Keith Haynes and I made an introductory visit to the Plant to meet and review Jeld-Wen's wastewater treatment
permit with Mr. Kevin Heape. The facilities appear to be in good condition and the plant was running well. We
reviewed the NPDES permit and did a plant walk around.

There are several sites around the plant where water of different types is being discharged (ie. - the water
filter backwash, and condensate from the roof on the south side of the plant). These discharges should be
investigated, analyzed (if needed) and directed to a permitted discharge.

As a reminder your permit requires that you contact the Division if there are any upsets and/or noncompliance
within 24 hours of the event. Weekdays [phone] and Nights, Weekends and Holidays [phone].

Dear Mr. English:
On November 22, 2013, I conducted an inspection at the Jeld-Wen facility to judge compliance of the
wastewater treatment plant (WWTP), including the pump station. The facilities appeared to be compliant with
permit NC0076180. Present during the inspection were you and Keith Morgan with Jeld-Wen and Ken Deaver
with Kace Enviromental, Inc.
Jeld-Wen requested their WWTP be temporarily closed down since the Marion operations have ceased. We
discussed the WWTP closure, the skeleton staff remaining onsite and how they should monitor the closed down
WWTP. We also discussed the pump and haul permit (WQ0036832) requested by Jeld -Wen and the necessary
operation and maintenance requirements associated with the pump station.

Inspected the facility, plant condition appears ok. Effluent was relatively clear with the presence of pin
floc.
As mentioned during the inspection, access to the plant via the current ladder is difficult. Carrying
equipment and sampling bottles is treacherous while trying to climb a ladder. Construction of a staircase
would be safer and more convenient.

The Marion Jeld-Wen facility is temporarily closed due to reduced demand for their product. They are
hopeful in a couple of years demand will return, but in the meantime have closed operations at the Marion
facility. In turn they have also closed their wastewater treatment plant (WWTP). There will be ~three people
remaining at the facility for maintenance and security operations. A pump and haul has been issued for this
wastewater. Please refer to permit WQ0036832.
WWTP Temporary Closure:
The cleaning and shut down of the WWTP appeared to be sufficient to protect the environment from
impacts. The WWTP was pumped and cleaned. Two plugs at the bottom of the WWTP have been removed
to allow rain water to be released. (Note: this WWTP is an above ground metal package plant.) There were
some remaining solids/grit (wood/resin?) caked to a small area at the bottom of the WWTP that will have to
be monitored to make sure they don't migrate. Monitoring should take place monthly and following
significant rainfall events. Without Jeld-Wen in operation there should be no discharge from discharge #2
or #3 either. (See the following question area for more information.)
Pump and Haul (Pump Station):
The pump station wet well will act as a holding basin for a pump and haul permit during the facility closure.
The high water alarm will remain active.
Prior to this WWTP being reactivated there are maintenance activities that need to take place; specifically
the plant should be sand blasted and repainted.
Log/Record keeping:
Onsite staff should record their observations of the WWTP, as well as the required record keeping for the
pump and haul permit. Log book should be kept on site.

The Marion Jeld-Wen facility is temporarily closed due to reduced demand for their product. They are
hopeful in a couple of years demand will return, but in the meantime have closed operations at the Marion
facility. In turn they have also closed their wastewater treatment plant (WWTP). There will be ~three people
remaining at the facility for maintenance and security operations. A pump and haul has been issued for this
wastewater. Please refer to permit WQ0036832.
WWTP Temporary Closure:
The cleaning and shut down of the WWTP appeared to be sufficient to protect the environment from
impacts. The WWTP was pumped and cleaned. Two plugs at the bottom of the WWTP have been removed
to allow rain water to be released. (Note: this WWTP is an above ground metal package plant.) There were
some remaining solids/grit (wood/resin?) caked to a small area at the bottom of the WWTP that will have to
be monitored to make sure they don't migrate. Monitoring should take place monthly and following
significant rainfall events. Without Jeld-Wen in operation there should be no discharge from discharge #2
or #3 either. (See the following question area for more information.)
Pump and Haul (Pump Station):
The pump station wet well will act as a holding basin for a pump and haul permit during the facility closure.
The high water alarm will remain active.
Prior to this WWTP being reactivated there are maintenance activities that need to take place; specifically
the plant should be sand blasted and repainted.
Log/Record keeping:
Onsite staff should record their observations of the WWTP, as well as the required record keeping for the
pump and haul permit. Log book should be kept on site.

and not be reissued. Jeld Wen received the subject invoice, since the
permit will not be reissued this invoice will be void, Jeld Wen does not
have to pay it.

