The North Carolina epartment of Transportation (NCDO'I) propos�s to repair a fill slope
by rosetting disconn cted pipe ends and extcnding a pipe by two qoints as needed; then
armoring the pipe en with rip rap and adjacent ditch line for lang {erm stabilization. This
repair site will also i volve removal of 57 stone that has washed do�Yn the streambed some
28ft. This activity w ll involve permanont surface water fill of 8LF �'or pipe extension and
permanent rip rap of OLF along the bank of a buffered stream. Temporary concurnnt stream
impacts of 28LF wil be necessary for containmenUdewatering in ol�der to maintain a dry
work area during the epair effort and stone rcmoval. Buffer impacts p��'2.4QQSF�(Zone 1)and
1,600SF (Zone 2) wil be necessary to access the creek for the 57stonb removal,pipe, slope,
an'�i ch repair; and k stabilization.Although this crossing is mapped has Wy(Worshem
sandy loam),no wet) ds oxist within project limits. I

a WQC 3820 requested from DWQ since we are dealing with a complaint site and plus we've since
determined the pipe could use extending to facilitate this repair. Please use the WBS No.
5.209211B to process this request and see attached application packet with pictures, figures,
and PCN. Color photos are also attached.
I will forward original 5 copies via mail, but wanted to go ahead and email this request as
we discussed on the phone late last week. If Sonia could email the approval letter ahead of
