DEMOLITION NOTES:
1.COORDINATE WITH DUKE ENERGY PRIOR TO ANY RELOCATION OR DEMOLITION OF THE EXISTING OVERHEAD POWER LINES.
2.THE CONTRACTOR SHALL FIELD VERIFY AND LOCATE ALL EXISTING UTILITIES ON SITE PRIOR TO DEMOLITION.
3.THE CONTRACTOR SHALL PERFORM DEMOLITION ACTIVITIES AS NOTED AND SHOWN ON THESE PLANS AND AS DIRECTED BY THE
OWNER.
4.IT WILL BE THE CONTRACTOR'S RESPONSIBILITY TO OBTAIN ANY PERMITS AND PAY FEES REQUIRED FOR DEMOLITION AND HAUL-OFF
FROM THE APPROPRIATE AUTHORITIES. THESE FEES ARE TO BE INCLUDED WITH THE BID.
5.THE CONTRACTOR SHALL PREPARE ALL DOCUMENTS AND ACQUIRE APPROPRIATE PERMITS AS REQUIRED PRIOR TO THE
COMMENCEMENT OF DEMOLITION.
6.THE DEMOLITION PLAN IS INTENDED TO DEPICT GENERAL DEMOLITION AND UTILITY WORK. IT IS NOT INTENDED TO IDENTIFY EACH
ELEMENT OF DEMOLITION OF RELOCATION. CONTRACTOR SHALL COORDINATE WITH THE OWNER AND APPROPRIATE UTILITY
COMPANY PRIOR TO WORK.
7.REMOVE AND DISPOSE OF ANY SIDEWALK, FENCES, STAIRS, WALLS, FOUNDATIONS, CONDUITS, LIGHT POLE BASES, DEBRIS AND
RUBBISH REQUIRING REMOVAL FROM THE WORK AREA IN AN APPROVED LANDFILL.
8.CONTRACTOR SHALL COORDINATE WITH UTILITY DEPARTMENT IN THE REMOVAL/RELOCATION OF EXISTING POWER POLES AND
OVERHEAD POWER LINES.
9.THE CONTRACTOR SHALL ARRANGE FOR RESETTING OF CURB BOXES, VALVE BOXES AND REMOVAL AND/OR RELOCATION OF
OVERHEAD UTILITIES AND POLES WITH THE APPROPRIATE UTILITY COMPANY.
10.INSTALL ALL EROSION AND SEDIMENT CONTROL DEVICES AND TREE PROTECTION PRIOR TO BEGINNING DEMOLITION WORK.
11.THE CONTRACTOR IS RESPONSIBLE FOR THE PROTECTION OF ALL UTILITIES TO REMAIN IN PLACE.
12.THE CONTRACTOR SHALL TAKE ALL PRECAUTIONS TO AVOID UNNECESSARY DAMAGE TO EXISTING ROAD SURFACE.
13.FINISH SURFACE TO BE REMOVED OR DEMOLISHED SHALL BE CUT ALONG LINES OF JOINTS WHICH WILL PERMIT A NEAT SURFACE
WHEN RESTORED.
14.SAWCUT AT INTERFACE OF PAVEMENT OR CURB TO REMAIN. SAWCUT EXISTING PAVEMENT AT THE R/W.
15.ALL EXISTING ITEMS TO REMAIN WHICH ARE DAMAGED DURING CONSTRUCTION SHALL BE RESTORED TO ITS ORIGINAL CONDITION AT
THE SOLE EXPENSE OF THE CONTRACTOR.
16.THE CONTRACTOR SHALL MAINTAIN ALL UTILITY SERVICES TO THE ADJOINING PROPERTIES DURING THE DEMOLITION PROCESS.
17.DO NOT INTERRUPT EXISTING UTILITIES SERVICING FACILITIES OCCUPIED AND USED BY THE OWNER OR OTHERS DURING OCCUPIED
HOURS EXCEPT WHEN SUCH INTERRUPTIONS HAVE BEEN AUTHORIZED IN WRITING BY THE OWNER AND THE LOCAL MUNICIPALITIES.
INTERRUPTIONS SHALL ONLY OCCUR AFTER ACCEPTABLE TEMPORARY SERVICE HAS BEEN PROVIDED.
18.SHOULD ANY UNCHARTED OR INCORRECTLY CHARTED EXISTING PIPING OR OTHER UTILITY BE UNCOVERED DURING EXCAVATION,
CONSULT THE ENGINEER IMMEDIATELY FOR DIRECTIONS BEFORE PROCEEDING FURTHER WITH WORK IN THE IS AREA.
19.IN ACCORDANCE WITH 40 CFR, PART 61, SUBPART M, A THOROUGH INSPECTION SHALL BE CONDUCTED FOR ASBESTOS-CONTAINING
MATERIALS, PRIOR TO CONDUCTING A RENOVATION OR DEMOLITION ACTIVITY OF AN EXISTING STRUCTURE. THE ASBESTOS
INSPECTION MUST BE CONDUCTED BY A NORTH CAROLINA ACCREDITED ASBESTOS INSPECTOR. ALL REGULATED AND/OR FRIABLE
ASBESTOS-CONTAINING MATERIALS IDENTIFIED DURING THE SURVEY, WHICH WILL BE DISTURBED, MUST BE PROPERLY REMOVED BY
ACCREDITED INDIVIDUALS BEFORE BEGINNING THE RENOVATION OR DEMOLITION ACTIVITY. IF REGULATED AND/OR FRIABLE
ASBESTOS CONTAINING MATERIALS TO BE REMOVED EXCEED 160 SQUARE FEET, 260 LINEAR FEET OR 35 CUBIC FEET, A PERMIT SHALL
BE APPLIED FOR THROUGH THE NORTH CAROLINA DEPARTMENT OF HEALTH AND HUMAN SERVICES.
20.UTILITY APPURTENANCES TO BE REMOVED INCLUDE BUT AREN'T LIMITED TO HYDRANTS, FIRE DEPARTMENT CONNECTIONS,
VALVES/VALVE BOXES, STORM DRAIN AND/OR SANITARY SEWER CLEANOUTS, STORM DRAIN INLETS.
EROSION CONTROL NOTES:
1.ALL "STD." NUMBERS REFER TO THE NORTH CAROLINA DEPARTMENT OF HEALTH AND ENVIRONMENTAL CONTROL
(NCDHEC) STANDARDS MANUAL.
2.ON-SITE BURIAL PITS REQUIRE AN ON-SITE DEMOLITION LANDFILL PERMIT FROM THE ZONING ADMINISTRATOR.
3.ANY GRADING BEYOND THE DENUDED LIMITS SHOWN ON THE PLAN IS A VIOLATION OF THE COUNTY EROSION
CONTROL ORDINANCE AND IS SUBJECT TO A FINE.
4.GRADING MORE THAN ONE ACRE WITHOUT AN APPROVED EROSION CONTROL PLAN IS A VIOLATION OF THE COUNTY
EROSION CONTROL ORDINANCE AND IS SUBJECT TO FINE.
5.ALL PERIMETER DIKES, SWALES, DITCHES, PERIMETER SLOPES AND ALL SLOPES STEEPER THAN 3 HORIZONTAL TO 1
VERTICAL (3:1) SHALL BE PROVIDED TEMPORARY OR PERMANENT STABILIZATION WITH GROUND COVER AS SOON AS
PRACTICABLE BUT IN ANY EVENT WITHIN 7 DAYS CALENDAR DAYS FROM THE LAST LAND DISTURBING ACTIVITY. ALL
OTHER DISTURBED AREAS SHALL BE PROVIDED TEMPORARY OR PERMANENT STABILIZATION WITH GROUND COVER AS
SOON AS PRACTICABLE BUT IN ANY EVENT WITHIN 14 CALENDAR DAYS FROM THE LAST LAND DISTURBING ACTIVITY.
6.ADDITIONAL MEASURES TO CONTROL EROSION AND SEDIMENT MAY BE REQUIRED BY A REPRESENTATIVE OF THE
NCDEQ ENGINEERING DEPARTMENT.