As we discussed on the phone, Jeld Wen will be using portable toilets in
the interim of getting a county health department system. Please let me
know once you get the permit from the county for the septic system.

Pump and Haul Information
Asbury’s Septic Tank Cleaning and Backhoe Service is the service we use to pump and
haul the holding tank.
Town of Old Fort WWTP run by Kace Environmental is the receiving WWTP

Asbury’s picked up approximately 528 gallons 24 times since December 2013.
The dates are:

Please pay particular attention to the following standard conditions contained within this permit:
Condition 1.1: Per 15A NCAC 02T 0204 the Permittee or its representative shall inspect the
permitted pump and haul facilities at least daily to prevent malfunctions and
deterioration, operator errors, and discharges that may cause or lead to the release of
wastes to the environment, a threat to human health, or a nuisance.
Condition 1.2: Requires an accurate record of the pump and haul activities must be maintained by the
Permittee.
It shall be responsibility of the Permittee to ensure that the as -constructed project meets the
appropriate design criteria and rules. Failure to comply may result in penalties in accordance with North
Carolina General Statute §143-215.6A through §143-215.6C and/or construction of additional or
replacement wastewater collection facilities.

Jeld-Wen is hereby authorized to:
Construct and operate the aforementioned domestic wastewater pump and haul operation.
The wastewater collected by this system shall be pumped by Asbury's Septic Tank Service and hauled to the Old
Fort Wastewater Treatment Facility (Permit No. NC0021229), for final treatment.
Division approval is based on acceptance of the certification provided by a North Carolina -licensed Professional
Engineer in the application. It shall be the Permittee's responsibility to ensure that the as -constructed project
meets the appropriate design criteria and rules. Failure to comply may result in penalties in accordance with
North Carolina General Statute §143-215.6A through §143-215.6C, construction of additional or replacement
facilities, and/or referral of the North Carolina -licensed Professional Engineer to the licensing board.
Construction and operation is contingent upon compliance with the Standard Conditions and any Special
Conditions identified below.
I. SPECIAL CONDITIONS
1. The Permittee shall provide for the installation and maintenance of an audible and visual high-water alarm on all
storage and pumping facilities associated with this pump and haul operation
2. The Asheville Regional Office, Surface Water Protection Group, must be notified at least 48 hours prior to the
commencement of the pump and haul activities so that a site inspection can be performed.
3. This permit shall become voidable unless the agreement between Jeld-Wen, Asbury's Septic Tank Service, and the Old
Fort Wastewater Treatment Facility for the collection, transportation, and final treatment of wastewater is in full force
and effect.
4. This approval shall expire (December 2, 2014). Thirty (30) days prior to the expiration date, a report shall be submitted
to the Asheville Regional Office, detailing final disposal solutions.

II. STANDARD CONDITIONS
1. Per 15A NCAC 02T .0204, the Permittee or its representative shall inspect the permitted pump and haul facilities at
least daily to prevent malfunctions and deterioration, operator errors, and discharges that may cause or lead to the
release of wastes to the environment a threat to human health, or a nuisance The Permittee shall maintain an
inspection log or summary including at least:
• Date and time of inspection,
• Observations made,
• Any maintenance repairs, or corrective actions taken by the Permittee.
The Permittee shall maintain this log of inspections for as long as the pump and haul activities are being conducted and
shall be made available to the Division or other permitting authority, upon request.
2. An accurate record of the pump and haul activities must be maintained by the Permittee, indicating:
• Date and time wastewater is removed from the facility;
• Name of contract hauler;
• Name of facility receiving wastewater;
• Volume of wastewater removed;
• Status of permanent disposal option
The Permittee shall maintain this log for as long as the pump and haul activities are being conducted and shall be made
available to the Division or other permitting authority, upon request.
3. This permit shall not be transferable. In the event there is a desire for the facilities to change ownership, or there is a
name change of the Permittee, a formal permit request shall be submitted to the Division accompanied by an
application fee, documentation from the parties involved, and other supporting materials as may be appropriate. The
approval of this request shall be considered on its merits and may or may not be approved.
4. This permit shall become voidable unless the subject pump and haul activities are carried out in accordance with the
conditions of this permit; 15A NCAC 2T; and other supporting materials unless specifically mentioned herein.
5. This permit shall be effective only with respect to the nature and volume of wastes described in the application and
other supporting data No type, kind or quantity of wastewater other than the pumped and hauled wastewater from
the facility shall be included in these pump and haul activities.
6. The wastewater collection facilities shall be properly maintained and operated at all times. Adequate inspection,
maintenance, and cleaning shall be provided by the permittee to insure proper operation of the subject facilities.
7. Noncompliance Notification: The Permittee shall report by telephone to a water quality staff member at the Asheville
Regional Office, telephone number 828-296-4500, as soon as possible but in no case more than 24 hours or on the
next working day, following the occurrence or first knowledge of the occurrence of either of the following:
a. If the flow volume estimated above is exceeded by any amount on any day; or
b. Any process unit failure due to known or unknown reasons, that renders the facility incapable of adequate
wastewater storage, such as failures of tanks, piping, pumps, aerators, compressors etc., or
c. Any failure of a pumping station, sewer line, etc. resulting in a spill or by-pass directly to receiving waters
without treatment of all or any portion of the influent to such station or facility; or
d. Any SSO and/or spill over 1,000 gallons; or
e. Any SSO and/or spill, regardless of volume, that reaches surface water

