This form may be returned to DWQ by the applicant, the applicant's authorized agent, or the project engineer. It is not necessary to send certificates from all of these. DWQ personnel (John Dorney and Sue Homewood) visited the site last year and looked at the proposed repair areas. DWQ concurs with the proposed repair activities. Due to the size of the stream to be repaired, DWQ approves exemption of the requirement for "work in the dry" at this site. The project is deemed issued.