7.A GRADING PLAN MUST BE SUBMITTED FOR ANY LOT GRADING EXCEEDING ONE ACRE THAT WAS NOT PREVIOUSLY
APPROVED.
8.DRIVEWAY PERMITS FOR CONSTRUCTION ENTRANCES IN NCDOT RIGHT OF WAY MUST BE PRESENTED AT PRE
CONSTRUCTION MEETING.
9.THIS SITE IS TO BE INSPECTED AND DOCUMENTED IN ACCORDANCE WITH THE NC GENERAL CONSTRUCTION PERMIT.
10.AREA UNDER EMBANKMENT SHALL BE CLEARED, GRUBBED, AND STRIPPED OF ANY VEGETATION AND ROOT
MATERIAL. THE BASIN AREA SHALL BE CLEARED.
11.THE FILL MATERIAL FOR THE EMBANKMENT SHALL BE FREE OF ROOTS OR OTHER WOODY VEGETATION AS WELL AS
OVERSIZED STONES, ROCKS, ORGANIC MATERIAL OR OTHER OBJECTIONABLE MATERIAL. THE EMBANKMENT SHALL BE
COMPACTED BY TRAVERSING WITH EQUIPMENT WHILE BEING CONSTRUCTED. SPILLWAYS SHOULD NOT BE
CONSTRUCTED THROUGH FILL SECTIONS. ALL SPILLWAYS SHOULD BE LINED AND/OR RIP-RAPPED.
12.SEDIMENT SHALL BE REMOVED AND TRAP RESTORED TO ITS ORIGINAL DIMENSIONS WHEN THE SEDIMENT HAS
ACCUMULATED TO DEPTH SHOWN ON STANDARD. REMOVED SEDIMENT SHALL BE DEPOSITED IN A SUITABLE AREA IN
SUCH A MANNER THAT IT WILL NOT ERODE.
13.THE TRAP SHALL BE INSPECTED AFTER EACH MEASURABLE RAIN AND REPAIRS MADE AS NECESSARY.
14.CONSTRUCTION OPERATION SHALL BE CARRIED OUT IN SUCH A MANNER THAT EROSION AND WATER POLLUTION IS
MINIMIZED.
15.ALL CUT AND FILL SLOPES SHALL BE 2:1 OR FLATTER, UNLESS CERTIFIED BY REGISTERED GEOTECHNICAL ENGINEER.
16.SEDIMENT BASIN EMBANKMENTS SHOULD BE PROVIDED WITH EROSION CONTROL AND STABILIZATION.
17.THE BASIN SHOULD ALSO BE ORIENTED SUCH THAT THE FILTER AND THE MAIN FLOW OF WATER AND SEDIMENT ARE
ON OPPOSITE ENDS ON THE LONGER BASIN DIMENSIONS.
18.WHENEVER TOPOGRAPHY ALLOWS, THE BASIN LENGTH SHOULD BE TWICE (2X) THE BASIN WIDTH, TO ALLOW FOR
SETTLING. BAFFLES SHALL BE INSTALLED IN ALL BASINS.
19.CLEANOUT STAKES SHALL BE PLACED IN ALL SEDIMENT BASINS AT THE LOW POINT IN THE BASIN. THE STAKES SHALL
BE MARKED SHOWING THE HALF FULL, CLEANOUT POINT, OF THE BASIN.
20.SAFETY FENCING 3' HIGH SHOULD BE PLACED AROUND ALL SEDIMENT BASINS.
21.FOR DESIGN OF SEDIMENT BASINS, REFER TO THE NORTH CAROLINA DEPARTMENT OF HEALTH AND ENVIRONMENTAL
CONTROL STANDARDS MANUAL.
22.FOR SLOPES GREATER THAN 10' IN LENGTH AND PROTECTED BY SILT FENCE AT THE TOE OF THE SLOPE, SLOPE
TERRACING WILL BE REQUIRED.
23.THE BERM ON SEDIMENT BASINS SHALL BE SEEDED ONCE FINAL GRADE HAS BEEN REACHED. THE SILT FENCE MAY BE
REMOVED IF PERMISSION HAS BEEN GRANTED BY THE CITY LAND DEVELOPMENT INSPECTOR AFTER THE GRASS HAS
GERMINATED AND STABLE GROUND HAS BEEN ESTABLISHED.
24.WASHED STONE AND WIRE BACKING SHALL BE USED WITH SILT FENCE WHENEVER SILT FENCE IS PLACED AT THE TOE
OF A SLOPE >10' VERTICAL OR ALONG ANY CHANNEL OR WATER COURSE WHERE 50' OF BUFFER IS NOT PROVIDED.
25.ALL TREES & BRUSH REMOVED DURING CLEARING & GRUBBING OF SITE SHALL BE CHIPPED ON SITE AND HAULED OFF
SITE FOR DISPOSAL.
26.ALL DISTURBED AREAS TO BE SEEDED.
27.NO DEMOLITION TO OCCUR PRIOR TO PRE-CONSTRUCTION MEETING.
NCDEQ STANDARD NOTES:
1.IF NECESSARY, SLOPES, WHICH EXCEED EIGHT (8) VERTICAL FEET SHOULD BE STABILIZED WITH SYNTHETIC OR
VEGETATIVE MATS, IN ADDITION TO HYDROSEEDING. IT MAY BE NECESSARY TO INSTALL TEMPORARY SLOPE
DRAINS DURING CONSTRUCTION. TEMPORARY BERMS MAY BE NEEDED UNTIL THE SLOPE IS BROUGHT TO GRADE.
2.STABILIZATION MEASURES SHALL BE INITIATED AS SOON AS PRACTICABLE IN PORTIONS OF THE SITE WHERE
CONSTRUCTION ACTIVITIES HAVE TEMPORARILY OR PERMANENTLY CEASED, BUT IN NO CASE MORE THAN
FOURTEEN (14) DAYS AFTER WORD HAS CEASED, EXCEPT AS STATED BELOW.
·WHERE STABILIZATION BY THE 14TH DAY IS PRECLUDED BY SNOW COVER OR FROZEN GROUND CONDITIONS
STABILIZATION MEASURES MUST BE INITIATED AS SOON AS PRACTICABLE.
·WHERE CONSTRUCTION ACTIVITY ON A PORTION OF THE SITE IS TEMPORARILY CEASED AND EARTH-DISTURBING
ACTIVITIES WILL BE RESUMED WITHIN 14 DAYS, TEMPORARY STABILIZATION MEASURES DO NOT HAVE TO BE
INITIATED ON THAT PORTION OF THE SITE.
3.ALL SEDIMENT AND EROSION CONTROL DEVICES SHALL BE INSPECTED ONCE EVERY CALENDAR WEEK. IF PERIODIC
INSPECTION OR OTHER INFORMATION INDICATES THAT A BMP HAS BEEN INAPPROPRIATELY, OR INCORRECTLY, THE
PERMITTEE MUST ADDRESS THE NECESSARY REPLACEMENT OR MODIFICATION REQUIRED TO CORRECT THE BMP
WITHIN 48 HOURS OF IDENTIFICATION.
4.PROVIDE SILT FENCE AND/OR OTHER CONTROL DEVICES, AS MAY BE REQUIRED, TO CONTROL SOIL EROSION
DURING UTILITY CONSTRUCTION. ALL DISTURBED AREAS SHALL BE CLEANED, GRADED, AND STABILIZED WITH
GRASSING IMMEDIATELY AFTER THE UTILITY INSTALLATION. FILL, COVER, AND TEMPORARY SEEDING AT THE END
OF EACH DAY ARE RECOMMENDED. IF WATER IS ENCOUNTERED WHILE TRENCHING, THE WATER SHOULD BE
FILTERED TO REMOVE SEDIMENT BEFORE BEING PUMPED BACK INTO ANY WATERS OF THE STATE.