Overflows and spills occurring outside normal business hours may also be reported to the Division of Emergency
Management at telephone number [phone] or [phone]. Persons reporting any of the above occurrences
shall file a spill report by completing and submitting Part I of Form CS-SSO (or the most current Division approved form)
within five days following first knowledge of the occurrence. This report must outline the actions taken or proposed to

be taken to ensure that the problem does not recur. Part II of Form CS-SSO (or the most current Division approved
form) can also be completed to show that the SSO was beyond control.
9. Failure to abide by the conditions and limitations contained in this permit; 15A NCAC 2T; and other supporting
materials may subject the Permittee to an enforcement action by the Division, in accordance with North Carolina
General Statutes §143-215.6A through §143-215.6C, construction of additional or replacement wastewater collection
facilities, and/or referral of the North Carolina -licensed Professional Engineer to the licensing board.
10. In the event that the wastewater collection facilities fail to perform satisfactorily, including the creation of nuisance
conditions, the Permittee shall cease operation of all pump and haul activities and shall take immediate corrective
action, including those as may be required by this Division, such as the construction of additional or replacement
facilities.
11 Any duly authorized officer, employee, or representative of the Division of Water Quality may, upon presentation of
credentials, enter and inspect any property, premises, or place on or related to the pump and haul collection facilities
at any reasonable time for the purpose of determining compliance with the Division's approval; may inspect or copy
any records that must be maintained under the terms and conditions of this permit; and may obtain samples.
12. The issuance of this permit shall not exempt the Permittee from complying with any and all statutes, rules, regulations,
or ordinances that may be imposed by other government agencies (local, state and federal) which have jurisdiction.
13. A copy this permit, the approved plans and specifications shall be maintained on file by the Permittee for the life of the
project.
14. When these pump and haul activities are no longer necessary, please send written notification to:

I need to get this in your hopper for the end of the year. I know you are off for several days and then there are the
Holidays, which is ok since this isn't pressing because this permit expires at the end of the month. But, I wanted to
get this on your radar since Jeld Wen is concerned because they were billed.
Jeld Wen will not be applying or granted another P&H, so it can expire. It will need to be closed out in BIMs, I
assume you are the person with those permissions.
Teresa can void this invoice.
And I have informed Jeld Wen they do not need to worry about this invoice.

Jeld Wen has a P&H permit (WQ0038218) that is expiring at the end of this month. It will not be reissued. Jeld Wen
received an invoice (#2017PR011621) for $810 for the period of 01/01/2018-12/31/2018.
I am not sure why they received this, they will not have the P&H in 2018. I believe this invoice may be in error. Jeld
Wen would like me to inform them on this invoice. My first thought is they do not need to pay this, right? Then my
next question is how to we get this invoice removed from the system.

