In accordance with your application received on February 28, 2002, please find attached Permit
No. WQ0021086 for the construction and operation of the subject wastewater collection system
extension. This Permit shall be effective from the date of issuance until rescinded and shall be
subject to the conditions and limitations as specified therein. This cover letter shall be considered a
part of this permit and is therefore incorporated therein by reference.
Please pay particular attention to condition three (3) of the permit which requires that the
wastewater collection facilities be properly operated and maintained in accordance with 15A NCAC 2H
.0227 or any individual system-wide collection system permit issued to the Permittee.
Permitting of this project does not constitute acceptance of any part of the project that does
not meet 15A NCAC 2H .0200; the Division of Water Quality's (Division) Gravity Sewer Minimum
Design Criteria adopted February 12, 1996 as applicable; and the Division's Minimum Design Criteria
for the Fast-Track Permitting of Pump Stations and Force Mains adopted June 1, 2000 as applicable,
unless specifically mentioned herein. Division approval is based on acceptance of the certification
provided by a North Carolina-licensed Professional Engineer in the application. It shall be the
Permittee's responsibility to ensure that the as-constructed project meets the appropriate design
criteria and rules. Failure to comply may result in penalties in accordance with North Carolina
General Statute 143-215.6A through 143-215.6C, construction of additional or replacement
wastewater collection facilities, and/or referral of the North Carolina-Licensed Professional Engineer
to the licensing board.

In accordance with the provisions of Article 21 of Chapter 143, General Statutes of North
Carolina as amended, and other applicable Laws, Rules, and Regulations, permission is hereby
granted to the City of Hendersonville for the construction and operation of approximately 450 linear
feet of eight inch gravity sewer to serve the Hendersonville Middle School as part of the City of
Hendersonville wastewater collection system and the discharge of approximately 7,500 gallons per
day of collected domestic wastewater into the City of Hendersonville's existing sewerage system,
pursuant to the application received on February 28, 2002, and in conformity with 15A NCAC 2H
.0200; the Division's Gravity Minimum Design Criteria adopted February 12, 1996 as applicable; the
Division's Minimum Design Criteria for the Fast-Track Permitting of Pump Stations and Force Mains
adopted June 1, 2000 as applicable; and other supporting data subsequently filed and approved by
the Department of Environment and Natural Resources and considered a part of this permit.
The sewage and wastewater collected by this system shall be treated in the City of
Hendersonville's Wastewater Treatment facility (NPDES Permit No. NC0025534) prior to being
discharged into Mud Creek.
If any parts, requirements, or limitations contained in this permit are unacceptable, you have
the right to request an adjudicatory hearing upon written request within 30 days following receipt of
this permit. This request must be in the form of a written petition, conforming to Chapter 150E of
North Carolina General Statutes, and filed with the Office of Administrative Hearings, 6714 Mail
Service Center, Raleigh, NC 27699-6714. Unless such demands are made, this permit shall be
final and binding.

This permit shall be effective from the date of issuance until rescinded and shall be subject
to the following specified conditions and limitations:

1. This permit shall become voidable unless the wastewater collection facilities are constructed
in accordance with the conditions of this permit: 15A NCAC 2H .0200; the Division of Water
Quality's (Division) Gravity Sewer Minimum Design Criteria adopted February 12, 1996 as
applicable; the Division's Minimum Design Criteria for the Fast-Track Permitting of Pump
Stations and Force Mains adopted June 1, 2000 as applicable; and other supporting materials
unless specifically mentioned herein.
2. This permit shall be effective only with respect to the nature and volume of wastes described
in the application and other supporting data.
3. The wastewater collection facilities shall be properly maintained and operated at all times.
The Permittee shall maintain compliance with an individual system-wide collection system
permit for the operation and maintenance of these facilities as required by 15A NCAC 2H
.0227. If an individual permit is not required, the following performance criteria shall be met
as provided in 15A NCAC 2H .0227:
a. The sewer system shall be effectively maintained and operated at all times to prevent
discharge to surface waters, and any contravention of the groundwater standards in
15A NCAC 2L .0200 or the surface water standards in 15A NCAC 2B .0200.
b. A map of the sewer system shall be developed prior to January 1, 2004 and shall
be actively maintained.
C. An operation and maintenance plan shall be developed and implemented.
d. Pump stations that are not connected to a telemetry system shall be inspected every
day, as defined in 15A NCAC 213 .0503(5). Pump stations that are connected to a
telemetry system shall be inspected at least once per week.
e. High-priority sewer lines shall be inspected at least once per every six-month period
of time.

f. A general observation of the entire sewer system shall be conducted at least once per
year.
g. Inspection and maintenance records shall be maintained for a period of at least three
years.
h. Overflows and bypasses shall be reported to the appropriate Division Regional Office
in accordance with 15A NCAC 2B .0506(a), and public notice shall be provided as
required by North Carolina General Statute 143-215.1C.
4. This permit shall not be transferable. In the event there is a desire for the wastewater facilities
to change ownership, or there is a name change of the Permittee, a formal permit request
shall be submitted to the Division accompanied by documentation from the parties involved,
and other supporting materials as may be appropriate. The approval of this request shall be
considered on its merits and may or may not be approved.
5. Construction of the gravity sewers shall be scheduled so as not to interrupt service by the
existing utilities nor result in an overflow or bypass discharge of wastewater to the surface
waters of the State.
6. Upon completion of construction and prior to operation of these permitted facilities, a
certification, a copy of the construction record drawings, as well as supporting design
calculations for any pump stations permitted as part of this project shall be received from a
North Carolina-licensed Professional Engineer certifying that the facilities have been installed
in accordance with this permit; 15A NCAC 2H .0200; the Division's Gravity Sewer Design
Criteria adopted February 12, 1996 as applicable; the Division's Minimum Design Criteria for
the Fast-Track Permitting of Pump Station and Force 'Main adopted June 1, 2000 as
applicable; and other supporting materials. If this project is to be completed in phases and
partially certified, you shall retain the responsibility to track further construction approved
under the same permit, and shall provide a final certificate of completion once the entire
project has been completed. A copy of the construction record drawings, indicating the
facilities constructed in the phase being certified, shall be submitted with each partial
certification. Mail the Engineer's Certification, one copy of the "Construction Record
Drawings", and one copy of the supporting design calculations to the Non-Discharge
Permitting Unit, 1617 Mail Service Center, Raleigh, NC 27699-1617.
7. A copy of the construction record drawings shall be maintained on file by the Permittee for the
life of the wastewater collection facilities.
8. Failure to abide by the conditions and limitations contained in this permit; 15A NCAC 2H
.0200; the Division's Gravity Sewer Design Criteria adopted February 12, 1996 as applicable;
the Division's Minimum Design Criteria for the Fast-Track Permitting of Pump Station and
Force Mains adopted June 1, 2000 as applicable; and other supporting materials may subject
the Permittee to an enforcement action by the Division, in accordance with North Carolina
General Statutes 143-215.6A through 143-215.6C.

9. In the event that the wastewater collection facilities fail to perform satisfactorily, including the
creation of nuisance conditions, the Permittee shall take immediate corrective action,
including those as may be required by this Division, such as the construction of additional or
replacement facilities.
10. The issuance of this permit shall not exempt the Permittee from complying with any and all
statutes, rules, regulations, or ordinances that may be imposed by other government agencies
(local, state and federal) which have jurisdiction, including but not limited to applicable river
buffer rules in 15A NCAC 2B .0200, erosion and sedimentation control requirements in 15A
NCAC Ch. 4 and under the Division's General Permit NCG0100000, and any requirements
pertaining to wetlands under 15A NCAC 2B .0200 and 15A NCAC 2H .0500.
11. Non-compliance Notification:
The Permittee shall report by telephone to the Asheville Regional Office, telephone number
828/251-6208 as soon as possible, but in no case more than 24 hours or on the next working
day, following the occurrence or first knowledge of the occurrence of either of the following:
a. Any process unit failure, due to known or unknown reasons, that renders the facility
incapable of adequate wastewater transport, such as mechanical or electrical failures of
pumps, line blockage or breakage, etc.; or
b. Any failure of a pumping station or sewer line resulting in a by-pass directly to receiving
waters without treatment of all or any portion of the influent to such station or facility.
Persons reporting such occurrences by telephone shall also file a written report in letter form
within five days following first knowledge of the occurrence. This report must outline the actions
taken or proposed to be taken to ensure that the problem does not recur.

INSTRUCTIONS: Indicate that you have included the following list of required application package items by signing your
initials in the space provided next to each item. Failure to submit all required items will lead to additional processing and review
time or the permit application.
A. Application Form-Submit one original and one copy of the completed and appropriately executed application form.
Any changes to this form will result in the application being retumed. The Division of Water Quality(Division)will
only accept application packages that have been fully completed with all applicable items addressed. You do not need
to submit detailed plans and specifications unless You respond NO to item IV 6 and/or item IV.10.
A. Attachment- Submit the completed and properly executed Form WSCA.10/99 for each watershed within the project
location.

_D. Certificates of Public Convenience and Necessity—If the application is being submitted in the name of a privately-
owned public utility, submit two copies of the Certificate of Public Convenience and Necessity, which demonstrates
that the public utility is authorized to hold the utility franchise for the area to be served by the sewer extension. If a
Certificate of Public Convenience and Necessity has not been issued, provide two copies of a letter from the NC
Utilities Commission's Public Staff that states that an application for a franchise has been received,that the service area
is contiguous to an existing franchised area,and/or that franchise approval is expected.
_ E. Operational Agreements— Submit one original and two copies of a properly executed operational agreement if the
sewer extension will be serving residential or commercial lots (e.g., houses, condominiums, townhomes, outparcels,
etc.)that will be owned by a homeowners'association or developer. If the applicant is a homeowners' association,use
Form HOA 10/99. If the applicant is a developer,use Form DEV 10199.
_ F. Flow Acceptance Letters — If the owner of the downstream sewers and/or WWTF is different from the applicant,
submit two copies of a flow acceptance letter from the owner of the downstream sewers and W WTF. Flow acceptance
letters must contain the following minimum information: applicant and project name, amount of flow accepted,and
name and permit number of the receiving sewers/WWTF. The flow acceptance must not expire prior to permit
issuance and must be dated less than a year prior to the application date. Intergovernmental agreements or other
contracts will not be accepted in lieu of a project-specific flow acceptance letter.

