DIFFUSE FLOW CONDITION
1 Diffuse flow plan aQproval The d�ffuse flow plan approved by th�s office consists of one level
spreader and engmeered vegetated filter strip and all assoc�ated stormwater conveyances the
gradmg and dramage patterns dep�cted on plan sheets dated October 2011 and received by the
DWQ on October 31 2011 No changes to the level spreader engmeered filter strip shall be
made wrthout wr�tten authorizat�on from the DWQ The diffuse flow devices shall have
mamtenance access and at least a 10 foot dramage easement on each s�de to allow for the ab�lrty
to mamta�n the structures and to perform correct�ve act�ons The stormwater easements shall�be
recorded to prov�de protect�on of the structures from potential alterattons by future property
owners
The approved diffuse flow must be mamtamed in perpetuiry m accordance wrth LS VFS
operation and maintenance(O& M)agreements s�gned by Mr Charles D Mast(Member
Highway 42 Assoc�ates LLC)on October 21 2011 The O& M agreements must transfer w�th
the sale of the property or transfer of ownership/responsibility for the BMP facil�t�es DWQ must
be notified promptly of every transfer