6. The wastewater collection facilities shall be properly maintained and operated at all times.
Adequate inspection maintenance, and cleaning shall be provided by the permittee to insure proper
operation of the subject facilities. [15A NCAC 02T 0403]
7. The Permittee shall report by telephone to a Water Resources staff member at the Asheville
Regional Water Resources Section telephone number 828-296-4500, as soon as possible, but in no
case more than 24 hour, following the occurrence or first knowledge of the occurrence of either of
the following:
a. If the flow volume estimated above is exceeded by any amount on any day; or
b. Any process unit failure due to known or unknown reasons, that renders the facility
incapable of adequate wastewater storage, such as failures of tanks, piping, pumps,
aerators, compressors etc.; or
c. Any failure of a pumping station, sewer line, etc. resulting in a spill or by-pass directly to
receiving waters without treatment of all or any portion of the influent to such station or
facility; or
d. Any SSO and/or spill over 1,000 gallons; or

e. Any SSO and/or spill, regardless of volume, that reaches surface water

initial verbal report. Overflows and spills occurring outside normal business hours may also be
reported to the Division of Emergency Management at telephone number [phone] or (919)
733-3300. Persons reporting any of the above occurrences shall file a spill report by completing and
submitting Part I of Form CS-SSO (or the most current Division approved form) within five days
following first knowledge of the occurrence. This report must outline the actions taken or proposed
to be taken to ensure that the problem does not recur. Part II of Form CS-SSO (or the most current
Division approved form) can also be completed to show that the SSO was beyond control. [G.S. 143-

8. Failure to abide by the conditions and limitations contained in this permit; 15A NCAC 02T; and other
supporting materials may subject the Permittee to an enforcement action by the Division, in
accordance with North Carolina General Statutes §143-215.6A through §143-215.6C, construction of
additional or replacement wastewater collection facilities and/or referral of the North Carolina -
licensed Professional Engineer to the licensing board. [15A NCAC 02T .0104 15A NCAC 02T .0108(b-

9. In the event that the wastewater collection facilities fail to perform satisfactorily, including the
creation of nuisance conditions, the Permittee shall cease operation of all pump and haul activities
and shall take immediate corrective action, including those as may be required by this Division, such
as the construction of additional or replacement facilities. [15A NCAC 02T 0110; 15A NCAC 02T
.0108(b)]
10. Any duly authorized officer, employee, or representative of the Division of Water Resources may,
u pon presentation of credentials enter and inspect any property, premises, or place on or related to
the pump and haul collection facilities at any reasonable time for the purpose of determining
compliance with the Division's approval; may inspect or copy any records that must be maintained
u nder the terms and conditions of this permit and may obtain samples. [15A NCAC 02T .0110]
11. The issuance of this permit shall not exempt the Permittee from complying with any and all statutes,
rules, regulations, or ordinances that may be imposed by the Division any other Federal, State or
Local government agencies which have jurisdiction or obtaining other permits which maybe
required by the Division or any other Federal, State, of Local government agencies. [G.S. 143-
215.1(b)]
12. Operation of this permit is based on acceptance of the certification attached to this permit,
completed by the Permittee and a North Carolina licensed Professional Engineer. Note that a
completed certification is only required for activities that met the definition of the Practice of
Engineering in accordance with G.S. 89C-3. In some instances this certification may not be required.
13. A copy this permit, and approved plans and specifications (if applicable) shall be maintained on file

14. This permit shall expire December 31 2015 and on the date of expiration this permit shall be

In accordance with the provisions of Article 21 of Chapter 143, General Statutes of North Carolina as
amended, and other applicable Laws Rules and Regulations, permission is hereby granted to the
Jeld-Wen
for the construction and operation of a domestic wastewater pump and haul operation to serve the
McDowell County Jeld-Wen facility, consisting of a wet well (inactive WWTP pump station) with an
estimated total storage capacity of 528 gallons. The facility from which pumping will occur shall include
a minimum 24 hours storage with on -site audible and visual high water alarms. The anticipated flow for
this pump and haul operation is expected to be 75 gallons per day of domestic wastewater, pursuant to
the application received January 6, 2015 and in conformity with 15A NCAC 02T and other supporting
data subsequently filed and approved by the Department of Environment and Natural Resources and
considered a part of this permit
This permit shall be effective from January 12, 2015, will expire on December 31, 2015, and shall be

Jeld-Wen is hereby authorized to:
Construct and operate the aforementioned domestic wastewater pump and haul operation.
The wastewater collected by this system shall be pumped and hauled to the Old Fort Wastewater
Treatment Facility (Permit No. NC0021229), for final treatment.

