Transportation Project along SR 1556 (Krissy Prease Road) in Columbus County. The project
will be completed in compliance with NCDENR Division of Water Quality General Certification
3820, and, USACE Nationwide Pernut #14.
This project is being submitted under the 401 Automated Payment Procedure. The State
Project Number is 6C.024107 and the permit fee is 2$ 40.00. •

The I�TOrth Cazolina Department of Transportation (NCDO"1� has scheduled to clear, grade, drain,
base and pave SR-1�56, Columbus County. Total project len�th is 0.75 miles. The roadway wiil
be �iidened to 20 feet within a Right of R'ay of 50 feet w�ith stabilized grass shoulders (4 foot)
provided. This project will require the replacemen[ of two (2) wet cross lines and fill in adjacent
wetlands as shown below:

Construction of this project will require impacts to the waters of the United States in the Lumber
River Basin. Permit sites #1 and #2 are unnamed tributaries of Pine Log Branch (Class C, Sw).
The project is located within the 03040203 Hydrologic Unit, sub basin 03-07-58.

. SR-1556 is located in Western Columbus County the majority of the surrounding country side is
agricultural farm fields. Krissy Prease itself serves as an entrance for farm fields, as well as
several homesites. There are both cleared portions and woods.
Permit Site #1 has both stream and wetland impacts. A perennial stream channel is located at
Station 31+74. The current culvert is 27 LF 18" RCP, NCDOT is proposing to increase that Yo a
40 LF 24" RCP. Headwater Forest wetlands are adjacent to the stream channel both up and
downstream. Wetlands are located upstream from Station 33+15—Station 32+30 and .
downstream from Station 32+51 — Starion 31+20.
Permit Site#2 has an existing 36 LF 48" CMP located at Station 13+08. NCDOT proposes to
replace this deteriorated culvert with a new 40 LF 48" RCP. This intermittent stream is an
unnamed tributary of Pine Log Branch. It rated a 25 utilizing the NC DWQ Stream Assessment
form.

Endangered Species: Plants and animals with Federal classification of Endangered(E) or
Threatened ('I) aze protected under provisions of Section 7 and Section 9 of the Endangered
Species Act of 1973, as amended. As of September ZZ, 2010, the U.S. Fish and Wildlife Service
lists eight(8) federally protected species for Columbus County.

A biological conclusion of"No Effect" was reached for all species during the February 24, 2011
survey for protected species. Protected species were not identified, nor, was the general habitat
for each species identitied.
REGULATORY APPROVALS
Application is hereby made for a Department of the Anny Nationwide Permit 14 /NCDENR 401
WQC 3820 as required by the above-described activities.
The National Register was reviewed in addition to conducting a field view of the project for
sienificant historic features.