5.ALL EROSION CONTROL DEVICES SHALL BE PROPERLY MAINTAINED DURING ALL PHASES OF CONSTRICTION UNTIL
THE COMPLETION OF ALL CONSTRUCTION ACTIVITIES AND ALL DISTURBED AREAS HAVE BEEN STABILIZED.
ADDITIONAL CONTROL DEVICES MAY BE REQUIRED DURING CONSTRUCTION IN ORDER TO CONTROL EROSION
AND/OR OFFSITE SEDIMENTATION. ALL TEMPORARY CONTROL DEVICES SHALL BE REMOVED ONCE CONSTRUCTION
IS COMPLETE AND THE SITE IS STABILIZED.
6.THE CONTRACTOR MUST TAKE NECESSARY ACTION TO MINIMIZE THE TRACKING OF MUD ONTO PAVED
ROADWAY(S) FROM CONSTRUCTION AREAS AND THE GENERATION OF DUST. THE CONTRACTOR SHALL DAILY
REMOVE MUD/SOIL FROM PAVEMENT, AS MAY BE REQUIRED.
7.TEMPORARY DIVERSION BERMS AND/OR DITCHES WILL BE PROVIDED AS NEEDED DURING CONSTRUCTION TO
PROTECT WORK AREAS FROM UPSLOPE RUNOFF AND/OR TO DIVERT SEDIMENT-LADEN WATER TO APPROPRIATE
TRAPS OR STABLE OUTLETS.
8.ALL WATERS OF THE STATE (WOS) , INCLUDING WETLANDS, ARE TO BE FLAGGED OR OTHERWISE CLEARLY MARKED
IN THE FIELD. A DOUBLE ROW OF SILT FENCE IS TO BE INSTALLED IN ALL AREAS WHERE A 50- FOOT BUFFER CANT BE
MAINTAINED BETWEEN THE DISTURBED AREA AND ALL WOS. A 10- FOOT BUFFER SHOULD BE MAINTAINED
BETWEEN THE LAST ROW OF SILT FENCE AND ALL WOS.
9.LITTER, CONSTRUCTION DEBRIS, OIL, FUELS, AND BUILDING PRODUCTS WITH SIGNIFICANT POTENTIAL FOR IMPACT
(SUCH AS STOCKPILES OF FRESHLY TREATED LUMBER) AND CONSTRUCTION CHEMICALS THAT COULD BE EXPOSED
TO STORM WATER MUST BE PREVENTED FROM BECOMING A POLLUTANT SOURCE IN STORM WATER DISCHARGES.
10.A COPY OF THE SWPPP, INSPECTIONS RECORDS, AND RAINFALL DATA MUST BE RETAINED AT THE CONSTRUCTION
SITE OR A NEARBY LOCATION EASILY ACCESSIBLE DURING NORMAL BUSINESS HOURS, FROM THE DATE OF
COMMENCEMENT OF CONSTRICTION ACTIVITIES TO THE DATE THAT FINAL STABILIZATION IS REACHED.
11.INITIATE STABILIZATION MEASURES ON ANY EXPOSED STEEP SLOPE (3H:1V OR GREATER) WHERE LAND-DISTURBING

12.MINIMIZE SOIL COMPACTION AND, UNLESS INFEASIBLE, PRESERVE TOPSOIL.

·SOAPS OR SOLVENTS USED IN VEHICLE AND EQUIPMENT WASHING.
16.AFTER CONSTRUCTION ACTIVITIES BEGIN, INSPECTIONS MUST BE CONDUCTED AT A MINIMUM OF AT LEAST ONCE
EVERY CALENDAR WEEK AND MUST BE CONDUCTED UNTIL FINAL STABILIZATION IS REACHED ON ALL AREAS OF THE
CONSTRUCTION SITE.
17.IF EXISTING BMPS NEED TO BE MODIFIED OR IF ADDITIONAL BMOS ARE NECESSARY TO COMPLY WITH THE
REQUIREMENTS OF THIS PERMIT AND/OR NC'S WATER QUALITY STANDARDS, IMPLEMENTATION MUST BE
COMPLETED BEFORE THE NEXT STORM EVENT WHENEVER PRACTICABLE. IF IMPLEMENTATION BEFORE THE NEXT
STORM EVENT IS IMPRACTICABLE, THE SITUATION MUST BE DOCUMENTED IN THE SWPPP AND ALTERNATIVE BMPS
MUST BE IMPLEMENTED AS SOON AS REASONABLY POSSIBLE.
18.A PRE-CONSTRUCTION CONFERENCE MUST BE HELD FOR EACH CONSTRUCTION SITE WITH AN APPROVED ON-SITE
SWPPP PRIOR TO THE IMPLEMENTATION OF CONSTRUCTION ACTIVITIES. FOR NON-LINEAR PROJECTS THAT
DISTURB 10 ACRES OR MORE THIS CONFERENCE MUST BE HELD ON-SITE UNLESS THE DEPARTMENT HAS APPROVED
OTHERWISE.
GENERAL STORM DRAINAGE NOTES:
1.ALL WORK AND MATERIALS SHALL CONFORM TO THE LATEST EDITION OF THE NCDOT STANDARD SPECIFICATIONS
UNLESS OTHERWISE SPECIFIED IN THIS MANUAL. ALL CONCRETE USED FOR DRAINAGE STRUCTURES SHALL HAVE A
MINIMUM COMPRESSIVE STRENGTH OF 3600 PSI AT 28 DAYS. THIS REQUIREMENT SHALL BE PROVIDED
REGARDLESS OF ANY LESSER COMPRESSIVE STRENGTH SPECIFIED IN THE NORTH CAROLINA DEPARTMENT OF
TRANSPORTATION STANDARD SPECIFICATIONS FOR ROADS AND STRUCTURES.
2.REINFORCED CONCRETE PIPE MAY BE USED IN ALL STORM DRAIN APPLICATIONS. HIGH DENSITY POLYETHYLENE
PIPE (HDPE) MAY BE SUBSTITUTED FOR PIPE DIAMETERS OF 48 INCHES OR LESS. CULVERTS 60 INCHES IN DIAMETER
OR GREATER MAY BE CORRUGATED ALUMINIZED METAL PIPE (CAMP) OR ALUMINUM WITH A MINIMUM 14 GAUGE
METAL.
3.ALL PIPE SHALL BE LAID WITH THE BELL OR GROOVE UPGRADE AND THE JOINT ENTIRELY INTERLOCKING.
4.THE MINIMUM COVER FOR ALL PIPES IS TWO (2) FEET MEASURED FROM THE FINAL SURFACE. SPECIAL
APPLICATIONS FOR LESS THAN TWO (2) FEET OF COVER WILL BE REVIEWED AND APPROVED BY THE CITY ENGINEER
INDIVIDUALLY. THE MAXIMUM COVER FOR STORM DRAINAGE PIPES SHALL AT A MINIMUM COMPLY WITH THE
REQUIREMENTS OF THE NORTH CAROLINA DEPARTMENT OF TRANSPORTATION HIGHWAY DESIGN BRANCH
ROADWAY DESIGN MANUAL, PART I, SECTION 5, AND "DRAINAGE DESIGN". STORM PIPE DESIGN THAT EXCEEDS
THESE CRITERIA MAY BE APPROVED AT THE DISCRETION OF THE CITY ENGINEER.
5.ALL PIPES IN STORM DRAIN STRUCTURES SHALL BE FLUSH WITH THE INSIDE WALL.
6.ALL STORM DRAIN STRUCTURES OVER THREE (3) FEET AND SIX (6) INCHES IN HEIGHT MUST HAVE STEPS IN
ACCORDANCE WITH STANDARD DETAILS SET FORTH IN THIS MANUAL.
7.THE INTERIOR SURFACES OF ALL STORM DRAINAGE STRUCTURES SHALL BE POINTED UP AND SMOOTHED TO AN
ACCEPTABLE STANDARD USING MORTAR MIXED TO MANUFACTURER'S SPECIFICATIONS.
