certification with record drawings. The Division hereby accepts this certification based upon its
execution and submittal by a North Carolina Professional Engineer. Please note that the record,
drawings will be forwarded to the Asheville Regional Office and have not been reviewed. If a
review is conducted, you will be notified of any non-conformity with the Minimum Design
Criteria.

The above Engineer's Certification must be completed and submitted to the address below with one (1)
copy of the "Record Drawings" (i.e., as-constructed plans) of the wastewater collection system. This
project shall not be considered complete and allowed to operate until this Engineer's Certification and
the 'Record Drawings" plans have been submitted. Any wastewater flow made tributary to the
wastewater collection system prior to completion of this Engineer's Certification shall be considered a
violation of the permit and shall subject the Permittee to appropriate enforcement actions.
