referenced project. The DWQ has determined that your application was incomplete and/or provided
inaccurate information as discussed below.

Nationwide Permit the USACE assigns to this project. You should contact the Raleigh
Regulatory Field office at [phone].
The permit reflects 50 linear feet of temporary stream impact. That necessitates a NW # from
the USACE as well as a $240 permitting fee made payable to DENR - DWQ to begin the
review and approval process.
Please submit this information within 30 calendar days of the date of this letter. This letter only addresses
the application review and does not authorize any impacts to wetlands, waters or protected buffers.
