STATE OF NORTH CAROLINA Q~c~~~~ ~~ ~D NZ32011 ~~^'OSrt e~q~~r DCPARTMCNT OF TRANSPORTATION Beverly Eaves Purdue Gene Conti GOVERNOR SECRETARY June 22, 2011 U.S. Army Corps of Engineers pp Regulatory Field Office ~ ~ ®r~ Post Office Box 1000 VV Washington, N.C. 27889-1000 Attention: Bill Biddlecome Subject: Request for a NW 3 Permit and DWQ 401 for the replacement of Bridge # 44 over Allison Creek on SR 1147 (Sandy Point Road) in Hyde County. The North Carolina Department of Transportation is requesting the issuance of a N W 3 permit and corresponding DWQ certification for the replacement of Bridge # 44 in Hyde County. The bridge is located on SR 1147 approximately 0.78 miles from the intersection of SRI 146. The bridge spans Allison . Creek, which is a tributary to the Pungo River. Allison Creek is classified by DWQ as class "SA, HQ W, NSW" waters and also designated as Primary Nursery Area. The current bridge is a 30 ft. long X 15 ft. wide one lane bridge comprised of a wooden deck, rails, support piles and bents. This bridge will be completely removed following NCDOT guidelines for bridge demolition found in the NCDOT BMP manual for construction and maintenance activities. The new structure will be comprised of precast concrete piles, support bents and bridge deck. This structure has been designed at 55 R. and widened to 21 ft. The additional length improves hydrologic conveyance along Allison Creek and the additional width will allow for two travel lanes, meeting current design standards and improving safety. M inor roadway approach widening will be conducted to safely transition the road surface into the new bridge width. The minor widening will result in 704.46 sq. ft. (0.016 acres) of coastal wetland impacts and 731.5 sq. ft (0.017 acres) of surface water impacts. This road will be temporarily closed dw~ing the replacement of the bridge structure. Coordination with affected landowners has taken place. There currently are no permanent residents living beyond the bridge. Included with this letter are the location map, permit drawings, and PCN. If you have questions please call meat (252) 482-7977. River basin: Tar-Pamlico 3. Project Description 3a. Describe the existing conditions on the site and the general land use in the vicinity of the project at the time of this application: The project area is in rural Hyde County. The surrounding land use is dominated by pine plantation. The area has been subdivided for development, however most of the property remains forested. The only structures within the project area, are SR 1147 (gravel road) and the existing 30 ft. long X 15 ft. wide wooden bridge crossing Allison Creek. This bridge provides the only access to lands beyond Allison Creek. 3b. List the total estimated acreage of all existing wetlands on the property: 0.20 acres 3c. List the total estimated linear feet of all existing streams (intermittent and perennial) on the property: 60' 3d. Explain the purpose of the proposed project: Maintain an existing roadway and bridge structure for the traveling public of North Carolina. The existing wooden bridge is has low suffiency rating of 46.5 and the structual integrity is continuing to decline with age. Therefore, NCDOT will replace the bridge with a new precast concrete structure at current design standards, meeting safety and material requirements. 3e. Describe the overall project in detail, including the type of equipment to be used: This project proposes to replace bridge # 44 on SR 1147. This structure has been designed at 55 ft. and widened to 21 ft. The additional length improves hydrologic conveyance along Allison Creek and the additional width will allow for two travel lanes, meeting current design standards and improving safety. Minor roadway approach widening will be conducted to safely transition the road surface into the new bridge width. The minor widening will result in 704.46 sq. ft. (0.016 acres) of coastal wetland impacts and 731.5 sq. ft (0.017 acres) of surface water impacts. Equipment used for construction of this project will include, crane, excavator, dump trucks, and paving equipment. 4. Jurisdictional Determinations 4a. Specifically describe measures taken to avoid or minimize the proposed impacts in designing project. The bridge replacement is going to stay on the existing alignment. The bridge replacement was originally proposed as a After further coordination with regulatory agencies NCDOT elected to build a longer bridge structure, which culvert structure . would improve Allison Creeks natural flow during ebbing and flooding events. This will likely improve fisheries habitat and Allison Creeks designation as a Primary Nursery Area. NCDOT minimized bridge and road shoulder width to the maximum extent practicable. This significantly reduced wetland and surface water impacts. 1 b. Specifically describe measures taken to avoid or minimize the proposed impacts through construction techniques. The bridge will be replaced using top down construction methods. NCDOT will follow Best Management Practices for bridge demolition and the control of erosion and sedimentation. 2.
