However, additional information is required before the review may be completed. Please address the items on the attached pages no later than the close of business on February 18, 2021. Please be aware that you are responsible for meeting all requirements set forth in North Carolina rules and regulations. Any oversights that occurred in the review of the subject application package are still the Applicant's responsibility. In addition, any omissions made in responding to the outstanding items in Sections A and B, or failure to provide the additional information on or before the above requested date may result in your application being returned as incomplete. Please reference the subject application number when providing the requested information. All revised and/or additional documentation shall be signed, sealed and dated (where needed), with two paper copies and one electronic copy submitted to my attention at the address below. Application: 1. Section III.1. of the Non -Discharge System Renewal form (FORM: NDSR 01-20) requires a site map in accordance with 15A NCAC 02T .0105(d) that provides the following information: a. Legend, north arrow, and scale b. Distance measurements verifying all setbacks are being met c. Stormwater drainage controls d. Location and ownership of property boundaries within 500 feet of the disposal area (including road/rail right-of-ways) e. All facility -related structures and fences within the treatment, storage, and disposal areas f. All habitable residences and places of assembly within 500 feet of all treatment, storage, and disposal sites g. Topographic contour intervals not exceeding 10 feet or 25 percent of total site relief h. Soil mapping units on all disposal sites i. Location of all wells, streams (ephemeral, intermittent, and perennial), springs, lakes, ponds, ditches, and other surface drainage features within 500 feet of all waste treatment, storage, and disposal site(s) j. Location and identification of each monitoring well (identify any background/upgradient wells) k. Latitude and longitude coordinates of all monitoring wells (decimal degrees to 6 digits) 1. Location and identification of major components of the waste disposal system in. Identification of all storage or treatment earthen structures that are part of the permit n. The wetted perimeter of all irrigation/infiltration fields with field names (named according to the approved permit) o. The delineation of compliance and review boundaries p. 100-year floodplain (if applicable) The site map submitted does not show the full permitted system, including, but not limited to the location of the treatment plant, location of the components of the 500,000 gallon stormwater collection and disposal facility, and the labeling of the rotary distributor infiltration beds. It also appears that the property lines are outdated on this map. Please submit a map that fits the above criteria. [15A NCAC 02T .0105(d)] Section IV. 1. of FORM: NDSR 01-20 requires a listing of all monitoring wells at the facility, along with their coordinates, gradient to the disposal field, and location in relation to the compliance and review boundaries for the disposal fields. Please fill out table IV. 1. for the active monitoring wells on site. Mr. Victor Perez January 19, 2021 Page 3 of 3 B. Existing Permit: 1. The facility description for the previous permit includes the following excerpt for a stormwater collection system: "the continued operation of a 500,000 gallon stormwater collection and disposal facility consisting of a 354 GPM wet well pump station with duplex pumps, thirteen full circle impact sprinklers, approximately 1,260 linear feet of 6-inch subsurface gravity drain pipe, 1,450 linear feet of 8-inch force main and all related valves, piping and appurtenances; The regional office indicates that this was put in place for when large storm events would cause a surge of stormwater that would mix with raw sewage at the original wastewater facility. However, it is unclear whether the current operations of this stormwater collection system reflect this original intent. Please provide a narrative about the current operations of this stormwater system, including any recent maintenance related to the system. This narrative should discuss the relationship of the stormwater system and the other permitted wastewater treatment and high -rate infiltration disposal facilities associated with the permit, and whether there is any interactions between these stormwater and wastewater facilities.