These specifications shall apply to the materials to be famished to complete the installation of the sanitary
sewer lines shown on the contract drawings. All pipe and appurtenant materials shall be of the class and type
as indicated on the drawings and as designated herein.
B. GENERAL RE0LME,,yfEhTS
All materials shall be first quality with smooth interior and exterior surfaces, free from cracks, blisters,
honeycombs and other imperfections, and true to theoretical shapes and forms throughout. All materials shall
be subject to the inspection of the Engineer at the plant, trench, or other point of delivery, for the purpose shall
culling and rejecting materials, which do not conform to the requirements of these specifications. Such
materials shall be marked by the Engineer and the Contractor shall remove it from the project site upon notice
being received of its rejection.
As particular specifications are cited, the designation shall be construed to refer to the latest revision under the
same specification number, or to superseding specifications under a new number except provisions in revised
specifications which are clearly inapplicable.
C. DLICCq,E IRON PIPE(DIP)(Standard)
Ductile iron pipe shall be as manufactured in accordance with ASTM A-746,ANSI Specification A21.50 and
A21.51 and shall be Class 50 unless otherwise specified on the drawings.
The pipe interior shall be cement mortar lined and seal coated, standard thickness, in accordan

A21.4.
The exterior of all pipe shall be coated with either a coal or asphaltic base bituminous pipe coating in
accordance with ANSI Specifications A21.8.
Pipe shall be famished with Slip Joints,Mechanical Joints, or Flanged Joints as indicated on the drawings and
in accordance with the specifications descried below:
(a) Slip Joints-This pipe joint shall be done by guiding the pin end of the pipe into the bell
end until contact is made with a gasket and by exerting a sufficient compressive force to
drive the plain end through the gasket until the plain end makes full contact with the base of
the bell.
Bells of slip joint pipe shall be contoured to receive a circular rubber gasket and plain ends
shall have a slight taper to facilitate installation.
The circular gasket shall be firmished by the pipe manufacturer and shall be manufactured
in accordance with ANSI Specification A21.11.
The pipe manufacturer shall also furnish the lubricant used to assist in the pipe installation.
(b) Mechanical Joints - This pipe joint is essentially the same as the slip joint except it is
furnished with a cast iron clamp which acts as a retainer to hold the circular rubber gasket in
place. All mechanical type joints shall be fimtished by the pipe manufacturer and
manufactured in accordance with ANSI Specification A21.11.
All bolts shall be tightened by means of torque wrenches in such a manner that the follower
shall be brought up toward the pipe evenly. If effective sealing is not obtained by

tightening the bolts to the specified. torques, the joint shall be disassembled and
reassembled after thorough cleaning.
(c) Flanged Joints - The flanged pipe joint is composed of flat steel plate shop tined on the
threaded end of the ductile iron pipe. The flanges shall be accurately faced at right angles to
the pipe axis and shall be drilled smooth and true.
Flanged joints shall be furnished with 125 lb. flanges drilled in accordance with ANSI
Specification B 16.1.
In geneml, flanged joints shall be made up with through bolts of the required size. Stud or
rap bolts shall be used only where shown or required.
Gaskets for flanged joints shall be the ring type of cloth inserted rubber or rubber with a
minimum thickness of 1/8 inch.
Connecting flanges shall be in proper alignment and no external force shall be used to bring
i them together. Bolts and gaskets shall be furnished by the installer of piping for joints
connecting the piping with equipment, as well as for those between pipe and fittings,
whether such equipment and piping is famished by the installer or not.
D. DUCTILE IRON PIPEL(Al na Lined)
ail
Ductile iron pipe shall be manufactured the same as standard specifications (ASTbi A-746, ANSI A2I.50 &
21.51)with the following coatings.
a• Outside Coating shall be a minimum of 1 mil red alkyd paint The last 6 in. of spigot, including the
spigot face, shall be coated with 8 mils of epoxy to prevent corrosion in the joint All pipe to be
stamped"NOT FOR POTABLE WATER"
b. Inside Lining shall be a high alumina cement mortar meeting all requirements of ANSI 21.4 and
Griffm Pipe Products Company or equal. The minimum lining thickness shall be 0.281 in. with
tapering permitted at the ends. Alumina cement shall be cement fondu as manufactured by LaFarge
Aluminous Cement Co.,Ltd.,or approved equal.
C. Inside Coating shall be standard seal coat over the high alumna cement lining. The entire inside of
the socket, including the gasket cavity shall be coated with a minimum of 8 mils of epoxy to prevent
corrosion in the joint
d• Whenever a thickness class of 47 through 49 is stated on the contract drawings, the pipe shall be
subject to the manufacturer's standards and the Engineer's written approval.
E. DUCTR E IRON PIPE(EPDXY i MD)
Ductile iron pipe shall be manufactured the same as standard specifications(ASTIM A-746,ANSI A21.50 and
21.51)with the following coatings:

b. Inside lining shall be a Koppers Coal Tar Epoxy 300 M applied in three (3) passes 8 mil dry
thickness to a total thickness of 24 mils. The lining shall cover the entire inside surface area of the
pipe socket with a minimum 8 mil dry thickness.

c. Whenever a thickness class of 47 through 49 is stated on the contract drawings, the pipe shall be
subject to the manufacturer's standards and the Engineefs written approval.
F. POLYVINYL CHI ORME omc hoh/ 1
Polyvinyl chloride pipe shall be as manufactured in accordance with ASTM D_3034. latest edition, and shall
be suitable for use as a gravity sanitary sewer pipe. The standard dimension ratio (SDR) shall be 35 unless
otherwise specified on the contract.
All polyvinyl chloride pipe shall be of an integral bell and spigot of the same material as the pipe. It shall have
a solid cross section, with rubber"0"ring securely locked in place at the point of manufacture.

The pipe manufacturer shall famish to the Engineer certification from an independent laboratory that the
alkalinity of the pipe is a least 90%calcium carbonate equivalent Certification shall also be furnished by the
pipe manufacturer stating the type of aggregate used in the pipe.
I. ELEMLE COTMINGS
Whenever it becomes necessary to join sewer pipe lines of dissimilar materials or pipe sizes, it shall be
required to use a flexible coupling. The coupling shall be made of virgin polyvinyl chloride(PVC),shall not
harden, and shall be impervious to all known soil conditions. The coupling shall provide a permanent
leakproof seal approved by the Southern Building Code Congress and manufactured in accordance'with
ASTM#C-594-70. The couplings shall be as manufactured by Fernco Joint Sealer Co. or an approved equal.

A. DIUMIMON OF WORK
The work covered under this section shall consist of dirnishing all labor, equipment, and services for the
installation of sanitary sewer piping as shown on the drawings and specified herein and in agreement with the
General Conditions of these contract documents.

The Contractor shall unload material so as to avoid deformation or other injury thereto. Material shall not be
rolled or dragged over gravel or rock during handling. The Connector shall store the appurtenances on sills
above storm drainage level and deliver installations after the trench is excavated. When any material is
damaged during transporting, unloading, handling or storing, the undamaged portions may be used or, if
damaged sufficiently, the Engineer will reject the material as being unfit for installation.
If any defective material is discovered after installation, it shall be removed and replaced with sound material
or shall be repaired by the Contractor in an approved manner and at his own expense.
C. FIEF ALIQDnMFNTAAID GgAbE
The layout of underground sanitary sewer lines and invert elevations at governing points are as shown on the
drawings.
The Contractor shall do all layout work for lines and grades from the information shown on the drawings or as
fiunished by the Engineer.
When a laser bears instrument is used to set line and grade, the unit must be maintained in good working
order, and the calibration checked daily for both alignment and percent grade. In the event the required
accuracy of alignment and grade is rut adhered to,the Engineer will prohibit the use of laser beams.
The Engineer shall field verify as-built invert elevations of all manholes as the work progresses
(approximately each 1,000 L.F.of line construction). The as-constructed elevations shall be within a tolerance
of 0.020 ft.± of the design elevations or the Contractor may be required to remove and relay the non-
conforming work at his expense. Field survey work by the Engineer shall not relieve the Contractor of his
responsibility for field layout
D. MPARATIMWE RIPE FOUNDATION
The pipe foundation shall be prepared to be uniformly firm and shall be true to the lines and grades as shown
on the plans. The preparation of the pipe bedding shall be in accordance with the typical trench cross sections
as shown on the drawings. Under no circumstances shall pipe be laid in water, on rock, or when trench
conditions or weather is unsuitable for such work.
Whenever the nature of the ground will permit, the excavations at the bottom of the trench shall have the
shape and dimensions of the outside lower thud of the circumference of the pipe,care being taken to secure a
firm bearing support uniformly throughout the length of the pipe. A space shall be excavated under and
around each bell to sufficient depth to relieve it of any load and to allow ample space for filling and finishing
the joint The pipe, when thus bedded firmly, shall be on the exact grade. In case the bed shaped in the
bottom of the trench is too low, the pipe shall be completely removed from position and earth of suiitable
quality shall be placed and thoroughly tamped to prepare a new foundation for the pipe. In no case shall the
pipe be of same, but a new and uniform support must be provided for the full length of the pipe. Where rock

or boulders are encountered in the bottom of the trench, the same shall be removed to such depth that no part
of the pipe, when laid to grade, will be closer to the rock or boulders than six (6) inches. A suitably tamped
ind shaped foundation of approved material shall be placed to bring the bottom of the trench to proper
subgrade over rock or boulders.
Where the foundation material is found to be of poor supporting value, the Engineer may make minor
adjustment in the location of the pipe to provide a more suitable foundation. Where this is not practical, the
foundation shall be conditioned by removing the existing foundation material by undercutting to the depth as
directed by the Engineer, within the limits established on the plans, and backfilling with either an approved
material secured from unclassified excavation or borrow excavation at the nearest accessible location along the
project,or foundation conditioning material consisting of crushed stone or gravel or a combination of sand and
crushed stone or gravel approved by the Engineer as being suitable for the purpose intended. The selection of
the type of backfill material to be used for foundation conditioning will be made by the Engineer.
The Contractor shall remove all water which may be encountered or which may accumulate in the trenches by
Pumping or bailing and no pipes shall be laid until the water has been removed from the trench. Water so
removed from the trench must be disposed of in such a manner as not to cause injury to work completed e r in
progress.
E. PIPE LAyIN�
Pipe shall be laid with bell ends facing in the direction of pipe laying, unless directed otherwise by the
Engineer. In all cases, pipe is to be installed in strict accordance with the manufacturer's recommendations
and the contract material specifications. The Engineer may augment any manufacturer's installation recommendations if in his opimiat it will best serve the interest of the Owner.
Proper tools, implements,and facilities satisfactory to the Engineer shall be provided and used for the safe and
convenient prosecution of pipe laying. All pipe and other materials used in the laying of pipe will be lowered
into the trench piece by piece by means of suitable equipment in such a manner to prevent damage to the pipe,
materials, to the protective coating on the pipe materials, and to provide a safe working condition to all
personnel in the trench. Each piece of pipe being lowered into the trench shall be clean, sound and free from
defects. It shall be laid on the prepared foundation, as specified elsewhere to produce a straight line on a
uniform grade, each pipe being laid so as to form a smooth and straight inside flow line. Pipe shall be
removed at any time if broken,injured or displaced in the process of laying same,or of backfilling the trench.
When cutting short lengths of pipe, a pipe cutter as approved by the Engineer will be used and care will be
taken to make the cut at right angles to the centerline of the pipe or on the exact skew as shown on the plans.
In the case of push-on pipe, the cut ends shall be tapered with a portable grinder, or coarse file to match the
manufactured taper.
During times when pipe laying is not in progress, the open ends of pipe shall be closed and no trench water or
other material shall be permitted to enter the pipe.
Where the pipe is laid on a grade of 10 percent or greater, the laying shall start at the bottom of the slope and
proceed upward with the bell end of the new pipe upgrade. All pipe laid on a grade of 10 percent or greater
shall require thrust blocking and keying as shown on the drawings and standard details.
Where pipe lines of different materials are joined together,a standard sewer repair coupling shall be used.The
couplings shall be Eastern Standard Sewer Repair Couplings (Mission Rubber Company), the Femco Joint
Sealer Company,or an equal product approved by the Engineer.

