Subject: Fill slope repair and pipe extension of a structure conveying an unnamed
tributary to Jordan Creek on, US 158 in Glanville County. WBS No.
5C.039011.
The North Carolina Department of Transportation (NCDO"r) proposes to repair a fill slope
by extending a pipe and then armoring the pipe ends with rip rap for long term stabilization.
This will involve permanent surface water fill of 12LF and permanent. rip rap of 20LF along
the bank of a buffered stream. Temporary concurrent stream impacts of 32 LF will be
necessary for dewatering in order to maintain a dry work area during the repair effort. Buffer
impacts of 1,800 SF (Zone 1) and 1,040 SF (Zone 2) will be necessary to access the creek
and for the fill slope to be repaired.
Application is hereby made for NCDENR-DWQ Section 401 Water Quality Certification
(WQC) No. 3820 and a Tar-Pamlico Riparian Buffer Authorization. A pre-construction
notification, construction plan sketch, project mapping, and photographs are attached
with this request. By copy of this letter we are notifying the USACE of the intent to work
under a non-reporting NWP 14. If you have any questions or need additional information,

The existing structure is too short for the fill section which is a slope stability concern. Site is surrounded by residential
and agricultural lands.

at this crossing.

12 feet of pipe will be added to the outlet end of this structure. The area will then be back filled to provide an adequate
slope section. Rip rap will be installed underlined with filter fabric for long term stability surrounding the mouth of the pipe
extension. Additionally rip rap armoring underlined with filter fabric will be added to the inlet for stability. Equipment may
include a backhoe an excavator, and grader.

On this one, if the pipe is being extended, then not all the buffer impact is stabilization
only. Some of it will be fill from the extension (if the pipe, correct?

Rob and Eric-
Please note that this is a pre-existing maintenance issue and is NOT associated with
Hurricane Irene.
Granville County Maintenance has an existing structure that is too short for the fill section
which is a stability concern on US 158 to west side of Oxford. This cross line will be
extended by 12ft to the outlet end, then the associated fill slope and shoulder section will
be rebuilt. This maintenance work should meet the conditions for working under a non-
reporting NWP 3 and WQC No. 3687.
Note that although wetland vegetation is observed at the outlet of this structure. This is
due to improperly draining ditches and the soils here are non hydric. Therefore no wetland
impacts are proposed.

For maintenance sites such as this one, NCDOT Division 5 has a procedure in place to provide
our construction crews with print outs of the general conditions along with a letter head
cover sheet explaining the intent to operate under a non-reporting permit as long as all
applicable conditions are meet. Oversight from both the Environmental Staff and REU Field
Operations Unit will ensure compliance with the permit conditions are met and that standard
BMPs are followed. Attached to this email is a courtesy PCN for your files along with site
maps and a construction plan. This work was scheduled to begin within the next few weeks.
