazardous waste for off: site transport and disposal. Guidance documents for hazardous waste
