A Compliance Evaluation Inspection report was issued on December 18, 2007. PERMIT: The current permit became effective on July 1, 2005, and expires on June 30, 2010. The permit properly describes the facility. The 0.033 MGD facility consists of an extended aeration package treatment plant with influent lift stations, bar screen, equalization basin, aeration basin, a rectangular clarifier, tablet chlorination, chlorinecontact chamber, tablet dechlorination, continuous recording flown measurement, tertiary sand filters, mud well, clear well, aerobic digester, and post aeration. The facility appears to be adequate for the wash stream being, treated. The certification classification for NCO0 3' 89 requires that the facility have an operator in responsible charge ((SRC) with a Grade 2 Biological Water Pollution Control System (WW-2) certification. The Division of Water Quality Compliance Database currently shows Mr. Billy Henson as the ORC and nine qualified. Back-up CRCs. However, the September, October, and November 2008 Discharge Monitoring Reports note that Mr. Carl Donahue is the ORC at the Facility. Please complete the. attached ORC form (if not clone already) if there has been a change to the ORC at the facility, the form mush be submitted. within 120 days` of an ORC change. CORDS PORTS Discharge :Monitoring Reports (DMRs) were reviewed for the period September 2007 through August 2008. The reports appeared to be properly completed. The ORC maintains detailed visitation logs and calibration records relative to the operation, maintenance, and self monitoring of the facility. Records and logs were readily available upon request. FACILITY SITE REVIEW: All of the existing components of the facility were in service and appeared to be operating properly at the time of the inspection. EFFLUENT/RECEIVING WATERS: The facility: discharges to Irvins Creek, which is a Class C water in the Catawba River Basin. There was no discharge at the time ofthe inspection. The receiving water slid not appear; to be adversely affected by the effluent. FLOW MEASUREMENT: Flow measurement is accomplished rising a Y}Notch weir as the primary device and a totalizer/continuous recording flow meter in accordance with NPD S permit it requirements. Spills occurring on weekends or holidays can be reported by calling the North Carolina Division of Emergency Management at 919f733- 3300 800/662-7956, or 800/8 -036 . A written report shall also be provided within five (5) days of the time of the incident. The report shall contain a description of the bypass, and its cause; the period of the bypass, including exact dates and times, and if the bypass has not been corrected, the anticipated time it is expected to continue; and steps taken (or planned) to reduce, eliminate, and prevent recurrence of the similar events. Any spill that reaches surface waters (i.e. any spill that reaches any water already present in a conveyance, stream, ditch, etc...) or any spill greater than 1,000 gallons on the ground that does not reach surface waters must be reported. An adequate spill response for those spills reaching surface waters should include an evaluation downstream of the point at which the spill entered surface waters to determine if a fish kill occurred. The evaluation should also include the collection of upstream dissolved oxygen and pH measurements for background information and dissolved oxygen and p measurements at multiple points downstream of the entry point to document any negative impact. The ORC maintains detailed lags and calibration records relative to the operation & maintenance and self -monitoring of the facility, FACILITY SITEREVIEW: The facility discharges to Irvins Creek, which is a Class C water in the Catawba River Basin, The CIR.C, Billy R. RECORDS/REPORTS: Discharge Monitoring Reports (DMR's) were reviewed for the period March 2001 through February 2002. The reports appeared to be properly completed. The CRC maintains detailed logs and calibration records relative to the operation & maintenance and self -monitoring of the facility. FACILITY SITE REVIEW: The facility discharges to Irwin Creek, which is a Class C water in the Catawba River Basin. All of the existing components of the facility were in service and appeared to be operating properly at the time of the inspection. FLOW MEASUREMENT: Flow measurement is accomplished using a V-Notch weir as the primary device and a totalizer/continuous recording to meter in accordance with NPDES permit requirements. Instrumentation Services, Inc. last calibrated the flow meter on August 24, 2001.