8.STORM DRAINAGE PIPING SHALL BE PLACED IN A STRAIGHT ALIGNMENT AT UNIFORM GRADE. NO CHANGES IN
ALIGNMENT SHALL BE ALLOWED EXCEPT AT CATCH BASINS, MANHOLES, OR OTHER JUNCTIONS THAT PROVIDE
APPROPRIATE CLEAN OUT ACCESS. THE MAXIMUM LENGTH BETWEEN ACCESS POINTS IS 300 LINEAR FEET.
9.ALL FRAMES, GRATES, RINGS, COVERS, ETC., MUST CONFORM TO THE STANDARDS SET FORTH IN THIS MANUAL.
10.ALL GRADED CREEK BANKS AND SLOPES SHALL BE AT A MAXIMUM OF TWO (2) FEET HORIZONTAL TO ONE (1) FOOT
VERTICAL (2:1) AND NOT TO EXCEED 10' WITHOUT TERRACING OR THE SLOPES SHALL BE DESIGNED BY A
PROFESSIONAL GEOTECHNICAL ENGINEER AND APPROVED BY THE CITY ENGINEER ON A CASE BY CASE BASIS.
11.ALL CONCRETE SHALL BE AT LEAST 3600 PSI. PRIOR APPROVAL SHALL BE OBTAINED IN ORDER TO USE PRE-CAST
STORM DRAINAGE STRUCTURES IN ANY STREET RIGHT-OF-WAY BY CITY ENGINEER.
12.CONCRETE PIPE USED WITHIN THE STREET RIGHT-OF-WAY SHALL BE A MINIMUM OF CLASS III REINFORCED
CONCRETE PIPE, WITH AMINIMUM DIAMETER OF FIFTEEN (15) INCHES (EIGHTEEN (18) INCHES MINIMUM ON
CROSS DRAIN CULVERTS WITHIN THE ETJ). INSTALLATION OF CLASS IV OR HIGHER CONCRETE PIPE SHALL BE
IDENTIFIED ON THE AS-BUILT PLAN AND THE CITY INSPECTOR SHALL BE GIVEN DOCUMENTATION AND
NOTIFICATION OF THIS INFORMATION PRIOR TO CONSTRUCTION.
13.CONCRETE MORTAR JOINTS SHALL BE USED FOR JOINING ALL CONCRETE PIPES. THE PIPE SHALL BE CLEAN AND
MOIST WHEN MORTAR IS APPLIED. THE LOWER PORTIONS OF THE BELL OR GROOVE SHALL BE FILLED WITH
MORTAR SUFFICIENT TO BRING THE INNER SURFACE FLUSH AND EVEN WHEN THE NEXT JOINT IS FITTED INTO
PLACE. THE REMAINDER OF THE JOINT SHALL THEN BE FILLED WITH MORTAR AND A BEAD OR RING OF MORTAR
FORMED AROUND THE OUTSIDE OF THE JOINT. THE APPLICATION OF MORTAR MAY BE DELAYED UNTIL FILL IS
COMPLETED WHEN THE PIPE IS LARGER THAN THIRTY (30) INCHES.
14.PERFORMED JOINT SEALER, WHICH CONFORMS TO AASHTO SPECIFICATION M-198 FOR TYPE B FLEXIBLE PLASTIC
GASKETS, MAY BE USED IN LIEU OF THE MORTAR JOINING METHOD.
15.THE PRODUCT USED SHALL BE CORRUGATED EXTERIOR/SMOOTH INTERIOR PIPE (TYPE S), CONFORMING TO THE
REQUIREMENTS OF AASHTO SPECIFICATION M294 (LATEST EDITION) FOR CORRUGATED POLYETHYLENE PIPE.
16.BELL AND SPIGOT JOINTS SHALL BE REQUIRED ON ALL PIPES INSIDE THE RIGHT-OF-WAY. BELLS SHALL COVER AT
LEAST TWO FULL CORRUGATIONS ON EACH SECTION OF PIPE. THE BELL AND SPIGOT JOINT SHALL HAVE AN "O"
RING RUBBER GASKET MEETING ASTM F477 WITH THE GASKET FACTORY INSTALLED, PLACED ON THE SPIGOT END
OF THE PIPE. PIPE JOINTS SHALL MEET ALL REQUIREMENTS OF AASHTO M294.
17.ALL HDPE PIPE INSTALLED MUST BE INSPECTED AND APPROVED BY THE CITY'S INSPECTOR PRIOR TO ANY BACKFILL
BEING PLACED. THE CITY INSPECTOR MUST BE PRESENT DURING THE BACKFILLING OPERATION AS WELL.
18.BACKFILL MATERIAL USED TO INSTALL HDPE PIPE WITHIN THE STREET RIGHT-OF-WAY SHALL BE SELECT MATERIAL,
CLASS II-IV, AS DEFINED BY SECTION 1016-3 OF THE NORTH CAROLINA DEPARTMENT OF TRANSPORTATION
STANDARD SPECIFICATIONS FOR ROADS AND STRUCTURES. UPON SUBMITTAL OF WRITTEN CERTIFICATION OF
MATERIAL SUITABILITY BY A LICENSED GEOTECHNICAL ENGINEER, NCDOT CLASS I SELECT MATERIAL MAY BE USED.
ALL BACKFILL MATERIAL SHALL BE APPROVED BY THE CITY INSPECTOR PRIOR TO PLACEMENT OF THE MATERIAL
WITHIN THE STREET RIGHT-OF-WAY.
19.THE MINIMUM LENGTH OF HDPE PIPE PERMITTED FOR USE SHALL BE FOUR (4) FEET. HDPE FLARED END SECTIONS
ARE NOT ALLOWED.
20.ALL HDPE PIPE INSTALLED SHALL BE THIRD PARTY CERTIFIED AND SHALL BEAR THE PLASTIC PIPE INSTITUTE'S (PPI)
CERTIFICATE STICKER.
21.ALL BACKFILL SHALL BE NON-PLASTIC IN NATURE, FREE FROM ROOTS, VEGETATIVE MATTER, WASTE,
CONSTRUCTION MATERIAL OR OTHER OBJECTIONABLE MATERIAL. SAID MATERIAL SHALL BE CAPABLE OF BEING
COMPACTED BY MECHANICAL MEANS AND SHALL HAVE NO TENDENCY TO FLOW OR BEHAVE IN A PLASTIC MANNER
UNDER THE TAMPING BLOWS OR PROOF ROLLING.
22.MATERIALS DEEMED BY THE ENGINEER AS UNSUITABLE FOR BACKFILL PURPOSES SHALL BE REMOVED AND
REPLACED WITH SELECT BACKFILL MATERIAL.
23.BACKFILLING OF TRENCHES SHALL BE ACCOMPLISHED IMMEDIATELY AFTER THE PIPE IS LAID. THE FILL AROUND THE
PIPE SHALL BE PLACED IN LAYERS NOT TO EXCEED EIGHT (8) INCHES, EACH LAYER SHALL BE THOROUGHLY
COMPACTED TO 95% OF THE MAXIMUM DENSITY OBTAINABLE WITH THE STANDARD PROCTOR TEST (A DENSITY OF
100% STANDARD PROCTOR IS REQUIRED FOR THE TOP EIGHT (8) INCHES).
24.COMPACTION REQUIREMENTS SHALL BE ATTAINED BY THE USE OF MECHANICAL COMPACTION METHODS. EACH
LAYER OF BACKFILL SHALL BE PLACED LOOSE AND THOROUGHLY COMPACTED IN PLACE.
25.UNDER NO CIRCUMSTANCES SHALL WATER BE PERMITTED TO RISE IN UN-BACKFILLED TRENCHES AFTER THE PIPE
HAS BEEN PLACED.

GENERAL NOTES
1.PRIOR TO CONSTRUCTION, ANY DISCREPANCIES IN THE PLANS AND NOTES SHALL BE BROUGHT TO THE DESIGN ENGINEER'S ATTENTION
IMMEDIATELY.
