Subject: Request for a NW 3 Permit and DWQ 401 for the replacement of Low Impact Bridge # 34 over
Allison Creek on SR 1148 (Sladesville Rd) in Hyde County. WBS # 45347.1.3
The North Carolina Department of Transportation is requesting the issuance of a NW 3 permit
and corresponding DWQ certification for the replacement of Low Impact Bridge # 34 in Hyde County. The
bridge is located on SR 1 148 approximately 0.17 miles south of the intersection of SR 1152. The bridge
spans Smith Creek, which is a tributary to the Pungo River. Smith Creek is classified by DWQ as class
"SC, NSW" waters. Bridge # 34 is currently a wooden structure that is 48 ft. long by 25.4 ft. wide. This
structure will be removed using NCDOT's guidelines for bridge removal. The proposed bridge will be
replaced on its existing alignment. The new bridge structure will be 57' in length by 30' wide and built
with precast cored slab concrete deck, concrete piles and concrete end bents. An off-site detour will be
utilized during construction. This bridge is following the Low Impact Bridge Process established by
FH WA, NCDOT and Regulatory Agencies.

Project Description - The NCDOT is proposing to replace an existing 48 ft. long X 25 ft. wide wood bridge with a
57 ft. long X 30 ft. wide concrete bridge. The new structure impacts have been minimized to the maximum extent
practicable. The roadway shoulder widening to accommodate the new bridge approach will result in 712 sq. ft.
(0.016 acres) of coastal wetland impact and 72 sq. ft. (0.002 acres) of surface water fill. This project has avoided
Tar-Pamlico buffer impacts. Do to the lengthening of the bridge structure, approximately 104 sq. ft. of wetland,
will be restored by removal of bridge approach fill to surrounding wetland elevation. NCDOT will follow all
guidelines and BMPs for the removal of the existing bridge and Erosion / Sedimentation Control.
