On June 2, 2011 the Division of Water Quality (DWQ) received your information for the above
referenced project. The DWQ has determined that your application was incomplete and/or provided
inaccurate information as discussed below.
Additional Information Requested:
® The 401 Certification cannot be processed until five (5) complete sets of the Preconstruction
Application Notification associated maps are received at the DWQ Central Office in Raleigh.
There was a revamping of the Preconstruction Notification Notice (PCN) in December 2008.
DWQ is unable to accept the old form.
