On June 28, 2011 the Division of Water Quality (DWQ) received your application sets for the above
referenced project. The DWQ has determined that your application was incomplete and/or provided
inaccurate information as discussed below.
Additional Information Requested:
® DWQ requests that you contact the US Army Corps of Engineers in Raleigh since they may
wish to permit this as a NW 12 or NW39. The application reflects impacts not only to buffers
but wetlands and streams as well. You may be required to pay a $240 permitting fee if
wetlands and stream are involved. You can reach Thomas Brown at 919-554-4884 Ext 22.
® DWQ asks that large scale maps provided in paper form also be provided on a CD. One (1)
data CD of full sized plans in TIFF Group 4 format (black and white, not grayscale or color).
If the plans are too large to store in TIFF format, they may be stored in PDF. You may also
use FTP site and email them to bev.stricklandQncdenr.g_ov. Bev can be reached at (919) 807-
6350 for clarification.
Please submit this information within 30 calendar days of the date of this letter. If you will not be able to
provide the requested information within that timeframe, please provide written confirmation that you
intend to provide the requested information, and include a specific timetable delineating when the
requested materials will be provided. If we do not hear from you in 30 calendar days, we will assume that
you no longer want to pursue this project and we will consider the project as returned. Please be aware
that any impacts requested within your application are not authorized (at this time) by the DWQ. Please
