Sullins Dee Freeman Governor Director Secretary May 19, 2011 MEMORANDUM To: Stephanie Braquet, Division Environmental Specialist, NCDOT Division 9, e-copy only From: Amy Euliss Subject: Site Visit Follow-up TIP No. 1-2304AC Davidson and Rowan Counties This memo is a follow-up to our site visit of the Yadkin River Bridges and 1-85 Widening project in Davidson and Rowan Counties on May 18, 2011. 1 appreciate the time you and other DOT staff, as well as Flat Iron-Lane staff spent with me with reviewing 401 and Construction Stormwater Permit sites for this project. The following items are provided to help document ourjoint site visit efforts and to assist the NCDOT staff with current project status documentation. There are sites that were not visited on this date. Only sites visited are discussed below. They are discussed in the order they were reviewed. Site 2 We did not visit this site. We are still waiting for turbidity and pH readings from the air drilling operation. No air drilling has occurred in the river, to date. Site 5 Installation of two pipes has been completed. Immediately upstream of the first pipe, two headcuts have formed. I requested installation of rock or log veins be installed to stop the headcut. The first vein needs to be installed immediately upstream of the pipe, where there is an obvious change in grade. The second needs to be installed at the line of orange fencing. If the drop of either vein is greater than 1', additional veins will need to be installed. Details of the log vein and cross vein were emailed to you on May 18, 2011. The section between the pipes has been rock lined. The inlet and outlet of the second pipe look good. Site 9 The pipe had just been laid. The channel inlet was over-excavated to allow for pipe installation and burial. I am concerned a headcut will develop here, as in site 5. 1 am okay with backfilling the hole with rock to bring the channel bed elevation even with the pipe. 1 suggest placing organic soil and/or smaller rock in with the class B to fill the voids. The rock should not be placed above the elevation of the pipe inlet. If a headcut still develops, a rock or log vein will need to be installed. In future pipe installations, 1 would suggest stock piling the material that is removed for the pipe installation, and using it to restore the stream bed to preconstruction contours. The inlet looked as though it had stabilized. Please monitor to see if a headcut forms. A channel has formed inside the pipe. Just beyond the pipe outlet, a silt check needs some maintenance. Once you know the measures you will take to restore the areas mentioned above, we will fill out an in- field modification to handle the changes. Please consult John Thomas with the USACE regarding any changes to the permit drawings. We will be back out in the next couple of months to continue to monitor on-going changes to the permitted areas. Again, thank you for your time.