It shall be the Permittee's responsibility to ensure that the as -constructed project meets the appropriate
design criteria and rules. Failure to comply maytesult in penalties in accordance with North Carolina
General Statute §143-215.6A through §143-215.6C, construction of additional or replacement facilities,
and/or referral of the North Carolina -licensed Professional Engineer to the licensing board.
Construction and operation is contingent upon compliance with the Standard Conditions and any Special
Conditions identified below.
1. SPECIAL CONDITIONS
1. The Permittee or its representative shall inspect the permitted pump and haul facilities at least daily
to prevent malfunctions and deterioration, operator errors, and discharges that may cause or lead
to the release of wastes to the environment, a threat to human health, or a nuisance condition. The
Permittee shall maintain an inspection log or summary including at least:
• Date and time of inspection;
• Observations made:
• Any maintenance, repairs, or corrective actions taken by the Permittee
The Permittee shall maintain this log of inspections for as long as the pump and haul activities are
being conducted and shall be made available to the Division or other permitting authority, upon

2. This permit shall become voidable unless the agreement between Jeld-Wen, Asbury's Septic Tank
Service, and the Town of Old Fort for the collection transportation, and final treatment of
wastewater is in full force and effect. [15A NCAC 02T.0204(b)]

II. STANDARD CONDITIONS
1. An accurate record of the pump and haul activities must be maintained by the Permittee, indicating:
• Date and time wastewater is removed from the facility;
• Name of contract hauler;
• Name of facility receiving wastewater;
• Volume of wastewater removed;
The Permittee shall maintain this log for as long as the pump and haul activities are being conducted
and shall be made available to the Division or other permitting authority, upon request. [15A NCAC
02T .0108]
2. This permit shall not be transferable. In the event there is a desire for the facilities to change
ownership, or there is a name change of the Permittee a formal permit request shall be submitted
to the Division accompanied by an application fee documentation from the parties involved and
other supporting materials as may be appropriate. The approval of this request shall be considered
on its merits and may or may not be approved. [15A NCAC 02T .0104 G.S. 143-215.1(d3)]
3. The Permittee shall provide for the installation and maintenance of an audible and visual high-water
alarm on all storage and pumping facilities associated with this pump and haul operation [15A NCAC
02T .0204(c)]
4. This permit shall become voidable unless the subject pump and haul activities are carried out in
accordance with the conditions of this permit; 15A NCAC 2T; and other supporting materials unless
specifically mentioned herein. [15A NCAC 02T.0110]
5. This permit shall be effective only with respect to the nature and volume of wastes described in the
application and other supporting data. No type kind or quantity of wastewater other than the
pumped and hauled wastewater from the facility shall be included in these pump and haul activities.

This is what Jeld Wen sent today. She sent the pump and haul info doc. They did have several pumping events in 2013. I
thought there were not many but apparently there were. No big deal I guess just an expensive toilet for those few
people onsite and not very consistent flows. The application estimated "75 gpd (#III. 3-4)
I have attached the p and h pdf for you in case you wanted to see the application. I had to reread it too since it was a
vague recollection. Apparently they did anticipate this being a two year shut down (# Vl.3.). And I don't recall what the
thought was for the November 1, 2015 date (#VI. 5.),,,,I can ask Bonnie if she knows why Mr. English wrote that if you
like.

Pump and Haul Information
Asbury's Septic Tank Cleaning and Backhoe Service is the service we use to pump and
haul the holding tank.
Town of Old Fort WWTP run by Kace Environmental is the receiving WWTP
Asbury's picked up approximately 528 gallons 24 times since December 2013.

The NPDES permit for the Jeld-Wen Wood Fiber Div expired on December 31, 2014. Federal (40 CFR
122) and state (15A NCAC 2H.0105 (e)) regulations require that permit renewal applications be filed at
least 180 days prior to expiration of the current permit.
To satisfy this requiiement, the renewal package should have been sent to the Division postmarked no
later than July 4, 2014.
The Division has not received a renewal request for the subject permit. This is a violation of
your permit at Part II. B. 10., which states "Any perinittee that has not requested renewal at least 180
days prior to expiration...will subject the permittee to enforcement procedures as provided in NCGS 143
215.6 and 33 USC 1251 et. seq.".
In order to prevent an assessment of civil penalties you must submit a completed permit application (see
enclosed forms) no later than January 12, 2015. Use the enclosed checklist to complete your renewal
package. The checklist identifies the items you must submit with the permit renewal application.