All backfill shall be compacted so as not to damage the pipe and appurtenances and shall be compacted to 95
percent of the Standard Proctor Test for the various types of backfill materials.
Methods of backfilling shall be in strict accordance with the pipe manufacturees recommendations. All
backfill materials shall have been approved by the Engineer. Select backfill material shall be used when
requested by the Engineer. Select material shall be defined as a finely graded material free from stones over
lit-inch in diameter,plastic clays,organic material, frozen lumps
the Engineer prior to its installation. and various debris and shall be approved by

Quantities of pipe installed in accordance with the requirements of this section shall be verified in the field by
the Engineer. Payment shall be made for total number of lineal feet of pipe installed at the unit prices bid in
each depth category. Price and payment shall constitute full compensation for furnishing all equipment, tools,
labor,and materials to complete the work as specified.

[YSPECTION AND TESTING
A. DESCRIPTION OF WORK
The work covered in this section shall consist of furnishing all labor, equipment, and services for the
Proper inspection and testing of the gravity sanitary sewer pipe lines installed in accordance with Section
07260.

Prior to inspection of any section(s) of gravity sanitary sewer pipe, the Contractor shall completely clean
the lines of all debris,silt, etc. The pipe line shall be ready for use by the Owner and shall be proved to be
in first class condition and constructed properly in accordance with the drawings and specifications.
The Contractor shall maintain the project, insofar as his construction work is concerned, in first class
condition for such time as it is necessary to satisfy the Engineer that all installations are correct and
acceptable.
C. INSPECTION AND rNFILTRAT(ON
Alignment and grade between manholes shall be tested by.the Engineer by flashing a light between
manholes. A practically full circle of light shall be seen when reviewed from the adjoining end of the
line. All defects disclosed as a result of this test shall be corrected by the Contractor at his expense.
PVC pipe shall pass go-no-go mandrel sized to 95 percent of the pipe diameter with the pipe in place and
properly backfilled. All pipe which will not pass the Mandrel shall be relayed or replaced by the
Contractor at no additional cost The allowable deflection(less than 5 percent) shall be calculated using
the pipe stiffness formula in ASTM D2321.
When the sewers are completed, they shall be inspected by the Engineer for conformance with the
Provisions of the plans and specifications particularly line and grade, and tested to determine the amount
Of ground water infiltration into the sewer. All visible and audible leaks will be stopped and the
remaining infiltration will be measured using a V-notch weir and/or other devices, which sball be
fimnished by the Contractor. The Contractor shall also famish all required assistance for measuring the
infiltration:
If infiltration into the whole system or any segment thereof exceeds 100 gallons per 24 hours per inch of
pipe diameter per mile of sewer, necessary corrective measures shall be taken by the Contractor to limit
the infiltration to the maximum specified above. The Engineer shall decide the number and length of
segments of sewer line on which the testing shall be performed.
D. AIR TESTING,
All gravity sanitary sewer lines may be subjected to a low pressure air test to determine the presence of
damaged pipe or faulty installation. If directed by the Engineer to conduct an air test the Contractor will
firrnish all facilities and personnel for conducting the test(s).
The acceptance air test shall be made after backfilling has been completed and compacted and in the
presence of the Engineer. The test shall be performed as described under ASTNIC-828, latest edition,
Standard Practice for Low Pressure Air Testing of V.C. Pipelines.

Failure of any section of the pipeline to meet the requirements of this test shall cause the'Contmctor to
determine, at his own expense, the sourc (s) of leakage, and repair or replace all defective materials or
workmanship. The repaired secdon(s) of line shall be re-tested to insure conformance

E. FINAL ACCEPTANCE
The Engineer will notify the Contractor, in writing, as to satisfactory letion of the work in any or all comp
sections of gravity sanitary sewer pipe included in the project

materials, tools, debris, etc., from the site(s) and leave the same in a neat, orderly condition acceptable to
the Engineer.
Final landscaping*requirements and restoration of surfaces shall then be completed by the Contractor in
accordance with their respective specifications and as shown on the drawings-

INSTALLATION, & TESTING
A. DESCRIPTION OF WORK
The work covered under this section shall consist of furnishing all materials. labor, equipment. and
services for the complete installation of sanitary sewer manholes.
H. -MA`IHOLE CONSTRUCTION
Sanitary sewer manholes shall be installed at each break in line or grade in sanitary sewer line as shown
on the contract drawings.
The manholes shall consist of precast reinforced concrete sections including a base section and conical top
section. These precast concrete sections shall conform to ASTM C-478, latest revision.
The minimum inside diameter of the manhole shall be five (5) feet unless noted otherwise on the
drawings. Flat slab tops may be used in non-traffic areas, for very shallow manholes, or where shown on
the drawings or approved by the Engineer.
Maximum allowable absorption of concrete shall be 8.0 percent. Minimum compressive strength of
concrete for sections shall be 4000 psi. Each precast concrete section shall be joined together with an "O"
ring rubber gasket conforming to Each
C-443 or an approved sealant conforming to Federal
Specifications 55500210.
Manhole inverts shall be constructed of concrete or concrete and brick of semicircular section conforming
to the inside diameter of the outlet sewer. Changes in size of pipe or grade shall be made gradually and
change in direction constructed by using true curves. Each manhole shall be provided with such channels
for all connecting sewer pipes.
C. MANHOLE INSTALLATION
The manhole foundation shall be prepared so as to provide a firm, level area on which to place the precast
concrete manhole base section When poor foundation soil is encountered or excess groundwater exists,
the foundation shall be excavated 12 inches below the final subgrade elevation backfilled with wash
stone to provide a proper foundation ed
The manhole sections shall be lifted from the side of the excavation to the bottom of the trench with
equipment and support slings capable of safely handling the heavy concrete pieces. The manhole shall be
set plumb and the rim and lid adjusted to the final finished surface grade with brick and mortar.
Pipe openings shall be exactly aligned to that of the pipe entering and leaving the manhole. The gravity
sanitary sewer pipe lines shall be placed in the manhole openings, properly aligned, and set to grade. Pipe
shall be firmly held in place, and opening around the exterior diameter of the pipe and the base opening
shall be filled with an expanding type or non-shrink type grout well rammed into place or with a flexible
Pipe connector in accordance with ASTM C-923 to provide a watertight connection.
Manhole steps shall be properly spaced as shown in the standard detail drawing but in no case more than
IS inches on center. The steps shall be made of heavy cast iron (1 inch thick), reinforced plastic(ASTM
2146-Type m, or aluminum(min. load 100 tbs.).

The Standard manhole casting shall be designed for heavy duty use with a 210 pound frame and l30
pound cover. Acceptable products include Sumter Machinery Company (4h[C-13 & 0,117-ll), Dewey
Brothers Foundry (#MHRCR-2001) or an approved equal
The frame and cover shall be properly set in a bed of mortar and aligned to fit the top section of the
manhole. Concrete brick,set in mortar, shall be used to adjust the top of the frame and cover to finished
grade; however, no more than four(4)courses of brick will be used for adjustment.
E. MANHOLE INVERTS
Manhole inverts and benches shall be constructed in accordance with the standard details shown on the
drawings. Invert shall be a U-shaped channel with a height of 0.8 of the diameter and be a smooth
continuation of the pipe. The benches shall be constructed with a slope of one (1) inch per foot to the
channel. The channel and invert shall be constricted with a minimum of 2000 psi concrete or brick fill
with concrete finish minimum one (1) inch thick. Where sewer changes directions at the manhole,
channel shall be constructed with a smooth curve with as large a radius as the diameter of the manhole
will allow.
F. MANHOLE
Standard drop manholes will be constructed only at those locations shown on the drawings or as approved
by the Engineer. The design of the drop connection shall be in accordance with the standard detail
drawing.
The cost of the extra pipe, labor, etc., required to construct a drop manhole will be included in the unit
price for the drop manhole at the depths listed.
G. INSPECTION AND ACCEPTANCE
Manholes shall be constructed to provide a true circular inside diameter with properly corbeled tops,'
satisfactory inverts and properly placed steps and castings., Any visible leaks in the manholes shall be
completely stopped to the satisfaction of the Engineer.
H. MANHOLE
Whe

Please pay particular attention to condition three (3) of the permit which requires that the
wastewater collection facilities be properly operated and maintained in accordance with 15A NCAC 2H
.0227 or any individual system-wide collection system permit issued to the Permittee.
Permitting of this project does not constitute acceptance of any part of the project that does
not meet 15A NCAC 2H .0200; the Division of Water Quality's (Division) Gravity Sewer Minimum
Design Criteria adopted February 12, 1996 as applicable; and the Division's Minimum Design Criteria
for the Fast-Track Permitting of Pump Stations and Force Mains adopted June 1, 2000 as applicable,
unless specifically mentioned herein. Division approval is based on acceptance of the certification
provided by a North Carolina-licensed Professional Engineer in the application. It shall be the
Permittee's responsibility to ensure that the as-constructed project meets the appropriate design
criteria and rules. Failure to comply may result in penalties in accordance with North Carolina
General Statute 143-215.6A through 143-215.6C, construction of additional or replacement

Carolina as amended, and other applicable Laws, Rules, and Regulations, permission is hereby
granted to the City of Hendersonville for the construction and operation of approximately 735 linear
feet of eight inch gravity sewer to serve the mobile home park being constructed by JLB
Construction, LLC as part of the City of Hendersonville wastewater collection system and the
discharge of approximately 2,520 gallons per day of collected domestic wastewater into the City of
Hendersonville's existing sewerage system, pursuant to the application received on February 28,
2002, and in conformity with 15A NCAC 2H .0200; the Division's Gravity Minimum Design Criteria
adopted February 12, 1996 as applicable; the Division's Minimum Design Criteria for the Fast-Track
Permitting of Pump Stations and Force Mains adopted June 1, 2000 as applicable; and other
supporting data subsequently filed and approved by the Department of Environment and Natural
Resources and considered a part of this permit.
The sewage and wastewater collected by this system shall be treated in the City of
Hendersonville's Wastewater Treatment facility (NPDES Permit No. NC0025534) prior to being
discharged into Mud Creek.
If any parts, requirements, or limitations contained in this permit are unacceptable, you have
the right to request an adjudicatory hearing upon written request within 30 days following receipt of
this permit. This request must be in the form of a written petition, conforming to Chapter 150B of
North Carolina General Statutes, and filed with the Office of Administrative Hearings, 6714 Mail
Service Center, Raleigh, NC 27699-6714. Unless such demands are made, this permit shall be
final and binding.

