Per the requirements of NC DEQ- Division of Energy, Mineral and Land Resources (DEMLR) and
your NOV letter dated December 17, 2020, on behalf of C&M Recycling Inc., CP Engineering and
Environmental Solutions (CPEES) is pleased to submit the attached NPDES Stormwater General Permit
(NCG130000) request for our facility located at 2930 Jeff Adams Dr., Charlotte, North Carolina. C&M
Recycling operations include receiving old concrete and asphalt blocks/debris and crush them to stone
products for recycling. C&M Recycling agrees with your SIC Code (5093-"non-metal waste and scrap")
determination and the selection of the NPDES Stormwater General Permit (NCG130000). The Notice of
Intent (NOI) application package includes:
• $100 —payment (Payable to NC DEQ-DEMLR);
• Completed NOI Forms (NCG130000);
• Site Location Map;
• Site Aerial Map with Outfall Locations; and
• Report.
