Subject: Request for a N W 3 Permit and DWQ 401 for the replacement of Low Impact Bridge # 28 over
Perquimans River on SR 1204 (Sandy Cross Rd) in Perquimans County. W BS # 45347.1.5
The North Carolina Department of Transportation is requesting the issuance of a N W 3 permit
and corresponding DWQ certification for the replacement of Low Impact Bridge # 28 in Perquimans
County. The bridge is located on SR 1204 approximately 0.10 miles south of the intersection of SR 1205.
The bridge spans the Perquimans River, which is a tributary to [he Albemarle Sound. The Perquimans
River is classified by DWQ as class "C, Sw" waters. Bridge # 28 is currently a wooden structure with
concrete deck [ha[ is 103 ft. long by 25 ft. wide. "I"his structure will be removed using NC DOT's guidelines
for bridge removal. The proposed bridge will be replaced on its existing alignment The new bridge
structure will be I 1 T in length by 33' wide and built with precast cored slab concrete deck, concrete piles
and concrete end bents. An off=site detour will be utilized during construction. This bridge is following the
Low Impact Bridge Process established by FH WA, NCDOT and Regulatory Agencies.