This permit shall be effective from the date of issuance until rescinded and shall be subject
to the following specified conditions and limitations:
1. This permit shall become voidable unless the wastewater collection facilities are constructed
in accordance with the conditions of this permit: 15A NCAC 2H .0200; the Division of Water
Quality's (Division) Gravity Sewer Minimum Design Criteria adopted February 12, 1996 as
applicable; the Division's Minimum Design Criteria for the Fast-Track Permitting of Pump
Stations and Force Mains adopted June 1, 2000 as applicable; and other supporting materials
unless specifically mentioned herein.
2. This permit shall be effective only with respect to the nature and volume of wastes described
in the application and other supporting data.
3. The wastewater collection facilities shall be properly maintained and operated at all times.
The Permittee shall maintain compliance with an individual system-wide collection system
permit for the operation and maintenance of these facilities as required by 15A NCAC 2H
.0227. If an individual permit is not required, the following performance criteria shall be met
as provided in 15A NCAC 2H .0227:
a. The sewer system shall be effectively maintained and operated at all times to prevent
discharge to surface waters, and any contravention of the groundwater standards in
15A NCAC 2L .0200 or the surface water standards in 15A NCAC 2B .0200.
b. A map of the sewer system shall be developed prior to January 1, 2004 and shall
be actively maintained.
C. An operation and maintenance plan shall be developed and implemented.
d. Pump stations that are not connected to a telemetry system shall be inspected every
day, as defined in 15A NCAC 2B .0503(5). Pump stations that are connected to a
telemetry system shall be inspected at least once per week.
e. High-priority sewer lines shall be inspected at least once per every six-month period
of time.

f. A general observation of the entire sewer system shall be conducted at least once per
year.
g. Inspection and maintenance records shall be maintained for a period of at least three
years.
h. Overflows and bypasses shall be reported to the appropriate Division Regional Office
in accordance with 15A NCAC 2B .0506(a), and public notice shall be provided as
required by North Carolina General Statute 143-215.1 C.
4. This permit shall not be transferable. In the event there is a desire for the wastewater facilities
to change ownership, or there is a name change of the Permittee, a formal permit request
shall be submitted to the Division accompanied by documentation from the parties involved,
and other supporting materials as may be appropriate. The approval of this request shall be
considered on its merits and may or may not be approved.
5. Construction of the gravity sewers shall be scheduled so as not to interrupt service by the
existing utilities nor result in an overflow or bypass discharge of wastewater to the surface
waters of the State.
6. Upon completion of construction and prior to operation of these permitted facilities, a
certification, a copy of the construction record drawings, as well as supporting design
calculations for any pump stations permitted as part of this project shall be received from a
North Carolina-licensed Professional Engineer certifying that the facilities have been installed
in accordance with this permit; 15A NCAC 2H .0200; the Division's Gravity Sewer Design
Criteria adopted February 12, 1996 as applicable; the Division's Minimum Design Criteria for
the Fast-Track Permitting of Pump Station and Force Main adopted June 1, 2000 as
applicable; and other supporting materials. If this project is to be completed in phases and
partially certified, you shall retain the responsibility to track further construction approved
under the same permit, and shall provide a final certificate of completion once the entire
project has been completed. A copy of the construction record drawings, indicating the
facilities constructed in the phase being certified, shall be submitted with each partial
certification. Mail the Engineer's Certification, one copy of the "Construction Record
Drawings", and one copy of the supporting design calculations to the Non-Discharge
Permitting Unit, 1617 Mail Service Center, Raleigh, NC 27699-1617.
7. A copy of the construction record drawings shall be maintained on file by the Permittee for the
life of the wastewater collection facilities.
8. Failure to abide by the conditions and limitations contained in this permit; 15A NCAC 2H
.0200; the Division's Gravity Sewer Design Criteria adopted February 12, 1996 as applicable;
the Division's Minimum Design Criteria for the Fast-Track Permitting of Pump Station and
Force Mains adopted June 1, 2000 as applicable; and other supporting materials may subject
the Permittee to an enforcement action by the Division, in accordance with North Carolina
General Statutes 143-215.6A through 143-215.6C.

9. In the event that the wastewater collection facilities fail to perform satisfactorily, including the
creation of nuisance conditions, the Permittee shall take immediate corrective action,
including those as may be required by this Division, such as the construction of additional or
replacement facilities.
10. The issuance of this permit shall not exempt the Permittee from complying with any and all
statutes, rules, regulations, or ordinances that may be imposed by other government agencies
(local, state and federal) which have jurisdiction, including but not limited to applicable river
buffer rules in 15A NCAC 2B .0200, erosion and sedimentation control requirements in 15A
NCAC Ch. 4 and under the Division's General Permit NCG0100000, and any requirements
pertaining to wetlands under 15A NCAC 2B .0200 and 15A NCAC 2H .0500.
11. Non-compliance Notification:
The Permittee shall report by telephone to the Asheville Regional Office, telephone number
828/251-6208 as soon as possible, but in no case more than 24 hours or on the next working
day, following the occurrence or first knowledge of the occurrence of either of the following:
a. Any process unit failure, due to known or unknown reasons, that renders the facility
incapable of adequate wastewater transport, such as mechanical or electrical failures of
pumps, line blockage or breakage, etc.; or
b. Any failure of a pumping station or sewer line resulting in a by-pass directly to receiving
waters without treatment of all or any portion of the influent to such station or facility.
Persons reporting such occurrences by telephone shall also file a written report in letter form
within five days following first knowledge of the occurrence. This report must outline.the actions
taken or proposed to be taken to ensure that the problem does not recur.

that the public utility is authorized to hold the utility franchise for the area to be served by the sewer extension. If a
Certificate of Public Convenience and Necessity has not been issued, provide two copies of a letter from the NC
Utilities Commission's Public Staff that states that an application for a franchise has been received,that the service area
is contiguous to an existing franchised area,and/or that franchise approval is expected.
E. Operational Agreements—Submit one original and two copies of a properly executed operational agreement if the
sewer extension will be serving residential or commercial lots (e.g., houses, condominiums, townhomes, outparcels,
etc.)that will be owned by a homeowners' association or developer. If the applicant is a homeowners'association,use
Form HOA 10/99. If the applicant is a developer,use Form DEV 10199.
F. Flow Acceptance Letters— If the owner of the downstream sewers and/or WWTF is different from the applicant,
submit two copies of a flow acceptance letter from the owner of the downstream sewers and W WTF. Flow acceptance
letters must contain the following minimum information: applicant and project name, amount of flow accepted, and
name and permit number of the receiving sewers/WWTF. The flow acceptance must not expire prior to permit
issuance and must be dated less than a year prior to the application date. Intergovernmental agreements or other
contracts will not be accepted in lieu of a project-specific flow acceptance letter.

The Contractor shall plan his activities so that all materials
and equipment can be stored within the limits of construction.
01500 .5 CONTROL OF EROSION, SILTATION AND POLLUTION
The Contractor shall take whatever measures necessary to
minimize soil erosion and siltation, water and air pollution
caused by his operations. The Contractor shall also comply with
the applicable regulations of all legally constituted
authorities relating to pollution prevention and control.
The Contractor shall keep himself fully informed of all such
regulations which in any way affect the conduct of the work, and
shall at all times observe and comply with all such regulations.
In the event of conflict between such regulations and the '
requirements of the specifications, the more restrictive
requirements shall apply.
The Engineer will limit the area over which clearing and
grubbing, excavation, borrow, and embankment operations are
performed whenever the Contractor' s operations do not , make .
effective use of construction practices and temporary measures
which will minimize erosion, or whenever construction operations
have not been coordinated to effectively minimize erosion, or
whenever permanent erosion control features are not being
completed as soon as permitted by construction operations .
The Contractor shall control dust throughout the life of
the project within the project area and at all other areas
affected by the construction of the project, including, but not
specifically limited to, unpaved secondary roads, haul roads,
access roads, disposal sites, borrow and material pits, and
production sites Dust control shall not be considered effective
where the amount of .dust creates a potential or actual unsafe
condition, public nuisance, or condition endangering the value,
utility, or appearance of any property.
The Contractor will not be directly compensated for any dust
control measures necessary, as this work will be considered
incidental to the work covered by the various contract items.
01500 . 6 RIGHT TO STOP WORK

The Engineer or his authorized representative has the authority
to stop the Contractor from further work progress if, during
construction, the Engineer deems the work being performed is not
in compliance with these contract documents .
01500.7 WORK MANAGEMENT
The Contractor shall place in charge of the work a competent and
reliable superintendent, who shall have the authority to act for
the Contractor and who shall be accountable to the Engineer. The
Contractor shall, at all times, employ labor and equipment
sufficient to accomplish the several classes of work to full
completion in the manner and time specified.

The Contractor shall provide, erect, and maintain all necessary
devices to control traffic and protect the public, the work and
workers. All traffic control shall be provided as established in
The Manual of Uniform Traffic Control Devices and any and all
supplements thereto of the North Carolina Department ' of
Transportation.
In special cases, as deemed necessary by the Engineer,
additional traffic control may be required as directed, by the.
Engineer.
01500.9 ENCROACHMENT/EASEMENT AGREEMENTS
It shall be the responsibility of the contractor to abide by any
and all conditions of any and all easements and/or encroachments
which are necessary for the accommodation of the work.
SECTION 02200 SEEDING
PART1 -GENERAL
All grassed areas disturbed during construction of this project shall be reseeded.
WORK INCLUDED:
Preparation of sub-grade to receive topsoil.
Spreading topsoil.
Seeding and fertilizing.
Seed protection on slopes.
Maintaining seeded areas until acceptance.
DELIVERY, STORAGE,AND HANDLING:
Deliver grass seed in original containers showing analysis of seed mixture,percentage of pure seed,year
of production, net weight, date of packaging and location of packing. Damaged packages are not
acceptable.
Deliver fertilizer and lime.,in waterproof bags showing weight, chemical analysis, and name of
manufacturer.
EXISTING CONDITIONS:
Beginning work of this Section means acceptance of existing conditions.
PART2 -PRODUCTS
GROWING MEDIA:
Fertilizer: 10-20-10, commercial type with 50 percent of the elements derived from organic sources.
Lime: Agricultural limestone not less than 85% of calcium carbonate and magnesium carbonate
combined.