2.THE FINAL CERTIFICATION FOR THIS FACILITY WILL INCLUDE A CERTIFICATION BY THE ON-SITE GEOTECHNICAL ENGINEER THAT THE
PROJECT WAS CONSTRUCTED PER THE APPROVED PLANS. IT IS THE RESPONSIBILITY OF THE CONTRACTOR TO COORDINATE WITH THE
ON-SITE GEOTECHNICAL ENGINEER FOR OBSERVATION AND TESTING SUCH THAT THE ON-SITE GEOTECHNICAL ENGINEER CAN CERTIFY THE
CONSTRUCTION.
3.ALL CONSTRUCTION ACTIVITY RELATED TO THE PROPOSED STORMWATER MANAGEMENT FACILITY SHALL BE PER THE DETAILS AND
SPECIFICATIONS SHOWN IN THESE DRAWINGS. SOILS, COMPACTION, AND OTHER MISCELLANEOUS DETAILS AND SPECIFICATIONS MAY BE
MODIFIED PER THE RECOMMENDATIONS OF THE ON-SITE GEOTECHNICAL ENGINEER. HOWEVER, PRIOR TO IMPLEMENTATION, THE
DESIGN ENGINEER SHALL BE NOTIFIED OF ANY DEVIATION FROM THESE DESIGN DRAWINGS, INCLUDING SHOP DRAWINGS FOR ANY
PROPOSED MODIFICATION.
4.DURING THE INITIAL STAGES OF CONSTRUCTION, THE STORMWATER MANAGEMENT FACILITY MAY BE USED AS A SEDIMENT BASIN FOR
EROSION CONTROL PURPOSES. IF SO, THE CONTRACTOR SHALL FOLLOW THE GENERAL CONSTRUCTION SEQUENCE BELOW:
4.1.THE CONTRACTOR SHALL CONSTRUCT THE ENTIRE FACILITY (PERMANENT OUTLET STRUCTURE, DAM, ETC.) WITH THE EXCEPTION OF
THE INTERIOR FINE GRADING FOR THE FACILITY. THE INTERIOR FINE GRADING WILL BE CONSTRUCTED ONCE THE EROSION CONTROL
PHASE IS COMPLETE.
4.2.THE TEMPORARY DRAW DOWN RISER (OR SKIMMER) SHALL BE CONNECTED TO THE PERMANENT 6"Ø UNDERDRAIN CONNECTION.
4.3.ONCE THE UPSTREAM DRAINAGE AREA IS STABILIZED AND THE EROSION CONTROL INSPECTOR APPROVES THE REMOVAL OF THE
SEDIMENT BASIN, THE CONTRACTOR SHALL REMOVE THE TEMPORARY DRAW DOWN RISER (OR SKIMMER) AND CLEAN-OUT THE
BASIN. ALL SEDIMENT, TRASH, ETC. SHALL BE DISPOSED OF PROPERLY (I.E. - PLACED IN A LANDFILL) AND NOT STOCKPILED IN AN
AREA WHERE WATER QUALITY COULD BE ADVERSELY AFFECTED.
4.4.ONCE THE BASIN IS CLEANED OUT, AND ALL EROSION CONTROL DEVICES REMOVED, THE CONTRACTOR SHALL CONSTRUCT THE
INTERIOR GRADING SHOWN ON THIS SHEET.
4.5.ONCE THE GRADING IS COMPLETE, THE CONTRACTOR SHALL REQUEST AN ON-SITE INSPECTION AND AN AS-BUILT SURVEY PRIOR TO
INSTALLATION OF THE STORMWATER MANAGEMENT FACILITY PLANTS. IF THE CONTRACTOR PLANTS THE PROPOSED VEGETATION
PRIOR TO AN AS-BUILT SURVEY (AND SUBSEQUENT APPROVAL), ANY CHANGES TO THE GRADING / RE-PLANTING OF PLANTS WILL BE
AT THE CONTRACTOR'S EXPENSE.
4.6.ONCE THE ENGINEER HAS APPROVED THE AS-BUILT GRADING, THE CONTRACTOR SHALL PLANT THE PROPOSED STORMWATER
MANAGEMENT FACILITY PLANTS SHOWN ON THE LANDSCAPE PLAN FOR THE FACILITY. AFTER COMPLETION OF THE PLANTING, THE
LANDSCAPE CONTRACTOR SHALL PROVIDE A LETTER TO THE ENGINEER CERTIFYING THAT THE PLANTS HAVE BEEN INSTALLED PER
THE APPROVED STORMWATER MANAGEMENT FACILITY PLANTING PLAN.
5.THE RETAINING WALL ALIGNMENT SHOWN ON THESE PLANS DEPICTS THE LOCATION OF THE FRONT FACE OF THE RETAINING WALL AT
THE BOTTOM.
6.THE RETAINING WALL IS TO BE A DESIGN-BUILD PROJECT(S) BY THE CONTRACTOR. IT SHALL BE THE CONTRACTOR'S RESPONSIBILITY TO
OBTAIN FINAL CONSTRUCTION DRAWINGS FROM A REGISTERED PROFESSIONAL ENGINEER AND GAIN ALL REQUIRED PERMITS NECESSARY
FOR THE CONSTRUCTION OF THE RETAINING WALL.
7.THE RETAINING WALL SHALL BE ASSUMED TO BE BACKFILLED WITH OFF-SITE BORROW MATERIAL OR PROCESSED FILL UNLESS
CONTRACTOR CAN PROVIDE OWNER WITH CONFIRMATION FROM THE GEOTECHNICAL ENGINEER AND THE RETAINING WALL DESIGNER
THAT READILY AVAILABLE ON-SITE SOILS CAN BE USED.
8.THE TOP AND BOTTOM OF WALL ELEVATIONS SHOWN ON THESE PLANS IDENTIFY FINISHED GRADE ELEVATIONS ONLY. THE EXTENT THAT
THE RETAINING WALL WILL BE EXTENDED BELOW GRADE TO THE FOOTING SHALL BE IDENTIFIED ON THE RETAINING WALL CONSTRUCTION
DRAWINGS (BY OTHERS).
9.PRIOR TO CONSTRUCTION, THE ONSITE GEOTECHNICAL ENGINEER SHALL VERIFY THE FILTRATION AND PERMEABILITY ASPECTS OF THE
PROPOSED SAND FILTER MATERIALS. SPECIAL ATTENTION SHALL BE PAID TO PREVENT MIGRATION OF FINER SOIL PARTICLES THROUGH
UNDERLYING GRAVEL LAYERS.
10.ONCE CONSTRUCTED, THE SAND FILTER SHALL NOT RECEIVE STORMWATER RUNOFF UNTIL THE ENTIRE CONTRIBUTING DRAINAGE AREA
TO THE SAND FILTER HAS BEEN COMPLETELY STABILIZED AND SITE CONSTRUCTION IS COMPLETE.
11.PRIOR TO PLACING ANY MEDIA (I.E. UNDERDRAINS, GRAVEL, OR SAND) WITHIN THE SAND FILTER, THE CONTRACTOR SHALL REQUEST AN
ONSITE MEETING WITH THE DESIGN ENGINEER TO ENSURE THE UPSTREAM DRAINAGE AREA IS COMPLETELY STABILIZED (I.E. GOOD
VEGETATIVE COVER). IF THE CONTRACTOR DECIDES TO PLACE THE MEDIA FOR THE SAND FILTER PRIOR TO APPROVAL FROM THE DESIGN
ENGINEER, THEN THE CONTRACTOR SHALL EXCAVATE/REPLACE, AS NECESSARY, THE MEDIA NEEDED FOR THE SYSTEM TO FUNCTION
PROPERLY AT HIS / HER EXPENSE SHOULD THE SAND FILTER NOT FUNCTION PROPERLY (I.E. WILL NOT DRAIN DUE TO SEDIMENT
DEPOSITION) DUE TO AN UNSTABILIZED UPSTREAM DRAINAGE AREA.