I talked with Bonnie at Jeld Wen and they completely understand what rescinding their WW (NC0076180) permit means.
She told me if/when they are able to reopen that facility they will have to meet air quality emission controls that would
require additional WW flows. Also she stated their current WWTP is dated, limited in size and not the type system they
want any more. So they understand they would be starting from ground zero with a new WWTP permit application.
I did ask her why Mr. English listed the November 2015 date, she did not know.

In accordance with the provisions of Article 21 of Chapter 143, General Statutes of North Carolina as
amended, and other applicable Laws, Rules and Regulations, permission is hereby granted to the
JELD —WEN, Inc.
for the construction and operation of a domestic wastewater pump and haul operation to serve the
McDowell County JELD-WEN, Inc. facility, consisting of a wet well (inactive WWTP pump station) with an
estimated total storage capacity of 528 gallons. The facility from which pumping will occur shall include
a minimum 24 hours storage with on -site audible and visual high water alarms. The anticipated flow for
this pump and haul operation is expected to be 75 gallons per day of domestic wastewater, pursuant to
the application received December 22 2015 and in conformity with 15A NCAC 02T and other supporting
data subsequently filed and approved by the Department of Environment and Natural Resources and
considered a part of this permit.
This permit shall be effective from (January 7, 2016), will expire on (December 31, 2017), and shall be
subject to the conditions and limitations as specified therein.

SUPPLEMENT TO PERMIT COVER SHEET
JELD-WEN, Inc. is hereby authorized to:
Construct and operate the aforementioned domestic wastewater pump and haul operation. The
wastewater collected by this system shall be pumped and hauled to Town of Old Fort Wastewater
Treatment Facility (Permit No. NC0021229) for final treatment.
It shall be the Permittee's responsibility to ensure that the as -constructed project meets the appropriate
design criteria and rules. Failure to comply may result in penalties in accordance with North Carolina
General Statute §143-215.6A through §143-215.6C, construction of additional or replacement facilities,
and/or referral of the North Carolina -licensed Professional Engineer to the licensing board.
Construction and operation is contingent upon compliance with the Standard Conditions and any Special
Conditions identified below.
I. SPECIAL CONDITIONS
1. The Permittee or its representative shall inspect the permitted pump and haul facilities during pump
and haul activities to prevent malfunctions and deterioration operator errors, and discharges that
may cause or lead to the release of wastes to the environment, a threat to human health, or a
nuisance condition. The Permittee shall maintain an inspection log or summary including at least:
a. Date and time of inspection;
b. Observations made;
c. Any maintenance repairs, or corrective actions taken by the Permittee.
The Permittee shall maintain this log of inspections for as Tong as the pump and haul activities are
being conducted and shall be made available to the Division or other permitting authority, upon
request. [15A NCAC 02T .0204(d)]
2. This permit shall become voidable unless the agreement between JELD-WEN, Inc, Ashbury's Septic
Tank Service and the Town of Old Fort for the collection, transportation, and final treatment of
wastewater is in full force and effect. [15A NCAC 02T 0204(b)]
II. STANDARD CONDITIONS
1. An accurate record of the pump and haul activities must be maintained by the Permittee, indicating:
a. Date and time wastewater is removed from the facility;
b. Name of contract hauler;
c. Name of facility receiving wastewater;
d. Volume of wastewater removed.

The Permittee shall maintain this log for as long as the pump and haul activities are being conducted
and shall be made available to the Division or other permitting authority, upon request. [15A NCAC
02T .0108]
2. This permit shall not be transferable. In the event there is a desire for the facilities to change
ownership, or there is a name change of the Permittee, a formal permit request shall be submitted
to the Division accompanied by an application fee documentation from the parties involved and
other supporting materials as may be appropriate. The approval of this request shall be considered
on its merits and may or may not be approved. [15A NCAC 02T .0104 G.S. 143-215.1(d3)]
3. The Permittee shall provide for the installation and maintenance of an audible and visual high-water
alarm on all storage and pumping facilities associated with this pump and haul operation [15A NCAC
02T .0204(c)]
4. This permit shall become voidable unless the subject pump and haul activities are carried out in
accordance with the conditions of this permit; 15A NCAC 2T; and other supporting materials unless
specifically mentioned herein. [15A NCAC 02T.0110]
5. This permit shall be effective only with respect to the nature and volume of wastes described in the
application and other supporting data. No type kind or quantity of wastewater other than the
pumped and hauled wastewater from the facility shall be included in these pump and haul activities.

