The diffuse flow info that we got on Dec 1 looked fine to me I know you still have a couple of issues on this proJect
right now but I went ahead and wrote stormwater and diffuse flow conditions that you can add to the 401 when the
time comes

STORMWATER AND DIFFUSE FLOW CONDITION
1 Diffuse flow plan approval The diffuse flow plan approved by tlus office consists of one
level spreader and engmeered vegetated filter strip and all associated stormwater
conveyances,the gradmg and dramage patterns depicted on plan sheets dated November
4, 2011 No changes to the level spreader engmeered filter strip shall be made without
wntten authonzation from the Division of Water Quality The diffuse flow devices shall
have mamtenance access and at least a 10 foot dramage easement on each side to allow
for the ability to mamtam the structures, and to perform corrective actions The
stormwater easements shall be recorded to provide protection of the structures from
potential alterahons by future property owners
The approved diffuse flow must be mamtamed an perpetuity m accordance with LS VFS
operation and mamtenance (O &M) agreements signed by Blair Booth(Manager,
Symphony Properties, LLC) on November 4, 2011 The O &M agreements must transfer
with the sale of the property or transfer of ownership/responsibility for the BMP
facilities DWQ must be notified promptly of every transfer
2 Stormwater mana ement p�APPROVED PLAN NEEDED) A copy of the
stormwater management plan with proof of approval by the Town of Wake Forest shall
be submitted to the DWQ WeBSCaPe Urut before the impacts specified in this
Certification may occur The stormwater management plan must mclude construction
plans and specifications Also, before any permanent buildmg is occupied at the subJect
site, the facilities (as approved by the Town of Wake Forest) shall be constructed and
operational The structural stormwater practices and dramage patterns approved by the
Town of Wake Forest must be mamtamed m perpetuity No changes to the structural
stormwater practices shall be made without wntten authonzation from the Town of Wake
Forest If the stormwater management plan is modified then a copy of the modified plan
with proof of approval by the Town of Wake Forest shall be submitted to the DWQ