Do not apply grass seed and fertilizer at same time, in same machine.
SEEDING:

Do not sow immediately following rain, when ground is too dry, or during windy periods.
Roll seeded area with roller not exceeding 112 lbs.
SEED PROTECTION ON SLOPES:
Cover seeded slopes where grade is 3:1 or greater with jute matting. Roll matting down over slopes
without stretching or pulling.
Lay matting smoothly on soil surface,burying top end of each section in narrow 6 inches trench. Leave
12 inches overlap from top roll over bottom roll. Leave 4 inches overlap over adjacent section.
Staple outside edges and overlaps at 36-inch intervals.
Lightly dress slopes with topsoil to ensure close contact between matting and soil.
In ditches,unroll matting in direction of flow. Overlap ends of strips 6 inches with upstream section on
top.
MAINTENANCE PERIOD:
Maintenance Period: Until final acceptance at end of warranty period.
MAINTENANCE:
Replant damaged grass areas showing root growth failure, deterioration,bare or thin spots,and eroded
areas.
RESTORATION:
Restore pavement, concrete, grassed areas,planted areas, and structures damaged during execution of
work of this Section.
ACCEPTANCE:
Seeded areas will be accepted at end of maintenance period when seeded areas are properly established
and otherwise acceptable.

SECTION 02227 ' WASTE MATERIAL DISPOSAL
02227 .1 DESCRIPTION
The work covered by this section consists of the disposal of
waste and debris in accordance with the requirements of these
specifications . Waste will be considered to be all excavated,
grubbed or removed materials which are not utilized in the
construction of the project .
02227 .2 GENERAL REQUIREMENTS
Waste shall be disposed of in areas that are outside of the
project area and provided by the Contractor, unless otherwise
required by the plans or special provisions or unless disposal
within the project area is permitted by the Engineer.
The Contractor shall maintain the earth surfaces of all waste
areas, both during the work and until the completion of all
seeding and mulching or other erosion control measures
specified, in a manner which will effectively control erosion
and siltation.
The following requirements shall also be applicable to all waste
or disposal areas other than active public waste or disposal
areas :
Rock waste shall be shaped to contours which are
comparable to and blend in with the adjacent topography where
practical, and shall be covered with a minimum six (6) inch
thick layer of earth material either from project waste or from
borrow.
Earth waste shall be shaped to contours which are
comparable to and blend in with the adjacent topography where
practicable, but in no case will slopes steeper than 2 : 1 be
permitted.
Construction debris, grubbed debris and all broken
pavement and masonry shall be covered with a minimum six (6)
inch thick layer of earth waste material from the project or
borrow. The completed waste area shall be shaped as required
above for disposal of earth waste.
Seeding and mulching shall be performed over all earth or
earth covered waste areas . The work of seeding and mulching
shall be performed in accordance with Section 02820 .
Where 'the Engineer has granted permission to dispose of
waste and debris within the project area, the. Engineer' s
approval of said disposal will be necessary to insure the
satisfactory appearance of the completed project.
Disposal of waste or debris in active public waste or disposal
areas. will not be permitted without prior approval by the
Engineer. Such 'disposal will not be permitted when, in the
opinion of the Engineer, 'it will result in excessive siltation
or pollution.
02227 .3 COMPENSATION
No direct payment will be made for the work .covered by this
section. Payment at the contract prices for the various items in
the contract will be full compensation for all work covered. by
this section including but not limited to furnishing any waste
areas; any right of access to waste areas; disposing of waste
and debris; dressing and shaping of waste areas; furnishing and
spreading earth material over debris, rock, broken pavement, and
masonry; clearing and grubbing of waste areas; and hauling waste
and debris to waste areas.
SECTION 02828 RESTORATION OF SURFACES
02828 .1 SCOPE
This section covers the furnishing of all labor, equipment and
materials necessary for the proper restoration of existing
surfaces disturbed or damaged as a result of construction
operations which are not specifically scheduled or specified for
topsoil and seeding, paving, landscaping or other surfacing.
02828.2 GENERAL
In general, the types of replacement included in this section
are seeding along pipelines, concrete sidewalks, driveways,
roadways, ditches, lawns and landscaped areas, curb and gutter.
Any damage to existing structures shall be repaired using
materials and workmanship equal to those of original
construction.
02828.3 SEEDING ALONG PIPELINES
All ground surfaces along pipelines, which are not classified as
lawns, landscaped areas, or pavement areas, but would be
classified as open fields, shall be raked smooth and seeded in
accordance with the sections entitled Landscaping. Large rocks,
clumps of earth and excessive spoil material shall be removed
from the area prior to seeding.
Shoulders of all roads shall be restored as specific for lawns
and landscaped areas.
Wooded areas, not classified as lawns shall be restored to as
near their original condition as possible.
02828 .4 CONCRETE SIDEWALKS
Concrete walks removed in connection with, or damaged as a
result of, construction operations under the Contract shall be
replaced with new construction. Such walks shall be constructed
of Class B concrete on a thoroughly compacted sub-grade and
shall have a vertical thickness of not less than 4 inches or the
thickness of the replaced walk where greater than 4 inches.
Concrete sidewalks which cross driveways shall have a minimum
vertical thickness of 6 inches. Such walks shall be constructed
of Class B concrete on a thoroughly compacted subgrade. The
subgrade for the walkway shall be the same as required in
Section. 02830.5 .
Walks shall be float finished, edged with an edging tool, and
grooved at intermediate intervals not in excess of the width of
the walk, uniform throughout the length of the walk in any one
direction.
Walks shall be provided with expansion joints spaced at an
interval not to exceed 40 feet or as directed by the Engineer.
02828.5 DRIVEWAYS
Unpaved driveways shall be surfaced with not less than 3 inches
of Crusher-run gravel, topped with 3 inches of stone, gravel; or '
other materials equal to that found in the original driveway.
Driveways shall be left in a condition better than their
original condition.
Concrete drives shall be replaced with Class B concrete and'=
shall have equal thickness and reinforcing steel to that of the
original drive or a thickness of 6 inches, whichever is greater.
Prior to placing the concrete a 6-inch aggregate base course
shall be placed in the drive area.
Bituminous or Asphaltic concrete drives shall be restored with a
4 inch aggregate base course and a 1 .5 inch surface course, as
defined in the section entitled Bituminous Pavement Repairs. '
02828 .6 ROADWAY REPLACEMENT
Bituminous or Asphaltic pavements shall include all areas paved
with blacktop; built-up pavements or oil and stone, tar and
stone and similar pavements constructed with a bituminous . or
asphalt and stone materials.
Immediately upon completion of refilling of the trench or
excavation in a specified area, the Contractor shall repair all
damage to the existing pavement. In the excavated area, the
repair shall consist of a 6-inch aggregate base course and a 2-
inch surface course as defined in the section entitled
Bituminous Pavement Repairs. If, in the 'opinion of the Engineer,
the area adjacent to the excavation has not been damaged to the
extent that the base course need to be replaced, restoration may
consist of a surface course of sufficient thickness to meet the
existing pavement .
Portland cement concrete roadways shall be replaced with Class B
Concrete and shall have equal thickness and reinforcing steel as
the original roadway. An aggregate of 6 inches shall be placed
prior to the placing of concrete.
Differential settlement of restored pavements shall be corrected
immediately.
02828.7 DITCHES
Ditches shall be regraded to the original grade and line. The
surface of all ditches shall be returned to the same condition
as found before commencing work.
02828. 8 LAWNS AND LANDSCAPED AREAS
Lawns and landscaped areas shall be regraded and replaced as
follows :
A. Grading shall be to the grade existing before construction
of the work under this Contract.
B. Lawn replacement shall be in accordance with the section
entitled Landscaping. Top soiled areas shall be replaced
with topsoil of equal quality and quantity.
C. Landscaped areas shall be replaced with shrubs, hedges,
ornamental trees, flowers, or other items to original
condition'.
02828 .9 CURB AND GUTTER
Curb and gutter removed with, or damaged as a result of
construction operations, injured or disturbed by the Contractor,
his agents, or employees, shall be replaced with new
construction to a condition similar and equal to that existing
before damage was incurred. Class B Concrete shall be used in
curb and gutter replacement.
02828 . 10 DAMAGE TO STRUCTURES
Any damage to existing structures shall be repaired by using
materials and workmanship equal to those of original
Construction. Extensively damaged structures, where the
Structural stability has been affected or which cannot be
repaired in a suitable fashion shall be replaced entirely.
Replacement shall not commence until approval of the plan of
replacement has been given by the Engineer. Replacement costs
shall be responsibility of the Contractor.
02828. 11 COMPENSATION
All work covered by this section .will be considered incidental
to other payment items to which it relates and no separate
payment will be made therefore, unless otherwise noted in the
bid documents .
SECTION 03303 BACKFILL
03303 .1 SCOPE
The work of this section consists of furnishing, loading,
placing and compacting of borrow materials necessary in order to
adequately replace undercut excavation material .
03303 .2 GENERAL
Select Backfill material to be used shall be Class II select
backfill. The Contractor, at his option, may substitute Class I
select backfill material for Class II; however, no additional
compensation shall be made as a result. of this substitution.
Select Backfill material shall be placed in sufficient lifts so
as to allow adequate compaction of material. Depth. ,of select
backfill material shall be such that the top of the backfill
material shall be at the proper depth to serve as sub-grade for
the pavement structure or, where applicable, as directed by the
Engineer or Owner.
Select Backfill material shall be placed only.at those locations
shown on the plans and/or. those places where the- material being
excavated is considered unsuitable for backfilling due to poor
site conditions. Contractor will be required to notify the Owner
of the existing conditions and secure the Owner' s authorization
to place the select backfill material:•-•
03303 .3 MATERIALS
Class I Select Backfill material shall be a soil material or a
coarse aggregate.
Soil shall meet the requirements of AASHTO M145 for soil
classifications A-1 or A-1-b.
Coarse aggregate shall meet the following gradation
requirements.

Class II select backfill material shall be a soil material which
meets the requirements of AASHTO M145 for soil classifications
A-2-4 or A-3 .
The Contractor may at his option, substitute Class I select
backfill material for Class II for select backfill material . No
additional compensation will be made to the Contractor as a
result of such substitution.
03303 .4 BACKFILL AND COMPACTION
Unless otherwise specified or .indicated on the Drawings, use
suitable material for backfill, which was removed in the course
of making construction excavations . Do not use frozen material
for the backfill and do not place backfill on frozen material .
Remove previously frozen material before backfill is placed.
The nature of the materials will govern both their acceptability
for backfill .and the methods best suited for their placement and
compaction. Both are subject to the approval of the Engineer. '
and/or Owner. Do not place rock or stone fragments larger than 4
inches in greatest dimension in the backfill . Do not drop large
masses of backfill material into the trench in such a manner as
to endanger the pipeline.
Backfill trenches immediately after approval of the pipeline
construction.
Pipes: Use suitable backfill carefully placed in uniform layers
not exceeding 6 inches in thickness to a depth of two (2) feet
over the top of the pipe. Place material and fill the area under
the pipe haunches. Place each layer, moisten; then uniformly
compact by use of hand, pneumatic, or mechanical tampers
exercising care to prevent lateral displacement. Areas of
backfill two (2) feet over top of pipe to top of trench shall be
backfilled with a suitable material containing no rocks larger
than 6 inches in greatest dimension and shall be free of
material with an exceptionally high void content . The initial
backfill shall meet the same requirements except no rocks over 4
inches in diameter will be allowed.
Moisten backfill above two (2) feet over the top of the pipe and
place in 8" layers. Compact each layer with hand, pneumatic or
mechanical compactor. Puddling or flooding of trench for
consolidation of backfill or use of wheel rolling by
construction equipment will not be permitted.
Roadways and Road Crossing; Use suitable backfill, material
placed in uniform layers not exceeding 6 inches in thickness for
full trench depth , and width,, thoroughly compacted with
mechanical tampers under optimum moisture conditions to 95%
compaction (100% for the top two (2) feet of sub-grade beneath
pavements) . Replace moved paving and base course with new
materials of equal or better quality and of the same texture and
color -as the adjacent roadway.
Cleanup: Grade all areas disturbed to a finish ordinarily
obtained from a blade grader with no abrupt changes in grade or
irregularities that will hold water. Prior to final inspection
and acceptance, remove all rubbish and excess material and leave
area a neat satisfactory condition.
Select backfill material shall be placed to the top of flexible
pipe, in accordance to the guidelines previously described, in
areas where the pipe will be in groundwater and deeper than 6
inches deep.
03303 .5 MEASUREMENT AND PAYMENT
Payment shall be for the number of cubic yards delivered and
incorporated into the project at locations called. for on the
plans or approved by the Engineer or Owner. Payment shall be
based on the unit price bid per cubic yard of backfill material .