6. The wastewater collection facilities shall be properly maintained and operated at all times.
Adequate inspection maintenance and cleaning shall be provided by the permittee to insure proper
operation of the subject facilities. [15A NCAC 02T 0403]
7. The Permittee shall report by telephone to a Water Resources staff member at the Asheville
Regional Water Resources Section, telephone number 828-296-4500, as soon as possible, but in no
case more than 24 hour, following the occurrence or first knowledge of the occurrence of either of
the following:
a. If the flow volume estimated above is exceeded by any amount on any day; or
b. Any process unit failure, due to known or unknown reasons, that renders the facility
incapable of adequate wastewater storage, such as failures of tanks, piping, pumps,
aerators, compressors etc.; or
c. Any failure of a pumping station, sewer line, etc., resulting in a spill or by-pass directly to
receiving waters without treatment of all or any portion of the influent to such station or
facility; or
d. Any SSO and/or spill over 1,000 gallons; or
e. Any SSO and/or spill, regardless of volume, that reaches surface water

reported to the Division of Emergency Management at telephone number [phone] or (919)
733-3300 Persons reporting any of the above occurrences shall file a spill report by completing and
submitting Part I of Form CS-SSO (or the most current Division approved form) within five days
following first knowledge of the occurrence. This report must outline the actions taken or proposed
to be taken to ensure that the problem does not recur. Part II of Form CS-SSO (or the most current

Division approved form) can also be completed to show that the SSO was beyond control. [G.S. 143-

8. Failure to abide by the conditions and limitations contained in this permit; 15A NCAC 02T; and other
supporting materials may subject the Permittee to an enforcement action by the Division, in
accordance with North Carolina General Statutes §143-215.6A through §143-215.6C, construction of
additional or replacement wastewater collection facilities, and/or referral of the North Carolina -
licensed Professional Engineer to the licensing board. [15A NCAC 02T .0104 15A NCAC 02T .0108(b-
c)]
9. In the event that the wastewater collection facilities fail to perform satisfactorily including the
creation of nuisance conditions, the Permittee shall cease operation of all pump and haul activities
and shall take immediate corrective action, including those as may be required by this Division, such
as the construction of additional or replacement facilities. [15A NCAC 02T 0110; 15A NCAC 02T
.0108(b)]
10. Any duly authorized officer, employee, or representative of the Division of Water Resources may,
upon presentation of credentials enter and inspect any property, premises, or place on or related to
the pump and haul collection facilities at any reasonable time for the purpose of determining
compliance with the Division's approval, may inspect or copy any records that must be maintained
under the terms and conditions of this permit; and may obtain samples. [15A NCAC 02T .0110]
11. The issuance of this permit shall not exempt the Permittee from complying with any and all statutes,
rules, regulations, or ordinances that may be imposed by the Division any other Federal State or
Local government agencies which have jurisdiction or obtaining other permits which may be
required by the Division or any other Federal State, of Local government agencies. [G.S. 143-
215.1(b)]
12. Operation of this permit is based on acceptance of the certification attached to this permit,
completed by the Permittee and a North Carolina licensed Professional Engineer. Note that a
completed certification is only required for activities that met the definition of the Practice of
Engineering in accordance with G.S. 89C-3. In some instances this certification may not be required.
13. A copy this permit, and approved plans and specifications (if applicable) shall be maintained on file
by the Permittee for the life of the project. [15A NCAC 02T 0104]
14. This permit shall expire December 31, 2017. Should the pump and haul activities stop prior to the
date of expiration send written notification to the Asheville Regional Office, Water Quality Section

A review of Jeld-Wen Fiber of NC's monitoring report for December 2006 showed the
following violations:
Compliance Issue
Failure to submit semi-annual monitoring for outfall 002.
Failure to submit semi-annual monitoring for outfall 003.
Remedial actions should be taken to correct this problem. The Division of Water Quality may
pursue enforcement action for this and any additional violations of State law.
Carefully review these violations and deficiencies and respond in writing to this office within 15
working days of receipt of this letter. You should address the causes of non-compliance and all
actions taken to prevent the recurrence of similar situations. If you should have any questions,
