Dear Mr. Lafrenaye:
On August 3, 2011, the Division of Water Quality (DWQ) received your application dated August 3, 2011 to fill or
otherwise impact 32 linear feet of intermittent stream channel (permanent impacts), 4,353 square feet of Zone 1
Neuse River basin protected riparian buffer, and 3,931 square feet of Zone 2 Neuse basin protected riparian buffer
to construct a UNC Hosptial Campus in Hillsborough. In response to a request for more information letter, DWQ
received additional information on October 14, 2011. The DWQ has determined that your application and
additional information was incomplete and/or provided inaccurate information as discussed below. The DWQ will
require additional information in order to process your application to impact protected streams and buffers on the
subject property. This project is on hold and incomplete until we receive the additional information requested
below. If we do not receive the requested information, your project will be formally returned as incomplete. Please
provide the following information so that we may continue to review your project.
Additional Information Requested:
1. Per the phone conversation between Chad Abbott and Annette Lucas on October 18, 2011, please provide
Filterra devices at the four inlets that serve the driveway from Waterstone Drive.
2. Per the requirements of the Neuse Riparian Buffer Rule, the outlet that discharges the runoff from the
Central Utility Plant and the Equipment Yard must meet diffuse flow requirements. Level spreader-
vegetated filter strip (LS-VFS) design requirements are set forth in Chapter 8 of the North Carolina
Stormwater BMP Manual, available at: http://portal.ilcdenr.org/web/wq/wS/SLI/bmp-manual. If it is not
practical to install a LS-VFS to serve this outlet, then please provide another BMP that removes a

minimum of 30% of the Total Nitrogen (TN) and Total Phosphorus (TP) from the stormwater generated
by these areas. If you have any questions, please contact Annette Lucas at (919) 807-6381.
3. Please provide a BMP Supplement Form for each proposed best management practice that will be used on
the site (including both the Design Summary and Required Items Checklist), as well as all of the required
items. The BMP Supplement Forms are available on the DWQ web site at:

4. Please be advised that all future phases of this site will also be required to meet Neuse Buffer Rule diffuse
flow requirements via either a BMP that removes 30% of TN and TP or a properly designed LS-VFS.
Please submit five copies of this information within 30 calendar days of the date of this letter. Please send
additional information to DWQ Central Office, Attn: Karen Higgins, 1650 Mail Service Center, Raleigh,
NC 27699-1650. If we do not receive this requested information within 30 calendar days of the date of this
letter, your project will be withdrawn and you will need to reapply with a new application and a new fee. If you
need additional time to compile the information requested above, please email Lauren Witherspoon
(lauren.witherspoon@ncdenr.gov) requesting an extension and indicate a new date for submittal.
This letter only addresses the application review and does not authorize any impacts to wetlands, waters or
protected buffers. Please be aware that any impacts requested within your application are not authorized (at this
time) by the DWQ. Please call Annette Lucas at (919) 807-6381 if you have any questions regarding this letter or
would like to set up a meeting to discuss this matter.