SECTION 03307 EXTRA PIPE BEDDING
03307.1 SCOPE
The work covered by this section consists of the furnishing and
installing of additional pipe bedding material over and above
that called for on the plans. This additional pipe bedding
material shall be installed only when designated -by the
Engineer/or Owner. And at the locations and depths designated by
the Engineer or Owner.
03307 .2 MATERIAL
The materials for this Bid Item shall be crushed stone or
crushed gravel meeting the requirements of ASTM C033 Gradation
67 .
03307 .3 INSTALLATION .
The installation of the additional bedding material shall be in
a manner designated by the Engineer/or Owner.
The pipe foundation shall be prepared in accordance with -the
applicable method shown on the plans and shall be true to line
and grade and uniformly firm.
Where necessary, the invert grade shall be cambered by an amount
sufficient to prevent the development of sag or back slope in
the flow line. The Engineer/or Owner will determine the amount
of camber used.
Bedding material shall be placed and shaped beneath the pipe
where so directed by the Engineer/or Owner.
The pipe foundation shall be shaped to fit the outside of the
pipe for at least 10% of its ' outside diameter under all pipe
culverts .
Where bell and spigot type is used recesses shall be excavated
to receive the pipe bells.
Where the foundation material is found to be of poor supporting
value or of rock, the Engineer/or Owner may make minor
adjustments in the location of the pipe to provide a more
suitable foundation. Where this is not practical, the foundation
shall be conditioned by removing the existing foundation
material by undercutting to the depth as directed by the
Engineer/or Owner, within the limits established on the plans,
and with backfilling with either a suitable material secured for
unclassified excavation or borrow excavation at the nearest
accessible location along the project or foundation combination
of sand and crushed stone or gravel approved by the Engineer/or
Owner as being suitable for the purpose intended. The selection
of the backfill of the type of backfill material to be used for
the foundation conditioning will be made by the Engineer/or
Owner.
When necessary the contractor shall provide for the temporary
diversion of water in order to maintain the pipe foundation in a
dry condition.
03307.4 MEASUREMENT AND PAYMENT
The quantity of bedding material to be paid for will be actual
number of tons of material, weighed in trucks on certified
platform scales or other certified weighing devices, which has
been used for shaping the pipe foundation in accordance with
section 03307 .3 .
SECTION 03400 TRENCH EXCAVATION AND BACKFILL COMPACTION
03400 .1 DESCRIPTION
The work covered by this section consists of the excavation and
satisfactory disposal of ' all materials excavated in the
construction of trenches.
Trenches will be defined as all excavation for the installation
of storm sewers, sanitary sewers, waterlines, manholes, catch
basins, hydrants, gate valves; sewer services, water taps,
drainage structures, drainage ditches and other unclassified
excavation as may be deemed necessary by the Engineer.
03400.2 GENERAL
The excavation shall be done to the ' lines, grades, typical
sections, and details shown on the plans or established by the
Engineer. All work covered by this section shall be coordinated
with the grading, construction of drainage structures, and other
work along the project, and shall be maintained in a
satisfactory condition so that adequate drainage is provided at
all times. Any roots, which protrude into the trench, shall be
trimmed flush with the sides of the trench. Trenches for the
pipeline shall be completed before the pipe is installed unless
otherwise permitted by the Engineer/or owner.
All excavation shall be by open cut unless otherwise authorized
by the Engineer. If the bottom of the excavation is found to
consist of rock or any materials that cannot be excavated to
give a uniform bearing surface, the material shall be removed at
a depth of at least 6 inches below established bottom grade and
backfilled to bottom grade with sand and thoroughly compacted in
place. Any excavations carried below the depths indicated,
without specific directions, shall be backfilled in the same
manner. The excavation shall be of sufficient width to allow a
clearance of not less than 6 inches between the side of the
trench and the outside of the pipe, or in case of pipe with a
bell, the outside of the bell of the pipe. This rule will apply
at all times, and consequently, proper allowance must be made
for additional space required for sheeting the trench where
necessary. Maximum trench width, unless as otherwise authorized
by the Engineer, as measured at a depth of 2 feet above the top
of the pipe shall be 30 inches or 10 inches clearance from the
outside of the pipe, whichever is greater.
SHEETING AND BRACING TRENCHES: If necessary, the Contractor,
will be required to keep the sides of the excavation vertical by
sheeting and/or bracing and to prevent movement by slides or
settling of the sides of the trench, in such a manner as . the
Engineer/or Owner may direct to prevent injury or displacement ,
of the pipe or appurtenances or diminish the working space
required at the side of the pipe. Also the Contractor may be
required as directed by the Engineer to leave sheeting and
bracing in place for the purpose of preventing injury to persons
or property or adjacent structures in place or to be
constructed.
No sheeting or bracing shall extend closer than 2 feet from the
ground surface, or within sub-grade, and no timbers shall be
left in the trench that may form pockets or cavities that cannot
easily be filled during the operation of backfilling . and
compacting the trench backfill . It is understood that the Owner
will be under no obligation to pay for sheeting. or bracing left
in place by the Contractor. Failure to sheet and brace trenches
or other' excivations shall be the Contractor's risk, and he will
be held responsible for caving, settlement, and all other. damage
resulting there from, If the Engineer is of the opinion, that at
any point, sufficient or proper supports have not been provided,
he may order additional supports put in at the Contractor' s
expense, but compliance with such - orders shall not , release
Contractor from responsibility for the sufficiency of, such
supports.
03400 .3 EXCAVATED MATERIALS
Excavated materials to be used for backfill will be approved. by
the Engineer, and if acceptable shall be neatly deposited at the
sides of the trenches where space is available. Where
stockpiling of excavated material is required, the Contractor.
shall so maintain his operations as to provide for natural
drainage and not. present an unsightly appearance.
Materials, which are in excess to the needs of the project, will
be disposed of according- to section 02227 .
03400 .4 BACKFILL AND COMPACTION
Backfill trenches immediately after approval of the pipeline
construction according to section 3303 .4 .
03400 . 5 MEASUREMENT AND PAYMENT
All work covered by this section will be considered incidental
to other payment items to which it relates and no separate
payment will be made therefore.
SECTION 07110 SEWER PIPE (GRAVITY) AND APPURTENANCE MATERIAL
07110 .1 SCOPE
These specifications shall apply to the materials to be
furnished and installed to complete the sanitary sewer line
installations in accordance with the plans. All pipe and
appurtenances shall be of the class and type as indicated on the
plans and designated herein.
07110.2 GENERAL
All materials shall be first quality with smooth interior and
exterior surfaces, free from cracks, blisters, honeycombs and
other imperfections, and true to theoretical shapes and forms
throughout. All materials shall be subject to the inspection of
the Engineer at the plant, trench, or other point of delivery,
for the purpose of culling and rejecting materials, which does
not conform to the requirements of these specifications. The
Engineer shall mark such material and the Contractor shall
remove it from the project site upon notice being received of
its rejection.
As. particular specifications are cited, the designation shall be
construed to refer to the latest revision under the same
specification number, or to superseding specifications under a
new number except provisions in revised specifications, which
are clearly inapplicable.
Polyvinyl Chloride Pipe
Polyvinyl Chloride (PVC) pipe shall meet the requirement of ASTM
D 3034, latest edition, and be suitable for use as a gravity
sewer conduit. The standard dimension ratio (SDR) shall be
thirty-five (35) unless otherwise specified. The pipe shall be
supplied in standard laying lengths of twenty (20) feet and
twelve and one-half (12 . 5) feet.
All PVC pipe joints shall be of an integral bell and spigot of
the same material as the pipe with a solid cross-section rubber
"O" ring securely locked in place at the point of manufacture.
Service saddles and other fittings shall be -supplied by the pipe
manufacturer and shall be of the same material and type of
construction as the pipe material.
Ductile Iron Pipe
Ductile. Iron Pipe shall be manufactured in accordance with ANSI
Specification A 21 .51. All Ductile Iron Pipe shall be Class 50
unless otherwise specified and shall be lined with cement mortar
not less than one sixteenth (1/16) inch thick conforming to ANSI
Specification A 21.4 .
Slip or "push-on" joints shall be manufactured in accordance
with ANSI Specification A 21 .11 . Bells of "slip" joint pipe
shall be contoured to receive a bulb shaped, circular rubber
gasket, and plain ends shall have a slight taper to facilitate
installation. The pipe manufacturer shall furnish the lubricant
.used in making up the. joints. The jointing shall be done by
guiding the plain end into the bell until contact .is made with
the gasket and by exerting a sufficient compressive force to
drive the joint home until plain end makes full contact with the
base of the bell.
Steel Casing Pipe
Steel casing, shall conform to ASTM A-53 , Grade B or ASTM A-139
Grade B standard specifications. Casing pipe shall have a wall
thickness as follows, unless otherwise noted in the bid
documents,:

07110 .3 MANHOLES
Precast ,concrete manhole bases, risers and cones shall conform
to ASTM C-478, latest revision for Precast Reinforced Concrete
Manhole sections . Tapered sections and transition sections,
where required, shall be of eccentric cone design, having the
same wall thickness and reinforcement as the cylindrical ring
sections . Flat slab tops shall be required for very shallow
manholes and where shown or specified. Cast iron manhole covers
and assemblies shall be cast into slab tops for access into
manholes.
Minimum compressive strength of concrete shall be 4, 000 psi
and the maximum permissible absorption shall be six and one-half
(6 . 5) percent. Risers shall be reinforced with a single cage of
steel placed within the center one-third (1/3) of the wall . The
tongue or the groove of the joint shall contain one (1) line of
circumferential reinforcement equal in area to that in the
barrel of the manhole riser.
The minimum cross-sectional area of steel per linear
foot shall be twelve one-hundredths (0 . 12) square
inches for larger sizes. Precast manhole sections shall
fit together readily and shall have a self-contained
"O" ring rubber gasket conforming to ASTM C-443 .
The quality of materials, the process of manufacture, and the
finished manhole sections shall be subject to inspection and
approval by the Engineer and his inspector. The manhole sections
shall be perpendicular to their longitudinal axis within the
limits listed in ASTM C-478 .
07110.4 MANHOLE FRAMES AND COVERS
Casting for manhole frames and covers shall be tough, even
grained, soft gray iron,. free .from burnt on sand and other
injurious defects and conform to the requirements of ASTM A 48,
latest revision Class 30 .
07110.5 BRICK
Brick for manholes and other structures ' shall conform to
applicable requirements of ASTM Specification C 62, latest
revision, Grade SW.'
07110.6 CONCRETE
Concrete for sewer structures shall consist of a mix of Portland
cement, fine and coarse aggregate and water to produce concrete
with a minimum compressive strength at twenty-eight (28) days of
not less than 3, 000 psi when tested according to ASTM
Designation C-39 or C-42 .

