Based on the period from June 1996 to June 1997 the Town of Alexander
Mills WWTP is operating at 75.60 of design capacity. During rainfall
events the WWTP exceeds the design capacity due to inflow/infiltration.
On typical days the WWTP will not have problems treating this wastewater.
The town officials are aware of the requirements of 2H .0223. The NPDES
Permit expires November 30, 1998, this office will encourage the Town of
Alexander Mills to connect to the Town of Forest City wastewater
collection and treatment system. The Town of Alexander Mills is located
approximately 0.5 miles from a Town of Forest City pump station. This
should make the int9rconnection possible during the next,NPDES Permitting
cycle.

DIVISION OF ENVIRONMENTAL MANAGEMENT UNLESS ALL OF
THE APPLICABLE ITEMS ARE INCLUDED WITH -THE SUBMITTAL
Required Items
a. One original and three copies of the completed and appropriately executed application form.
b. Three sets of detailed plans and specifications signed and sealed by a North Carolina
Professional Engineer. The plans must include a general location map, a plan view of the
sewer extension, a profile of the sewer extension, details on the pump station, and must
show' the proximity of the sewer extension to other utilities and natural features.
Specifications may be omitted for delegated authorities. Each sheet of the plans and the first
page. of the specifications must be signed and sealed. Three copies of the existing permit if
a renewal or modification.
C. Three copies of all calculations, including pump selection, friction calculations, cycle
time, pump curves (including system curves applicable with one pump running, two
pumps running,_three pumps running, etc.), and evaluation of downstream pump stations.
These items must be submitted under the signature and seal of the North Carolina
Professional *Engineer.
d. The appropriate permit processing fee, in accordance with 15A NCAC 2H .0205(c)(5).
e. If the owner/authority of the wastewater treatment facility (WWTF) that will be accepting
the wastewater flow from this project is different from the applicant of the project, then a
letter must be provided from the owner/authority of the WWTF specifying the volume of
flow that will be accepted. The letter should be a recent letter and should refer4o the project
by the same name as that identified on the application and the plans/specifications.
f. An Operational Agreement (original and two copies) must be submitted if the sewer
extension will be serving single family residences, condominiums, mobile homes, or town
houses and if the subject sewer extension is owned by the individual residents, a
homeowners association, or a developer.

In accordance with your application received August 18, 1997, we are forwarding herewith Permit
No. WQ0014305 dated October 21, 1997, to the Town of Alexander Mills for the construction and
operation of the subject wastewater collection extension.
This permit shall be effective'from the date of issuance until rescinded and shall be subject to the
conditions and limitations as specified therein.
If any, parts, requirements, or limitations contained in this permit are unacceptable, you have the
right to request an adjudicatory hearing upon written requestwithin thirty (30) days following receipt of
this permit. This request must be in the form of a written petition, conforming to Chapter 150B of the
North Carolina General Statutes, and filed with the Office of Administrative Hearings, P.O. Drawer
27447, Raleigh, NC 27611-7447. Unless such demands are made this permit shall be final and binding.

