2011 This approval is a modification to an approval issued by the Division on January 12, 2009 After reviewmg your modification application we have determined that this fill is covered by General issued by the U S A.rmy Corps of Engineers In addition you should secure any other applicable federal state or local permrts before you proceed with your pro�ect mcludmg (but not limited to) those 0 42 acres of isolated wetland m response to previous violations at the site as required m the January 12 2009 approval 3 The permittee shall visually momtor the wetland restoration site and vegetative plantings to assess and ensure complete restoration to appropnate wetland conditions zncludmg appropnate vegetation growth and hydnc soils development The momtonng shall be conducted quarterly for a rrummum of 1 year after final planring and annually after that for a mirumum of three years The permittee may be required to conduct additional restoration activities if the proposed restoration plan is considered unsuccessful An annual momtonng report shall be submitted to the Division of Water Quality Wmston Salem Regional Office at the address noted on this letterhead The first report is due witlun one year of completion of the final plantmg 4 Modified and/or addrtional deed notifications or similar mechamsms shall be placed on all retained �unsdictional wetlands waters and protective buffers wrthm the newly platted subdivision in order to assure compliance for future wetland water and buffer impact These mechamsms shall be put m place at the time of recordmg of the property or of individual lots whichever is appropnate A sample deed nohfication can be downloaded from the 401/Wetlands Umt web site at http //h2o enr state nc us/ncwetlands The text of the sample deed notification may be modified as appropnate to surt to tlus pro�ect 5 Upon fimshmg the pro�ect the Applicant shall fill out and return the enclosed Certificate of Completion to notify NCDWQ when
