your appl�catton rece�ved by the Divis�on of Water Quality(DWQ)on September 26 2011 and add�t�onal
information received by DWQ on November 8 2011 (dated November 4 2011) After reviewtng your
application we have dectded that the impacts are covered by General Water Quality Certification Number(s)3819
(GC3819) The Certification(s)allows you to use Nationwide Perm�t(s)NW 12 when issued by the US Army
Corps of Engineers{USACE) This letter shall also act as your approved Author�zation Certificate for impacts to
the protected riparian buffers per 15A NCAC 2B 0233 In addition you should obtam or otherwise comply with
any other required federal state or local permits before you go ahead with your pro�ect mcludmg(but not lim�ted
to) Erosion and Sed�ment Control and Non d�scharge regulations Also,this approval to proceed w�th your
proposed impacts or to conduct�mpacts to waters as depicted �n your appl�cation shall expire upon
expiration of the 404 Permit
Th�s approval is for the purpose and destgn that you described m your applicat�on If you change your pro�ect
you must notify us and you may be requ�red to send us a new appl�cat�on If the property is sold the new owner
must be grven a copy of this Certification and approval letter and is thereby responsible for complymg with all
cond�t�ons If total fills for this pro�ect(now or in the future)exceed one acre of wetland or 150 lmear feet of

2 No Waste Spoil Solids or Fill of Any K�nd
No waste spoil sol�ds or fill of any kmd shall occur tn wetlands waters or r�parian areas beyond the
footpr�nt of the im�acts depicted in the Pre Construct�on Notification All construction act�vities
includmg the design mstallation operat�on and mamtenance of sediment and erosion control Best
Management Pract�ces shall be performed so that no v�olat�ons of state water quality standards statutes
or rules occur
3 Eros�on and sediment control practices must be in full compl�ance with all specifications governmg the proper
design tnstallation and operation and mamtenance of such Best Management Practices in order to protect
surface waters standazds
a The erosion and sediment control measures for the proJect must be des�gned mstalled operated and
mamtamed in accordance wtth the most recent version of the North Carolma Sed�ment and Erosion
Control Plannmg and Destgn Manual
b The des�gn installation operat�on and mamtenance of the sed�ment and eros�on control measures
must be such that they equal or exceed the requ�rements spec�fied �n the most recent version of the
North Cazolma Sed�ment and Eroston Control Manual The devices shall be maintamed on all
construct�on sites borrow sites and waste p�le(spoil)pro�ects mcludmg contractor owned or leased
borrow pits assoc�ated with the pro�ect
c Sufficient mater�als requ�red for stab�lizat�on and/or repair of eroston control measures and
stormwater rout�ng and treatment shall be on site at all times
4 Sed�ment and Erosion Control Measures
Sediment and erosion control measures shall not be placed in wetlands or waters w�thout pr�or approval
from the D�vision If placement of sed�ment and erosion controt devices in wetlands and waters is
unavo�dable des�gn and placement of temporary erosion control measures shall not be conducted m a
manner that may result m d�s equilibrium of wetlands or stream beds or banks ad�acent to or upstream
and down stream of the above structures All sed�ment and erosion control devtces shall be removed and
the natural grade restored w�th�n two(2) months of the date that the Div�sion of Land Resources or locally
delegated program has released the pro�ect

5 Buffer Mrtigation
You are required to mit�gate for tmpacts to the protected r�par�an buffers The required area of mrtigation
to compensate for tmpacts to Zone 1 of the protected riparian buffers is 20 421 square feet as requ�red
under th�s Author�zat�on Certificate and 15A NCAC 2B 0233 We understand that you intend to
purchase credrts from the Contentnea Little Contenfiea Creek Mrtigat�on Bank in Greene County as
described m your applicat�on to meet this mit�gation requ�rement Our records show that the ContenMea
Little Contentnea Creek Mttigation Bank m Greene County has sufficient credits available for release for
your pro�ect Th�s contribution has been determined by the DWQ to be a surtable method to meet the
mrtigat�on requirement Unhl the ContenMea Little Contentnea Creek Mrtigatton Bank recerves and
clears your check and proof of payment has been prov�ded to th�s Office no�mpacts specified in this
Author�zation Certificate shall occur For account�ng purposes,th�s Authorizahon Cerhficate
authonzes payment into the Contentnea-Little Contentnea Creek 1VLt�gat�on Bank to meet the

wetland or water pr�or to any land disturb�ng activities to ensure compl�ance with 15 NCAC 2H Sect�on