12.ANY REMOVED TOPSOIL SHALL BE STOCKPILED FOR USE IN PLANTING (SEEDING) ON THE DAM EMBANKMENT ONCE FINAL GRADES (AS
SHOWN ON THE GRADING PLAN) HAVE BEEN ESTABLISHED WITH COMPACTED FILL. PRIOR TO TOPSOIL INSTALLATION, THE CONTRACTOR
SHALL SCARIFY THE THE TOP 2"-3" OF THE BERM SECTION TO PROMOTE BONDING OF THE TOPSOIL WITH THE COMPACTED FILL. THE
TOPSOIL DEPTH SHALL RANGE FROM 3"-4" ON THE DAM EMBANKMENT. PLEASE NOTE THE STOCKPILED TOPSOIL SHALL BE AMENDED, AS
DIRECTED BY A LANDSCAPE PROFESSIONAL, PRIOR TO INSTALLATION ON THE EMBANKMENT.
13.IT IS ANTICIPATED THAT DEWATERING WILL BE NECESSARY IN THE EXCAVATION AREAS (E.G. - EMBANKMENT SUB GRADE, INTERIOR
PORTIONS OF THE STORMWATER FACILITY, KEY TRENCH, ETC.). THEREFORE, THE CONTRACTOR SHALL FURNISH, INSTALL, OPERATE, AND
MAINTAIN ANY PUMPING EQUIPMENT, ETC. NEEDED FOR REMOVAL OF WATER FROM VARIOUS PARTS OF THE STORMWATER FACILITY
SITE. DURING PLACEMENT OF FILL WITHIN THESE AREAS, THE CONTRACTOR SHALL KEEP THE WATER LEVEL BELOW THE BOTTOM OF THE
EXCAVATION / CONSTRUCTION AREAS. THE MANNER IN WHICH THE WATER IS REMOVED SHALL BE SUCH THAT THE EXCAVATION BOTTOM
AND SIDE SLOPES ARE STABLE, WITH NO SEDIMENT DISCHARGED FROM THE SITE (I.E. PUMPED WATER MAY NEED TO BE DIRECTED TO AN
APPROVED EROSION CONTROL DEVICE SUCH AS A DIRT BAG (ACF ENVIRONMENTAL), OR ENGINEER APPROVED EQUIVALENT, PRIOR TO
DISCHARGE).
14.THE BEDDING MATERIAL FOR THE STORMWATER CONTROL MEASURE SHALL BE SPECIFIED BY THE ON-SITE GEOTECHNICAL ENGINEER.
TYPICALLY, A WELL-GRADED GRANULAR MATERIAL WILL BE USED FOR THE BEDDING. PLEASE NOTE THAT IF CONSTRUCTION EQUIPMENT
WILL BE OPERATING FOR AN EXTENDED PERIOD OF TIME ON THE BEDDING, THEN THE APPROPRIATE MEASURES (E.G. ENGINEERED
FABRIC, STIFF GEOGRID, ETC.) SHALL BE TAKEN TO ENSURE THE INTEGRITY OF THE BEDDING IS NOT COMPROMISED.
OUTLET STRUCTURE MATERIAL SPECIFICATIONS
1.THE 15"Ø RCP OUTLET BARREL SHALL BE CLASS III RCP, MODIFIED BELL AND SPIGOT, MEETING THE REQUIREMENTS OF ASTM C76-LATEST.
THE PIPES SHALL HAVE CONFINED O-RING RUBBER GASKET JOINTS MEETING ASTM C-443-LATEST. THE PIPE JOINTS SHALL BE TYPE R-4.
2.THE STRUCTURAL DESIGN FOR THE 4' X 4' (INTERNAL DIMENSIONS) RISER BOX WITH EXTENDED BASE SHALL BE BY OTHERS. PRIOR TO
ORDERING THE STRUCTURES, THE CONTRACTOR SHALL PROVIDE, TO THE DESIGN ENGINEER FOR REVIEW, SHOP DRAWINGS AND
SUPPORTING STRUCTURAL CALCULATIONS SEALED BY A P.E. REGISTERED IN NORTH CAROLINA DEMONSTRATING THE PERTINENT VERTICAL
LOADS ARE SUPPORTED BY THE CONCRETE RISER STRUCTURE.
3.THE RISER BOX OUTLET STRUCTURE SHALL BE PROVIDED WITH STEPS 16" ON CENTER. STEPS SHALL BE PROVIDED ON THE INNER WALL OF
THE RISER BOX. STEPS SHALL BE IN ACCORDANCE WITH NCDOT STD. 840.66. SEE SHEET C9.02. PLEASE REFER TO SHEET C9.02 FOR LOCATION
OF THE RISER STEPS.
4.THE CONCRETE ANTI-FLOTATION BLOCKS SHALL BE PRECAST AS THE EXTENDED BASE OF THE RISER BOX DURING FABRICATION. THE
PRECAST BASE SHALL BE INCLUDED AS PART OF THE SHOP DRAWINGS THAT WILL BE SUBMITTED TO THE ENGINEER FOR REVIEW (SEE ITEM
2 ABOVE). IN LIEU OF A PRE-CAST BASE, THE CONTRACTOR MAY OPT TO CAST-IN-PLACE THE ANTI-FLOTATION BLOCK IN THE FIELD.
HOWEVER, PRIOR TO CONSTRUCTING THE CAST-IN-PLACE BASE, THE CONTRACTOR SHALL SUBMIT SHOP DRAWINGS TO THE ENGINEER FOR
REVIEW DENOTING THE FOLLOWING:
A.STEEL REINFORCEMENT
B.THE CONNECTION MECHANISM TO JOIN THE ANTI-FLOTATION BLOCK WITH THE RISER SECTION
5.IF THE ANTI-FLOTATION BLOCK IS PRECAST AS PART OF THE RISER BASE SECTION, THE ENTIRE PRECAST RISER BOX STRUCTURE SHALL HAVE
A SHIPPING WEIGHT OF 14,316 LBS. THE STRUCTURE WEIGHT SHALL BE THE SHIPPING WEIGHT AND SHALL BE DETERMINED BY
SUBTRACTING THE WEIGHT OF THE FACTORY BLOCK OUT FROM THE GROSS STRUCTURE WEIGHT. THIS INFORMATION SHALL BE SHOWN ON
THE SHOP DRAWINGS SUBMITTED TO THE ENGINEER FOR REVIEW. IF THE CONTRACTOR OPTS TO CAST-IN-PLACE THE ANTI-FLOTATION
BLOCK, THE CONTRACTOR SHALL ENSURE THE WEIGHT OF THE ENTIRE RISER STRUCTURE IS EQUAL TO THE MINIMUM LISTED ABOVE.
6.THE RISER BOX JOINTS SHALL BE SEALED USING BUTYL RUBBER SEALANT CONFORMING TO ASTM-C990-LATEST. IF NECESSARY, THE
CONTRACTOR SHALL INCORPORATE A WATERSTOP INTO THE RISER BOX JOINT TO ENSURE A WATERTIGHT CONNECTION. THE CONTRACTOR
SHALL PARGE JOINTS ON BOTH THE INSIDE AND OUTSIDE WITH NON-SHRINK GROUT.
7.PRIOR TO ORDERING, THE CONTRACTOR SHALL SUBMIT TRASH RACK SHOP DRAWINGS TO THE ENGINEER FOR REVIEW. CONTRACTOR SHALL
ENSURE THAT AN ACCESS HATCH IS PROVIDED WITHIN THE TRASH RACK (SEE DETAIL FOR LOCATION) THAT WILL ALLOW FOR FUTURE
MAINTENANCE ACCESS. CONTRACTOR SHALL ALSO PROVIDE A CHAIN AND LOCK FOR SECURING THE ACCESS HATCH. NOTE THE ACCESS
HATCH SHALL LINE UP WITH THE ACCESS STEPS AFTER INSTALLATION.

