Dear Mr. McMillan:
Enclosed is a 401 Water Quality Certification application for your review. This
application is submitted on behalf of Carolina Helpers, LLC for the construction of a road that
will provide access to a residence within the Chadwick Creek Tract. This site is located off of
Live Oak Drive in Pamlico County, NC. The applicant attempted to minimize wetlands impacts
as much as possible in the site design, but because of the location and extent of wetlands, he is
unable to access uplands within the tract without some disturbance. Approximately 0.493 acres
of 404 wetlands will be impacted in order to construct the road. To mitigate for these impacts,
the applicant proposes to preserve approximately five acres of wetlands on site.
The Pre-Construction Notification form, associated maps, and site plan are enclosed for
your review. Additionally, the $240 application fee is enclosed. Please contact me if you have

The purpose of the project is to provide access to uplands within the site for construction of a home. The applicant may
build another home within the site at a later date.

The applicant will clear the proposed road corridor and house pads of vegetation. Then fill material will be hauled to the
site and the 14'-wide road (35' wide corridor) will be constructed. Utilities will be placed within the road right-of-way. Once
the road is complete, a house ad will be graded. The house will then be built and a septic sstem will be installed.

adjacent to the site is EFH for red drum and smooth dogfish. Proposed project is not anticipated to affect EFH.