Air Content (by volume) of the mix shall be between four (4) and
eight (8) percent when placed. The consistency of the concrete
shall be such as to allow it to be worked into place without
segregation or excessive laitance. Slump shall be no more than
four (4) inches.
07110 .7 MORTAR
Mortar for masonry in sewer structures shall be 1 :2 cement sand
mix, except that hydrated lime may be substituted for not more
than ten (1096) percent by weight, of the cement.
07110 . 8 FLEXIBLE COUPLINGS
Whenever it becomes necessary to join sewer pipe lines of
dissimilar materials or pipe sizes it shall be required to use a
flexible coupling. The coupling shall be made of virgin poly_.
vinyl chloride (PVC) shall not harden, and shall be impervious
to all known soil conditions. The coupling shall provide a
permanent leak proof seal approved by the Southern Building Code
Congress and manufactured in accordance by Fernco Joint Sealer
Company or an approved equal .
SECTION 07120 SANITARY SEWER (GRAVITY) PIPE INSTALLATION
07120.1 DESCRIPTION OF WORK
The work covered under this section shall consist of
furnishing all
labor, equipment and services for the installation of
gravity sanitary sewer lines as shown on the drawings
and specified .herein and in agreement with the General
Conditions of these contract documents .
07120 .2 HANDLING AND STORING MATERIALS '
The Contractor shall unload pipe so as to avoid deformation or
other injury thereto. Pipe shall not be placed within pipe of a
.larger size and shall not be rolled or dragged over gravel or
rock during handling. The Contractor shall store the pipe on
sills above storm drainage level and deliver for laying after
the trench is excavated. When any joint or section of pipe is
damaged during transporting, unloading, handling, or storing,
the undamaged portions of the joint or section may be used where
partial lengths are needed, or, if 'damaged sufficiently, the. ..
Engineer will reject the joint or section as being unfit for
installation.
If any defective pipe is discovered after installation,
it shall be removed and replaced with sound pipe or
shall be repaired by the Contractor in an approved
manner and at his own expense.
07120 .3 PIPE ALIGNMENT AND GRADE
The layout of gravity sanitary sewer lines and invert elevations
at governing points are as shown on the drawings.
The Contractor shall do all layout work for lines and grades
from that information shown on the drawings or as furnished by
the Engineer.
When a laser beam instrument is used to set line and grade, the
unit must be maintained in good working order, and the
calibration checked daily for both alignment and percent grade.
In the event the required accuracy of alignment and grade is not
adhered to, the Engineer will prohibit the use of laser beams .
07120.4 PREPARATION OF -PIPE FOUNDATION .
The pipe foundation shall be prepared to be uniformly. firm and
shall be true to the lines and grades as shown on the plans .
The preparation of the pipe bedding shall be in accordance with
the typical trench cross - sections as shown on the drawings .
Under no circumstances shall pipe be laid in water, on
rock, or when trench conditions or weather is
unsuitable for such work.
Whenever the nature of the ground will permit, the excavations
at the bottom of the trench shall have the shape and dimensions
of the outside lower third of the circumference of the pipe,'
care being taken to secure a firm bearing support uniformly
throughout the length of the pipe. A''space shall be excavated
under, and around each bell to sufficient depth to relieve it of
any load and to allow ample space for filling and finishing the
joint. The pipe, when thus bedded firmly, shall be on the exact
grade. In case the bed shaped in the bottom of the trench is too
low, the pipe shall be completely removed from position, and
earth of suitable quality shall be placed and. thoroughly tamped
to prepare a new foundation for the pipe. In no case shall the
pipe be brought to grade by blocking up under the barrel or bell.
of same, but a new and uniform support must be provided for the
full length of the pipe. Where rock or boulders are encountered
in the bottom of the trench, the same shall be removed to such
depth that no part of the pipe, when laid to grade, will be
closer to the rock or boulders than six (6) inches . A suitably
tamped and shaped foundation of approved material shall be
placed to bring the bottom of the trench to. proper subgrade over
rock or boulders .
Where the foundation material is found to be of poor supporting:
value, the Engineer may make minor adjustment in the location of
the pipe to provide a more suitable foundation. Where this is
not practical, the foundation shall be conditioned by removing
the existing foundation material by undercutting to the depth as
directed by the Engineer, within the limits established on the
plans, and backfilling with either an approved material secured
from unclassified excavation or borrow excavation at the nearest-
accessible location along the project, or foundation
conditioning material consisting of crushed stone or gravel or a
combination of sand and crushed stone or gravel approved by the
Engineer as being suitable for the purpose intended. The
selection of the type of backfill material to be used for
foundation conditioning will be made by the Engineer.
The Contractor shall remove all water, which may be encountered,
or which may accumulate in the trenches by pumping or bailing
and no pipes shall be laid until the water has been removed from
the trench. Water so removed from the trench must be disposed of
in such a manner as not to cause injury to work completed or in
progress .
07120 .5 PIPE LAYING
Pipe shall be laid• with bell ends facing. in the direction of
pipe laying, unless directed otherwise by the Engineer. In all
cases, pipe is to be installed in strict accordance with the
manufacturer' s recommendations and the contract material
specifications . - The Engineer may augment any manufacturer' s
installation recommendations if, in his opinion, it will best
serve the interest of the Owner.
Proper tools, implements, and facilities satisfactory to the
Engineer shall be provided and used for the safe and convenient
prosecution of pipe laying. All pipe and other materials used in
the laying of pipe will be lowered into the trench piece by
piece by means of suitable equipment in such a manner to prevent
damage to the pipe, materials, to the protective coating_ on the
pipe materials, and to provide a safe working condition to all
personnel in the trench. Each piece of pipe being lowered into
the trench shall be clean, sound and free from defects. It shall
be laid on the prepared foundation, as specified elsewhere to
produce a straight line on a uniform grade, each pipe being laid
so as to form a smooth and straight inside flow line. Pipe shall
be removed at any time if broken, injured or displaced in the
process of laying same., or of backfilling the trench.
When cutting short lengths of pipe, a pipe cutter as approved by
the Engineer will be used and care will be taken to make the cut
at right angles to the centerline of the pipe or on the exact
skew as shown on -the plans. In the case of push-on pipe, the cut
ends shall be tapered with a portable grinder, or coarse file to
match the manufactured taper.
During times when pipe laying is not in progress, the open ends
of pipe shall be closed and no trench water or other material
shall be permitted to enter the pipe.
Where the pipe is laid on a grade of ten (10) percent or
greater, the laying shall start at the bottom of the slope and
proceed upward with the bell end of the new pipe upgrade. All
pipe laid on a grade of ten (10) percent or greater shall
require thrust blocking and keying as shown on the drawings and
standard details.
Where pipelines of different materials are joined together a
standard sewer repair coupling shall be used. The coupling's
shall be Eastern Standard Sewer Repair Couplings (Mission Rubber
Company) , the Fernco Joint Sealer Company or an equal product
approved by the Engineer.
07120 . 6 BACKFILLING
All backfill shall be compacted so as not to damage the pipe and
appurtenances and shall be compacted to ninety-five (95) percent
of the Standard Proctor Test for the various types of backfill
materials.
Methods of backfilling shall be in strict accordance with the
pipe manufacturer' s recommendations . The Engineer/owner shall
have approved all backfill materials. Select backfill material
• shall be used when requested by the Engineer. Select material
shall be defined as a finely graded material free from stones
over one-half (1/) 2 inch in diameter, plastic clays, organic
material, frozen lumps and various debris and shall be approved
by the Engineer prior to its installation.
Care shall be taken during backfill and compaction operations to
maintain alignment. and prevent .damage , to the joints . The
backfill shall be kept free from stones, frozen lumps, chunks of
highly plastic clay, or other objectionable material . All pipe
backfill areas shall be graded and maintained in such a
condition that erosion or saturation will not damage the pipe
bed or backfill.
Heavy equipment shall not be operated over any pipe until it has
been properly backfilled and has a minimum cover as required by
the plans. Where any part of the required cover is above the
proposed finish grade, the Contractor shall place, maintain, and
finally remove such material at no cost to the Owner. Pipe which
becomes mis-aligned, shows 'excessive settlement, or has been
otherwise damaged by the Contractor' s operations shall be.
removed and replaced by the Contractor at no cost . to the Owner.
The Contractor shall maintain all pipes installed in a condition
that will function continuously from the time the pipe is
installed until the project is accepted.
07120.7 MEASUREMENT AND PAYMENT
The Engineer shall verify quantities of pipe installed in
accordance with the requirements of this section in the field.
Payment shall be made for total number of lineal feet of pipe
installed at the unit prices bid in each depth category. Price
and payment shall constitute full compensation for furnishing
all equipment, tools, labor, and materials to complete the work
as specified.
SECTION 07290 SANITARY SEWER MANHOLE MATERIAL, INSTALLATION & TESTING
07290 . 1 DESCRIPTION OF WORK
The work covered under this section shall consist of furnishing
all materials, labor, equipment, and services for the complete
installation of sanitary sewer manholes .
07290.2 MANHOLE CONSTRUCTION
Sanitary sewer manholes shall be installed at each break in
line or grade in each sanitary sewer line as shown on the
contract drawings.
The manholes shall consist of precast reinforced concrete
sections including a base section and conical top .section.
These precast concrete sections shall conform to ASTM C-478,
latest revision.
The minimum inside diameter of the manhole shall be five' (5)
feet unless noted otherwise on the drawings. Flat slab tops may
be used in non-traffic areas, for very shallow manholes, or
where shown on the drawings or approved by the Engineer.
Makimum allowable absorption of concrete shall be eight (8) per
cent . Minimum compressive strength of concrete for all sections
shall be 4000 psi. Each precast concrete section shall be
joined together with a 11011 ring rubber gasket conforming to
ASTM C-443 or an approved sealant conforming to Federal
Specification 55500210 .
Manhole inverts shall be constructed of concrete or concrete
and brick of semicircular section conforming to the inside
diameter of the outlet sewer. Changes in size of pipe or grade
shall be made gradually and change in direction constructed by
using true curves. Each manhole shall be provided with such
channels for all connecting sewer pipes .
07290 .3 MANHOLE INSTALLATION
The manhole foundation shall be prepared so as to provide a
firm, level area on which to place the precast concrete manhole
base section. When. poor foundation soil is encountered or
excess groundwater exists the foundation shall be excavated
twelve (12) inches below the final subgrade elevation
backfilled with washed stone to provide a proper foundation.
The manhole sections shall be lifted from the side of the
excavation to the bottom of the trench with equipment and
support slings capable of safely handling the heavy concrete
pieces. The manhole shall be set plumb and adjusted to the
final finished surface grade with brick and mortar.
Pipe openings shall be exactly aligned to that of the pipe
entering and leaving the manhole. The gravity sanitary sewer
pipelines shall be placed in the manhole openings, properly
aligned, and set to grade. Sanitary sewer shall be connected to
the manholes using lock joint flexible manhole sleeves or
equal . For large diameter pipe where a flexible rubber sleeve
is not available the pipeline shall be sealed into the manhole
using an expanding type or non-shrink type grout.
Manhole steps shall be properly spaced as shown in the standard
detail drawing, but in no case more than fifteen (15) inches on
. . center. The steps shall be made of heavy cast iron (10 inch
thick) , reinforced plastic (ASTM. 2146-Type II). , or aluminum
(min. load 1000 lbs. ) .
07290 .4 MANHOLE FRAMES AND COVERS
Frames and covers shall be cast iron of superior quality, tough
and even texture. Castings shall be gray iron conforming to
ASTM A-48, size as indicated, free from blow holes, porosity,
hard spots, shrinkage distortion, or other..defects, well
cleaned and coated with asphalt paint . This paint shall result .
in a smooth coating, tough and tenacious when cold, not tacky
and not brittle. The bearing surface between frame and cover
shall be machined to prevent rocking and rattling.
The standard manhole casting shall be designed for heavy-duty
use with a 210-pound frame and 130 pound cover. Acceptable
products include Sumter Machinery Company (#MC-18 & MF-11) ,
Dewey Brothers Foundry (#MHRCR-2001) or an approved equal .
Special waterproof manhole frame and covers shall be installed
only at those locations indicated on the contract drawings.
Acceptable products include Neenah Foundry, Sumter Machinery
Co. , Dewey Brothers Foundry, or an approved equal .
The frame and cover shall be properly set in a bed of mortar
and aligned to fit the top section of the manhole. Concrete
brick, set in mortar, shall be used to adjust the top of the
frame and cover to finished grade; however, no more than four
(4) courses of brick will be used for adjustment .
07290. 5 MANHOLE INVERTS
Manhole inverts and benches shall be constructed in accordance
with the"standard details shown on the drawings . Invert shall
be a U-shaped 'channel with a height of eight-tenths (0 . 8) of
the diameter and be a smooth continuation of the pipe. The
benches shall be constructed with a slope of one (1) inch per
foot to the channel. The channel and invert shall be
constructed with a minimum of 2000 psi concrete or brick fill
with concrete finish minimum one (1) inch thick. Where sewer
changes directions at the manhole, channel shall be constructed
with a smooth curve with as large a radius as the diameter of
the manhole will allow.
07290 . 6 MANHOLE DROPS
Standard drop manholes will be constructed only at those
locations shown on the drawings or as approved by the engineer.
The design of the drop connection shall be in accordance with
the standard detail drawing.
The cost of the extra pipe, labor, etc. required to construct a
drop manhole will be included in the unit price for the drop
manhole at the depths listed.
07290 .7 INSPECTION AND ACCEPTANCE
Manholes shall be constructed to provide a true circular inside
diameter with properly corbelled tops, satisfactory inverts and
properly placed steps and castings . Any visible leaks in the
manholes shall be completely stopped to the satisfaction of the
Engineer.
07290 . 8 MANHOLE VENTS
Where designated on the contract drawings, a four (4) inch
diameter vent pipe shall be installed as an integral part of
the manhole. The vent pipe is to be tapped in to the upper most
section of the manhole, anchored in concrete and extended
vertically to the elevation shown on the drawings . The pipe
shall have a reverse bend and screen to prohibit rain and
foreign materials from entering pipe.
The pipe material shall be Schedule 40 Galvanized Steel with
two coats of epoxy paint approved by the Engineer.
07290 .9 MEASUREMENT AND PAYMENT
The Engineer shall verify quantities of manholes installed in
accordance with the requirements of this section in the field.
Payment shall be made for Standard Manholes or Standard Drop
Manholes at the prescribed depth measured from the top of the
frame to the low pipe invert at the respective unit price bid.
Payments for watertight manhole frame and covers and manhole
vents will be considered to be included in the unit price bids
for the respective manholes. Payment shall constitute full
compensation for furnishing all equipment, tools, labor and.
incidentals and performance of all work necessary to install
the manholes as specified.
SECTION 07295 SANITARY SEWER SERVICE CONNECTIONS
07295 . 1 DESCRIPTION OF WORK
The work covered under this section shall consist of furnishing
all materials, labor, equipment, and services for the complete
installation of a sanitary sewer service connection from the
sanitary sewer (gravity) main line to the edge of the property
to be served as shown on the project drawings .
07295 .2 CONNECTION TO MAIN
The standard sewer service connection shall be four (4) inches
in diameter unless shown otherwise on the drawings, and shall
connect to the main at a "wye" branch connection installed with
the pipe line as it is being laid. The "wye" branch shall be of
the same material as the main pipe line. Direct taps into the
sewer main will not be acceptable unless approved by the. .,, .•
Engineer prior to the laying of the main line.
07295.3 CONNECTION TO MANHOLE
When shown on the drawings or directed by the Engineer, a sewer
service connection shall be made into a manhole. The connection
shall be made at the manhole invert. The invert shall be rebuilt
so as to provide a smooth transition from service connection to
main line. Inverts and benches shall be constructed in -such a
way as to prevent the deposition of solids in the manhole.
07295.4 SERVICE LINE
The service line shall be installed from the "wye" branch
connection to the edge of the public or utility right-of-way as
shown on the plans.
The sewer service line will be constructed of either PVC-pipe
(PVC schedule 40) pipe, or Ductile Iron (CL 350) as shown on the
plans . When the service line pipe material is different from
that of the main line a flexible, water tight, rubber transition
coupling shall be used to join the service line to the "wye"
connection. The service line shall be laid at a minimum grade of
one (1 . 0) percent with a minimum cover of three ,(3 . 0) feet. Less
than three (3) feet of cover will require the use of ductile
iron pipe.
07295 .5 CLEANOUT AND PLUG
At the edge of the public or utility right-of-way a "cleanout"
shall be installed. The cleanout shall consist of a "wye" branch
connection, forty-five (45) degree bend, riser pipe, and
threaded plug installed flush with finished ground elevation.
The end of the utility owner' s sewer service connection shall
terminate at the end of the pipe which will normally extend five
(5) feet beyond the "wye" branch for the cleanout. A watertight
plug shall be installed at the .end of this line until such time
as the property owner directs the utility owner to connect his
facilities to the sewer system.
07295 . 6 MEASUREMENT AND PAYMENT'
Payment for the furnishing and installation of all service
connections ("wye" branches, service line, clean outs, plugs,
couplings, etc. ) as described in this section and as shown on
the plans, shall be covered under the contract unit price bid
for each Sewer Service Connection.

