Carolina Administrative Code 211 .0 105, under the authority of North Carolina General Statutes
143-215.3(a)(1), (1 a) and (lb). Because this fee was not fully paid. within 30 clays after being
billed, this letter initiates action to revoke the subject permit, pursut to 15 NCAC 2H .010 (b)
(2) (k) (4), and G. S. 143- 15 1 (b) (3)�
Effective 60 days from receipt of this notice, subject permit is hereby revoked unless the
required Annual Administering and Compliance Monitoring Fee is received within that time.