2.ALL FILL MATERIALS TO BE USED FOR THE DAM EMBANKMENT SHALL BE TAKEN FROM BORROW AREAS APPROVED BY THE ON-SITE
GEOTECHNICAL ENGINEER. THE FILL MATERIAL SHALL BE FREE FROM ROOTS, STUMPS, WOOD, STONES GREATER THAN 6", AND FROZEN
OR OTHER OBJECTIONABLE MATERIAL. THE FOLLOWING SOIL TYPES ARE SUITABLE FOR USE AS FILL WITHIN THE DAM EMBANKMENT AND
KEY TRENCH: ML AND CL.
3.FILL PLACEMENT FOR THE EMBANKMENT SHALL NOT EXCEED A MAXIMUM 8" LIFT (UNCOMPACTED). EACH LIFT SHALL BE CONTINUOUS
FOR THE ENTIRE LENGTH OF EMBANKMENT. BEFORE PLACEMENT OF FILL FOR THE BERM SECTION, ALL UNSUITABLE MATERIAL SHALL BE
REMOVED AND THE SURFACE PROPERLY PREPARED FOR FILL PLACEMENT. FILL MATERIAL ADJACENT TO ALL SPILLWAY AND DRAINAGE
STRUCTURES SHALL BE PLACED IN 4-INCH (UNCOMPACTED) LIFTS AND HAND-COMPACTED TO THE SAME COMPACTION AND MOISTURE
REQUIREMENTS AS THE ENTIRE EMBANKMENT.
4.ALL FILL SOILS USED IN THE EMBANKMENT CONSTRUCTION SHALL BE COMPACTED TO AT LEAST 95% OF THE STANDARD PROCTOR
MAXIMUM DRY DENSITY (ASTM-698). THE FILL SOILS SHALL BE COMPACTED AT A MOISTURE CONTENT WITHIN -1 TO +3 PERCENT OF ITS
OPTIMUM MOISTURE CONTENT. COMPACTION TESTS SHALL BE PERFORMED BY THE ON-SITE GEOTECHNICAL ENGINEER DURING
CONSTRUCTION TO VERIFY THAT THE PROPER COMPACTION LEVEL HAS BEEN REACHED. THE FILL SHOULD BE COMPACTED USING A
SHEEPSFOOT TYPE COMPACTOR. IN ORDER TO PREVENT DAMAGE TO THE PIPE, NO COMPACTION EQUIPMENT SHALL CROSS ANY PIPE
UNTIL MINIMUM COVER IS ESTABLISHED ALONG THE PIPE.
5.THE DESIGN ENGINEER SHALL BE PROVIDED WITH REPORTS AND CERTIFICATION, BY THE ON-SITE GEOTECHNICAL ENGINEER, THAT THE
GEOTECHNICAL ASPECTS OF THE FACILITY HAVE BEEN CONSTRUCTED PER PLAN. THESE REPORTS AND CERTIFICATION WILL BE NEEDED
DURING THE AS-BUILT CERTIFICATION PROCESS FOR THIS STORMWATER FACILITY. THEREFORE, IT SHALL BE THE CONTRACTOR'S
RESPONSIBILITY TO COORDINATE TESTING AND OBSERVATION WITH THE ON-SITE GEOTECHNICAL ENGINEER.
6.TESTING OF THE NEW FILL MATERIALS SHALL BE PERFORMED TO VERIFY THAT THE RECOMMENDED LEVEL OF COMPACTION IS ACHIEVED
DURING CONSTRUCTION. THEREFORE, ONE DENSITY TEST SHALL BE PERFORMED FOR EVERY 2,500 SQUARE FEET OF AREA FOR EVERY LIFT
OF FILL OR AS RECOMMENDED BY THE ON-SITE GEOTECHNICAL ENGINEER.
7.TESTING WILL BE REQUIRED ALONG THE 15"Ø RCP OUTLET BARREL AT A FREQUENCY OF ONE TEST PER 25 LF OF PIPE PER VERTICAL FOOT
OF FILL OR AS DIRECTED BY THE ON-SITE GEOTECHNICAL ENGINEER.
STATEMENT OF RESPONSIBILITY
1.ALL REQUIRED MAINTENANCE AND INSPECTIONS OF THE STORMWATER MANAGEMENT FACILITIES SHALL BE THE RESPONSIBILITY OF THE
OWNER, PER THE EXECUTED OPERATION AND MAINTENANCE AGREEMENT FOR THIS FACILITY.
CONSTRUCTION SEQUENCE
1.PRIOR TO CONSTRUCTION, THE OWNER SHALL OBTAIN A LAND DISTURBING (GRADING) PERMIT AND AN "APPROVAL TO CONSTRUCT" FROM
ROWAN COUNTY AND ALL OTHER NECESSARY PERMITS FROM APPLICABLE AGENCIES.
2.INSTALL ALL SEDIMENT AND EROSION CONTROL MEASURES PER THE APPROVED SEDIMENT AND EROSION CONTROL PLAN. THE
CONTRACTOR SHALL MAINTAIN ALL APPROVED SEDIMENT AND EROSION CONTROL MEASURES THROUGHOUT THE ENTIRE PROJECT, AS
REQUIRED. THE CONTRACTOR SHALL RECEIVE APPROVAL FROM THE EROSION CONTROL INSPECTOR, AS REQUIRED BY GOVERNING
AGENCIES, PRIOR TO ANY CLEARING.
3.CLEAR AND GRUB AREA WITHIN THE LIMITS OF THE PROPOSED DAM CONSTRUCTION. ALL TREES AND THEIR ENTIRE ROOT SYSTEMS MUST
BE REMOVED FROM THE DAM FOOTPRINT AREA AND BACKFILLED WITH SUITABLE SOIL MATERIAL. THE BACKFILLED AREAS SHALL BE
COMPACTED TO THE SAME STANDARDS AS THE DAM EMBANKMENT. THE REMAINING AREA OF THE EMBANKMENT SHALL BE STRIPPED TO
A SUITABLE DEPTH AS DIRECTED BY THE ON-SITE GEOTECHNICAL ENGINEER. ANY RESIDUAL SOILS TO BE LEFT IN PLACE MUST BE WELL
SCARIFIED TO PROMOTE BONDING OF THE NEW EMBANKMENT FILL. NO EMBANKMENT MATERIAL SHALL BE PLACED FOR THE DAM OR KEY
TRENCH UNTIL APPROVAL OF THE DAM SUBGRADE / TRENCH IS OBTAINED FROM THE ON-SITE GEOTECHNICAL ENGINEER.
4.EXCAVATE FOR THE NEW KEY TRENCH ALONG THE CENTERLINE OF THE PROPOSED DAM EMBANKMENT. THE TRENCH SHALL EXTEND A
MINIMUM OF 5 FT BELOW EXISTING GRADE OR 2 FT BELOW THE CONCRETE CRADLE AND SHALL HAVE A MINIMUM BOTTOM WIDTH OF 5
FEET. THE KEY TRENCH SIDESLOPES SHALL BE A MINIMUM OF 1:1 (H:V). THE KEY TRENCH SHALL BE COMPACTED TO THE SAME
SPECIFICATION LISTED IN ITEM 4 OF THE SECTION TITLED "BERM AND SOIL COMPACTION SPECIFICATIONS." DEPENDING UPON ON-SITE SOIL
CONDITIONS ENCOUNTERED DURING EXCAVATION, THE ON-SITE GEOTECHNICAL ENGINEER MAY VARY THE DEPTH AND DIMENSIONS OF
THE KEY TRENCH AS DEEMED NECESSARY. THE ON-SITE GEOTECHNICAL ENGINEER SHALL RETAIN DOCUMENTATION OF ANY VARIATION FOR
FUTURE AS-BUILT SUBMITTALS TO THE NCDEQ.
5.BEGIN PLACEMENT OF BACKFILL WITHIN THE KEY TRENCH. THE KEY TRENCH SHALL BE COMPACTED TO THE SPECIFICATIONS LISTED ITEM 4
OF THE SECTION TITLED "BERM AND SOIL COMPACTION SPECIFICATIONS." THE KEY TRENCH SHALL BE TESTED PER THE SPECIFICATIONS
LISTED IN THAT SECTION.