WASTEWATER COLLECTION PERMIT
In accordance with the provisions of Article 21 of Chapter 143, General Statutes of North Carolina as
amended, and other applicable Laws, Rules, and Regulations
PERMISSION IS HEREBY GRANTED TO
Town of Alexander Mills
Rutherford County
FOR THE
construction and operation of a 55 GPM pump station (PS-1, High School) with duplex pumps, on -site
audible and visual high water alarms, an emergency power system complete with engine -generator set,
controls, raceway, conductors, transfer arrangements, auto dialer, and required appurtenances,
approximately 7,520 linear feet of three inch force main, a 105 GPM pump station (PS-2, Middle School)
with duplex pumps, on -site audible and visual high water alarms, an emergency power system complete
with engine -generator set, controls, raceway, conductors, transfer arrangements, auto dialer and required
appurtenances, and approximately 14,300 linear feet of four inch force main, to serve the Chase Middle
School and Chase High School in the Town of Alexander Mills, and the discharge of 18,900 GPD of
collected domestic wastewater into the Town of Alexander Mills' existing sewerage system, pursuant to
the application received August 18, 1997, and in conformity with the project plan, specifications, and
other supporting data subsequently filed and approved by the Department of Environment, Health and
Natural Resources and considered a part of this permit.
This permit shall be effective from the date of issuance until rescinded and shall be subject to the
following specified conditions and limitations:
This permit shall become voidable unless the facilities are constructed in accordance with the
conditions of this permit, the approved plans and specifications, and other supporting data.
2. This permit is effective only with respect to the nature and volume of wastes described in the
application and other supporting data.
3. The facilities must be properly maintained and operated at all times.
4. The sewage and wastewater collected by this system shall be treated in the Town of Alexander
Mills Wastewater Treatment Facility prior to being discharged into the receiving stream.
5. This permit is not transferable. In the event there is a desire for the facilities to change ownership,
or there is a name change of the Permittee, a formal permit request must be submitted to the
Division of Water Quality (Division) accompanied by an application fee, documentation from the
parties involved, and other supporting materials as may be appropriate. The approval of this
request will be considered on its merits and may or may not be approved.
6. Construction of the sewers, pump station(s) and force main shall be scheduled so as not to
interrupt service by the existing utilities nor result in an overflow or bypass discharge of
wastewater to the surface waters of the State.
7. Upon completion of construction and prior to operation of this permitted facility, a certification
must be received from a professional engineer certifying that the permitted facility has been
installed in accordance with this- permit, the approved plans and specifications, and other
supporting materials. Mail the Certification to the Non -Discharge Permitting Unit, P.O. Box
29535, Raleigh, NC 27626-0535.
8. A copy of the approved plans and specifications shall be maintained on file by the Permittee for a
minimum of five years from the date of the completion of construction.
9. Failure to abide by the conditions and limitations contained in this permit may subject the Permittee
to an enforcement action by the Division, in accordance with North Carolina General Statute 143-
215.6A to 143-215.6C.
10. The issuance of this permit does not preclude the Permittee from complying with any and all
statutes, rules, regulations, or ordinances which may be imposed by other government agencies
(local, state, and federal) which have jurisdiction.
11. The Permittee shall provide for the pump Station and force main the following items:
a. Pump on/off elevations located so that 2-8 pumping cycles may be achieved per hour in the
pump station,
b. An air relief valve located- at all high points along the force main,
c. A screened vent for the wet well,
d. Fillets located in the wet well at the intersection of the flooring and sidewalls,
e. Three feet of cover (minimum) over the force main or the use of ferrous material where three
feet cannot be maintained,
f. Sufficient devices which will protect the pump station from vandals, and
g. Flood protection if the pump station is located below the 100-year flood elevation..
12. In the event that the facilities fail to perform satisfactorily, including, the creation of nuisance
conditions, the'Permittee shall take immediate corrective action, including those as may be required
by this Division, such as the construction of additional or replacement wastewater collection
facilities.
13. NONCOMPLIANCE NOTIFICATION:
The Permittee shall report by telephone to the Asheville Regional Office, telephone number 704-
'251-6208, as soon as possible, but in no case more than 24 hours or on the next working day
following the occurrence or first knowledge of the occurrence of either of the following:
a. Any process unit failure, due to known or unknown reasons, that renders the facility incapable
of adequate wastewater transport, such as mechanical or electrical failures of pumps, line
blockage or breakage, etc.; or
b. Any failure of a pumping station or sewer line resulting in a by-pass directly to receiving
waters without treatment of all or any portion of the influent to such station or facility.

Persons reporting such occurrences by telephone shall also file a written report in letter form within
five (5) days following first knowledge of the occurrence. This report must outline the actions
taken or proposed to be taken to ensure that the problem does not recur.
