Box 2895 Boone, NC 28607 Subject Property: Temple of the High Country STORMWATER MANAGEMENT PLAN APPROVAL Dear Mr. Price: On March 7, 2011, the Division of Water Quality (DWQ) received a courtesy copy (dated March 7, 2011) of your Pre-Construction Notification (PCN) form to impact 0.091 acre of wetland for the purpose of constructing the proposed Temple of the High Country in Watauga County. On June 10, 2011, the Division of Water Quality (DWQ) received a stormwater management plan (dated June 6, 2011) for the Temple of the High Country project in order to meet the requirements of General Water Quality Certification 3821 and the Protocol for 401 SMP Approval and Implementation. The DWQ has reviewed your SMP and supporting information, and approves the revised SMP to satisfy the stormwater condition required by the 401 Certification.. This approval is for the purpose and design described in your application. If you change your project, you must notify us and you may be required to send us a new application. If the property is sold, the new owner must be given a copy of this approval letter and is thereby responsible for complying with all conditions. In order to maintain compliance with the approved SMP, please follow the conditions listed below: 1. Description of the SMP: The SMP approved by this office consists of one infiltration system and all associated stormwater conveyances, inlet and outlet structures, the grading and drainage patterns depicted on plan sheets dated June 3, 2011 (engineer's sealed date). No changes to the structural stormwater practices shall be made without written authorization from the DWQ. The stormwater management structures shall have maintenance access and at least a 10-foot drainage easement on each side to allow for the ability to maintain the structures, and to perform corrective actions. The stormwater easements shall be recorded to provide protection of the structures from potential alterations by future property owners. 2. Operation and Maintenance of BMPs: The approved SMP must be maintained in perpetuity in accordance with wet detention basin operation and maintenance (O & M) agreements signed by Mr. Steven C. Price. The O & M agreements must transfer with the sale of the property or transfer of ownership/responsibility for the BMP facilities. DWQ must be notified promptly of every transfer.
