The attached letter and check was returned to us indicating the subject application had the "Incorrect Permitting Fee." I had a phone conversation with Mr. Ian McMillan on July 22, 2011 to discuss the Incorrect Permitting Fee Letter. Mr. McMillan advised me to adjust the Stream Impacts (3a)(S1/S2) on Page 5 of the Pre-Construction Notification (PCN) to accurately identify total impacts of the proposed project - which is 140 linear feet. Please refer to the enclosed modified Page 5 of the PCN (5 copies provided) which has the corrections and the enclosed check for $240.00 for permit fees associated with minor water quality application. The DWQ has determined that your application was incomplete and/or provided inaccurate information as discussed below. Additional Information Requested: ® This project based on the noted stream impact of 245 linear feet will require a permitting fee of $570. Check # 28621 for $240 is enclosed. You may submit the original check with another one for $330; or you may submit one check for the full amount. Please submit this information within 30 calendar days of the date of this letter.
