Please find attached a fast track application package for the modification of permit #
WQ0029614 The Residences at Boca Bay. This package includes the application fee and two copies of:
• Two copies of application form FTA 04-16.
• Two copies of the USE for the project.
• The application fee of $480.
Please let the following serve as a brief narrative for the project:
The most recent permit for this project was granted on July 20,2007. At that time the flow
allocated was 11,200 gpd. The original design was for the project build out which included residences
and a clubhouse with a pool. The original permit included 27 three to four bedroom residences and an
amenity center/pool complex. The current state of development includes a total of 183 bedrooms. This
includes 3 bedrooms allocated for the pool/amenity center.
The next phase of development is seeking an additional 117 bedrooms for a total number 200
bedrooms. This yields a flow of 24,040 gpd (200 x 120 = 24,040).
The current allocation is for 11,200 gpd so the difference being applied for in this application is
12,840 gpd (24,040—11,200 = 12,840).
Please note that this application does not propose any changes to the lift station since it was
originally designed for this ultimate flow. The lift station has a stand-by generator in place.

111. Certification Statement:
I Kyle Breuer certify to the best of my knowledge that the addition of
the volume of wastewater to be permitted in this project has been evaluated along the route to the receiving
wastewater treatment facility and that the flow from this project is not anticipated to cause any capacity
related sanitary sewer overflows or overburden any downstream pump station en route to the receiving
treatment plant under normal circumstances, given the implementation of the planned improvements
identified in the planning assessment where applicable. This analysis has been performed in accordance
with local established policies and procedures using the best available data. This certification applies to
those items listed above in Sections I and lI plus all attached planning assessment addendunis for which I
ant the responsible party. Signature of this form certifies that the receiving collection system or treatment
works has adequate capacity to transport and treat the proposed new wastewater.
