Ian McMillan L NCDENR Division of Water Quality end 1650 Mail Service Center Raleigh, NC 27609 RE: 401 Water Quality Certification Application; Chadwick Creek Tract Pamlico County, NC Dear Mr. McMillan: Enclosed is a 401 Water Quality Certification application for your review. This application is submitted on behalf of Carolina Helpers, LLC for the construction of a road that will provide access to a residence within the Chadwick Creek Tract. This site is located off of Live Oak Drive in Pamlico County, NC. The applicant attempted to minimize wetlands impacts as much as possible in the site design, but because of the location and extent of wetlands, he is unable to access uplands within the tract without some disturbance. Approximately 0.493 acres of 404 wetlands will be impacted in order to construct the road. To mitigate for these impacts, the applicant proposes to preserve approximately five acres of wetlands on site. The Pre-Construction Notification form, associated maps, and site plan are enclosed for your review. Additionally, the $240 application fee is enclosed. Please contact me if you have any questions. Thank you for your assistance with this project. Sincerely, - c K____ L jj??? Kim Williams Wetland Scientist Encl.
