On January 7, 2021, Melissa Joyner from the Fayetteville Regional Office of the Division of Energy,
Mineral and Land Resources, conducted a site inspection for the Lisk Trucking, Inc. facility located at
6975 NC Highway 742 N., Wadesboro, Anson County, North Carolina. A copy of the Compliance
Inspection Report is enclosed for your review. Phillip Powell, Jr., Sand Division Supervisorwas also
present during the inspection and his time and assistance is greatly appreciated. The site visit and file
review revealed that the subject facility is covered by NPDES Stormwater General Permit NCG080000,
Certificate of Coverage NCGO80198. Permit coverage authorizes the discharge of stormwater from the
facility to receiving waters designated as Cabin Branch Creek, a Class C waterbody in the Yadkin Pee
Dee River Basin.
As a result of the site inspection, the following permit conditions violations are noted:
1)Stormwater Pollution Prevention Plan (SPPP)
Failure to develop, record or properly implement a Stormwater Pollution Prevention Plan. NCG080000.
Part II, Section A
A Stormwater Pollution Prevention Plan has not been developed, recorded, or properly implemented.
2)Qualitative.Monitoring
Failure to conduct Qualitative monitoring. NCG080000, Part II, Sec. D
Qualitative monitoring has not been conducted and recorded in accordance with permit requirements.

have occurred as well as a Plan of.Action to prevent these violations from recurring.
Thank you for your attention to this matter. This office requires that the violations, as detailed above,
be properly resolved. These violations and any future violations are subject to a civil penalty

Inspection Summary:
Melissa Joyner met with Mr. Chris Tucker, General Manager and Mr. Phillip Powell, Jr., Sand Division Supervisor. The
facility had previously been issued Notices of Violations on November 13, 2008 and on May 31,2016 for not having a
Stormwater Pollution Prevention Plan (SPPP)and not conducting Analytical and Qualitative Monitoring. She initially reviewed
the SPPP and noted the following items which should be included in this document:
1.All parts of the SPPP need to be reviewed/updated (if needed) annually,with the person in charge of the SPPP
signing/dating a statement that this has occurred.
2.A site map which more clearly shows the drainage areas and notes the percentage of each drainage area which is
impervious. The map should be updated, showing the location of the sludge pit and the correct location of the Outfall. The
structural best management practices(BMP's)which are being and will be utilized should also be shown/labeled on the site
map.
3.A narrative which fully provides the information as specified in General Permit NCG080000.
4.A list of significant spills or the notation that no spills have occurred for the past 3 years should be documented annually.
5. Annual certification that the outfall has been evaluated for the presence of non-stormwater discharges.
6.A feasibility study about possibly modifying operations and/or storage practices to eliminate or reduce exposure of
materials and processes to rainfall and run-on flows. This should be done annually.
7.A BMP summary which lists the site structural and non-structural BMP's.
8. Information about the facility's Spill Prevention and Response Procedures (SPRP)specifically in regards to stormwater
management.The names of those responsible for implementing the SPRP should be included as a written list, signed and
dated by those individuals.
9. Information about the facility's Preventative Maintenance and Good Housekeeping Program including the information
specified in General Permit NCG080000. Facility inspections in regards to stormwater BMP's should occur twice per year
and be documented in the SPPP.
The Analytical and Qualitative Monitoring records were reviewed. Some Analytical Monitoring forms from 2020 were emailed
to the Inspector after the inspection. Since the last inspection on May 26, 2016,the facility was in Tier Two, monthly
monitoring, due to exceedances of the benchmark value for Total Suspended Solids(TSS). No Analytical and Qualitative
monitoring occurred in October and November, in 2016, or October through December, in 2017,or in April, 2018. Monitoring
only occurred once in 2019. The facility came back into compliance in 2020 and has now resumed semi-annual monitoring.
The facility grounds were inspected.A rain gauge will need to be installed on the site. The location of the single outfall will
need to be changed from the middle of the intermittent stream to the end of the swale, prior to where water discharges from
it into the stream.
The haul road near the access to the sludge pit was eroding and will need stabilization to prevent potential impacts to the
intermittent stream. The sludge pit and area adjacent to it where a sand pile was located will need additional, protective
measures installed. The sludge pit will need to be continuously cleaned out in a timely manner.