6.PRIOR TO INSTALLATION, SUBGRADE CONDITIONS ALONG THE SPILLWAY PIPES SHOULD BE EVALUATED BY THE ON-SITE GEOTECHNICAL
ENGINEER TO ASSESS WHETHER SUITABLE BEARING CONDITIONS EXIST AT THE SUBGRADE LEVEL. SHOULD SOFT OR OTHERWISE
UNSUITABLE CONDITIONS BE ENCOUNTERED ALONG THE PIPE ALIGNMENTS, THESE MATERIALS SHOULD BE UNDERCUT AS DIRECTED BY THE
GEOTECHNICAL ENGINEER. THE UNDERCUT MATERIALS SHALL BE REPLACED WITH ADEQUATELY COMPACTED STRUCTURAL FILL, LEAN
CONCRETE OR FLOWABLE FILL AS DIRECTED BY THE ON-SITE GEOTECHNICAL ENGINEER.
7.IN ORDER TO HELP PROTECT THE SOIL SUBGRADE FROM DETERIORATION (DUE TO EXPOSURE, RAINFALL, SEEPAGE, AND RUNOFF) BEFORE
THE CRADLE CAN BE POURED, IT IS STRONGLY RECOMMENDED THAT A 3" TO 4" THICK CONCRETE MUD MAT BE POURED OVER THE
SUBGRADE ONCE IT IS APPROVED BY THE ON-SITE GEOTECHNICAL ENGINEER. THE MUD MAT WILL ALSO PROVIDE BEARING FOR THE BLOCKS
THAT TEMPORARILY SUPPORT THE SPILLWAY PIPE UNTIL THE CRADLE CAN BE POURED. THE METHOD OF BLOCK SUPPORT FOR THE PIPE
PROPOSED BY THE CONTRACTOR SHOULD BE SUBMITTED TO THE JOHN R. McADAMS COMPANY FOR REVIEW.
8.BEGIN CONSTRUCTION OF THE NEW EMBANKMENT. FILL MATERIALS SHALL BE PLACED IN MAXIMUM 8" THICK LIFTS PRIOR TO
COMPACTION, UNLESS DIRECTED OTHERWISE BY THE ON-SITE GEOTECHNICAL ENGINEER. FILL LIFTS SHALL BE CONTINUOUS OVER THE
ENTIRE LENGTH OF FILL. IF IT IS NECESSARY, THE EMBANKMENT FILL MATERIAL WILL BE OVERBUILT IN HORIZONTAL LIFTS AND CUT BACK TO
FINAL GRADE IN ORDER TO ACHIEVE PROPER COMPACTION.
9.AS CONSTRUCTION OF THE EMBANKMENT MOVES FORWARD, IT WILL BE NECESSARY TO INSTALL THE CONCRETE CRADLE. THIS MAY BE
CONSTRUCTED USING ONE OF THE FOLLOWING METHODS:
9.1.IF THE PROPOSED STRUCTURAL FILL MATERIAL IS UTILIZED AS THE FORMWORK FOR THE CONCRETE CRADLE, THEN THE STRUCTURAL
FILL SHOULD BE INSTALLED AND COMPACTED UP TO THE TOP OF CONCRETE CRADLE ELEVATION. ONCE THE STRUCTURAL FILL
REACHES THE NEXT DOWNSTREAM JUNCTION BOX OR HEADWALL AND IS COMPACTED TO THE ELEVATION OF THE TOP OF THE
CONCRETE CRADLE, EXCAVATE THE CONCRETE CRADLE TRENCH PER THE PROVIDED DETAILS AND CONSTRUCT THE CONCRETE CRADLE
AS PER THE PROVIDED CONCRETE CRADLE DETAIL.
9.2.IF THE PROPOSED STRUCTURAL FILL IS NOT UTILIZED AS THE FORMWORK FOR THE CONCRETE CRADLE, THEN PRIOR TO
CONSTRUCTING THE STRUCTURAL FILL EMBANKMENT, THE FORMWORK FOR THE CONCRETE CRADLE SHOULD BE INSTALLED ON
EXISTING GROUND AND / OR THE MUD MAT. THE CONCRETE CRADLE SHALL BE CONSTRUCTED PER THE PROVIDED DETAILS.
10.INSTALL RISER / BARREL ASSEMBLY, ALONG WITH THE EMERGENCY DRAIN SYSTEM. THE DRAIN VALVE LOCATED WITHIN THE RISER BOX
SHALL BE KEPT OPEN UNTIL AN AS-BUILT CERTIFICATION HAS BEEN COMPLETED BY THE ENGINEER AND AN APPROVAL TO IMPOUND HAS
BEEN ISSUED BY ALL APPLICABLE AGENCIES.
11.INSTALL 15" RCP OUTLET BARREL SPILLWAY FILTER FROM THE DETAILS SHOWN ON SHEET C9.03.
12.CONSTRUCT EMBANKMENT PER SPECIFICATIONS LISTED IN THE SECTION TITLED "BERM AND SOIL COMPACTION SPECIFICATIONS" AND
REQUIREMENTS OF THE ON-SITE GEOTECHNICAL ENGINEER. ALL CHARACTERISTICS OF THE EMBANKMENT FILL MATERIAL SHALL MEET THE
STANDARDS SET FORTH IN "BERM AND SOIL COMPACTION SPECIFICATIONS", INCLUDING COMPACTION AND MOISTURE REQUIREMENTS. IF
NECESSARY TO ACHIEVE PROPER COMPACTION, THE EMBANKMENT FILL MATERIAL WILL BE OVERBUILT IN HORIZONTAL LIFTS AND CUT
BACK TO PROPER FINAL GRADE. ANY HAND COMPACTION ACTIVITIES AROUND SPILLWAY OR DRAIN STRUCTURES SHALL BE CONDUCTED IN
4-INCH LOOSE LIFTS AND BE TO THE SAME COMPACTION AND MOISTURE REQUIREMENTS AS THE ENTIRE EMBANKMENT. ALL COMPACTION
AND MOISTURE TESTING SHALL BE CARRIED OUT AS DIRECTED BY THE ON-SITE GEOTECHNICAL ENGINEER AND AS LISTED IN THE SECTION
TITLED "BERM AND SOIL COMPACTION SPECIFICATIONS".
4.UPON COMPLETION OF DAM EMBANKMENT, PROMPTLY STABILIZE AND SEED DAM EMBANKMENT PER SEEDING SCHEDULE.
5.SCHEDULE A FINAL AS-BUILT INSPECTION AND AS-BUILT SURVEY WITH THE ENGINEER. AN AS-BUILT INSPECTION SHOULD BE SCHEDULED A
MINIMUM OF 60 DAYS BEFORE A PERMIT TO IMPOUND IS REQUIRED. ANY COMMENTS OR DEFICIENCIES IN THE DAM CONSTRUCTION MUST
BE CORRECTED TO THE SATISFACTION OF THE ENGINEER AND OWNER BEFORE CERTIFICATION SHALL BE GRANTED. UPON FINAL APPROVAL
FROM NCDEQ, CLOSE THE 8"Ø BOTTOM DRAIN VALVE IN THE RISER BOX AND BEGIN IMPOUNDING WATER. NO WATER SHALL BE

AREA UNDER PIPE AND PIPE HAUNCHES AS TO LEAVE NO VOIDS UNDER THE PIPE BEFORE POURING ON THE OPPOSITE SIDE
OF THE TRENCH. POUR ENTIRE CRADLE AS ONE LIFT (VERTICALLY) PER DRAWINGS.
6. CONCRETE SHALL BE FINISHED "ROUGH" SO AS TO ACHIEVE A MORE ADEQUATE BOND BETWEEN THE CONCRETE IN THE
CRADLE AND THE FIRST LIFT OF STRUCTURAL FILL.
7. ALLOW CRADLE TO CURE FOR A MINIMUM OF 7 DAYS BEFORE ANY VIBRATING COMPACTION EQUIPMENT IS USED IN
