Subject: Courtesy Notification for a NW 3 permit for pipe replacement on SR 1125, West Millpond
Road in Washington County.
This is a courtesy notification from the North Carolina Department of Transportation for a NW 3
permit for pipe replacement on SR 1125 (West Millpond Road) in Washington County. The current pipes
are failing and need to be replaced to secure safe travel and allow better drainage. There are currently two
pipes 54' x 36" RCP and the two proposed pipes are 60' x 48" CMP. The longer pipes are needed for
wider shoulders to allow safer passage and will allow better flow.
NCDOT also would like to remove silt and woody debris from the adjacent stream for
approximately 1720 feet. Then, at the point of the stream entering the woodline, NCDOT would like to
snag and remove woody debris by hand for approximately 1050 feet. Historically, this drainage feature has
been maintained by cleaning out silt and woody debris by machinery. The dimensions of the current
drainage are approximately 8-10 feet wide and 4-6 feet deep upstream of the pipes, and 4-6 feet wide and
3-4 feet deep downstream of the pipes. The drainage feature will not be widened or deepened. Only silt
and woody debris will be removed. All spoil will be placed in an upland area. Bill Biddlecome of the U.S.
Army Corps of Engineers has given us permission to do this work and a copy of the letter is included in this
package. A site location map depicting this work in included along with the PCN form. If you need any

SR 1119 and SR 1125 in an unnamed tributary which flows into Kendrick Creek, south of the town
of Roper, in Washington County, North Carolina.
Description of Activity: Remove accumulated sediments from a channelized headwater stream east
of SR 1119 and SR 1125. The length of the clean up will be no longer than 1720 linear feet and 1050
linear feet for wood debris as depicted on the submitted work plans.
Your work as proposed does not require Department of the Army authorization for the following
reason(s):

III. Project Information
Attach a vicinity map clearly showing the location of the property with respect to local
landmarks such as towns, rivers, and roads. Also provide a detailed site plan showing property
boundaries and development plans in relation to surrounding properties. Both the vicinity map
and site plan must include a scale and north arrow. The specific footprints of all buildings,
impervious surfaces, or other facilities must be included. If possible, the maps and plans should
include the appropriate USGS Topographic Quad Map and NRCS Soil Survey with the property
boundaries outlined. Plan drawings, or other maps may be included at the applicant's discretion,
so long as the property is clearly defined. For administrative and distribution purposes, the
USACE requires information to be submitted on sheets no larger than 11 by 17-inch format;
