Dear Mr. Lafrenaye:
On August 3, 2011, the Division of Water Quality (DWQ) received your application dated August 3, 2011 to fill or
otherwise impact 32 linear feet of intermittent stream channel (permanent impacts), 4,353 square feet of Zone 1
Neuse River basin protected riparian buffer, and 3,931 square feet of Zone 2 Neuse basin protected riparian buffer
to construct a UNC Hosptial Campus in Hillsborough. The DWQ has determined that your application was
incomplete and/or provided inaccurate information as discussed below. The DWQ will require additional
information in order to process your application to impact protected streams and buffers on the subject property.
This project is on hold and incomplete until we receive the additional information requested below. If we do not
receive the requested information, your project will be formally returned as incomplete. Please provide the
following information so that we may continue to review your project.
Additional Information Requested:
DWQ does allow new stormwater conveyances to directly discharge into a stream without
30% nitrogen removal and attenuated flow through the riparian buffer per [15A NCAC 02B
.0233(6)]. In addition, DWQ does not allow concentrated flow from manmade conveyances
through the buffer.
a. Please provide a diffuse flow plan for the site.
b. You must show that this site meets diffuse flow requirements with a level spreader or
other BMP per Chapter 8 of the BMP Manual (see
http://portal.ncdenr.org/web/wq/ws/su/bmp-manual).

Please submit five copies of this information within 30 calendar days of the date of this letter. Please send
additional information to DWQ Central Office, Attn: Karen Higgins, 1650 Mail Service Center, Raleigh,
NC 27699-1650 and copy Lauren Witherspoon (lauren.witherspoon@ncdenr.gov). If we do not receive this
requested information within 30 calendar days of the date of this letter, your project will be withdrawn and you
will need to reapply with a new application and a new fee. If you need additional time to compile the information
requested above, please email Lauren Witherspoon (lauren.witherspoon@ncdenr.gov) requesting an extension
and indicate a new date for submittal.
This letter only addresses the application review and does not authorize any impacts to wetlands, waters or
protected buffers. Please be aware that any impacts requested within your application are not authorized (at this
time) by the DWQ. Please call Lauren Witherspoon at 919-791-4251 if you have any questions regarding this
