I will also upon receipt of the executed 0 &M Agreement forward it along with a hard copy of the Supplement Form. Thank you for your assistance in this matter. If you should have further questions or need any additional information please feel free to call or e-mail. Ron Sutton, P.E. From: Lucas, Annette [mailto:annette.lucas(dncdenr.gov] Sent: Thursday, September 15, 20112:18 PM To: rsutton(fterring-sutton.com Cc: Witherspoon, Lauren; Chapman, Amy Subject: 11-0777 Batts Farm II Ron, I have looked at the design of the level spreader and I am not seeing any major design issues. However, as we just discussed on the phone, a LS-VFS Supplement Form was not included with the submittal. Please complete this form and check to make sure that all of the required items listed on the form were in your submittal that we received on August 18, 2011. If the form and any additional information has been emailed to Lauren and me by 5:00 pm tomorrow (September 16, 2011), then I will include it in my review. You need to also submit two hard copies of the information to me at the address below as soon as possible. If we don't receive the information by the end of the day tomorrow, then we will send a letter requesting the form. Thanks, Annette Annette M. This maintenance record will be kept in a log in a known set location. Any deficient BMP elements noted in the inspection will be corrected, repaired or replaced immediately. These deficiencies can affect the integrity of structures, safety of the public, and the removal efficiency of the BMP. Important maintenance procedures: - Immediately after the filter strip is established, any newly planted vegetation will be watered twice weekly if needed until the plants become established (commonly six weeks). - Once a year, the filter strip will be reseeded to maintain a dense growth of vegetation - Stable groundcover will be maintained in the drainage area to reduce the sediment load to the vegetation. - Two to three times a year, grass filter strips will be mowed and the clippings harvested to promote the growth of thick vegetation with optimum pollutant removal efficiency. Turf grass should not be cut shorter than 3 to 5 inches and may be allowed to grow as tall as 12 inches depending on aesthetic requirements (NIPC,1993). Forested filter strips do not require this type of maintenance. Once a year, the soil will be aerated if necessary. - Once a year, soil pH will be tested and lime will be added if necessary. After the filter strip is established, it will be inspected quarterly and within 24 hours after every storm event greater than 1.0 inch (or 1.5 inches if in a Coastal County). Records of operation and maintenance will be kept in a known set location and will be available upon request. Inspection activities shall be performed as follows.