part of this permit and is therefore incorporated therein by reference.
Please pay particular attention to condition three (3) of the permit which requires that.the
wastewater collection facilities be properly operated and maintained in accordance with 15A NCAC 2H .
.0227 or any individual system-wide collection system permit issued to the Permittee.
Permitting of this project does not constitute acceptance of any part of the project that does
not meet 15A NCAC 2H .0200; the Division of Water Quality's (Division) Gravity Sewer Minimum
Design Criteria adopted February 12, 1996 as applicable; and the Divisionos Minimum Design
Criteria for the Fast-Track Permitting of Pump Stations and Force Mains adopted June 1, 2000 as
applicable, unless specifically mentioned herein. Division approval is based on acceptance of the
certification provided by North Carolina-licensed Professional Engineer in the application. It shall
be the Permittee's responsibility to ensure that the as-constructed project meets the appropriate
design criteria and rules. Failure to comply may result in penalties in accordance with North Carolina
General Statute 143-215.6A through 143-215.6C, construction of additional or replacement
wastewater collection facilities, and/or referral of the North Carolina-Licensed Professional Engineer
to the licensing board.

Carolina as amended, and other applicable Laws, Rules, and Regulations, permission is hereby
granted to the City of Hendersonville for the construction and operation of approximately 1,500 linear
feet of eight (8) inch gravity sewer to serve Mountain Manor Homes of Dunroy on Rutledge as part
of the City of Hendersonville's wastewater collection system and the discharge of 9,360 gallons per
day of collected domestic wastewater into the City of Hendersonville's existing sewerage system,
pursuant to the application received on December 11, 2001 and in conformity with 15A NCAC 2H
.0200; the Division's Gravity Minimum Design Criteria adopted February 12, 1996 as applicable; the
Division's Minimum Design Criteria for the Fast-Track Permitting of Pump Stations and Force Mains
adopted June 1, 2000 as applicable; and other supporting data subsequently filed and approved by
the Department of Environment and Natural Resources and considered a part of this permit.
The sewage and wastewater collected by this system shall be treated in the City of
Hendersonville's Wastewater Treatment facility (NPDES Permit No. NC0025534) prior to being
discharged into Mud Creek in the French Broad River Basin.
If any parts, requirements, or limitations contained in this permit are unacceptable, you have
the right to request an adjudicatory hearing upon written request within 30 days following receipt of
this permit. This request must be in the form of a written petition, conforming to Chapter 150E of
North Carolina General Statutes, and filed with the Office of Administrative Hearings, 6714 Mail
Service Center, Raleigh, NC 27699-6714. Unless such demands are made, this permit shall be
final and binding.

WASTEWATER COLLECTION SYSTEM EXTENSION PERMIT
This permit shall be effective from the date of issuance until rescinded and shall be subject
to the following specified conditions and limitations:
1. This permit shall become voidable unless the wastewater collection facilities are constructed
in accordance with the conditions of this permit: 15A NCAC 2H .0200; the Division of Water
Quality's (Division) Gravity Sewer Minimum Design Criteria adopted February 12, 1996 as
applicable; the Division's Minimum Design Criteria for the Fast-Track Permitting of Pump
Stations and Force Mains adopted June 1, 2000 as applicable; and other supporting materials
unless specifically mentioned herein.
2. This permit shall be effective only with respect to the nature and volume of wastes described
in the application and other supporting data.
3. The wastewater collection facilities shall be properly maintained and operated at all times.
The Permittee shall maintain compliance with an individual system-wide collection system
permit for the operation and maintenance of these facilities as required by 15A NCAC 2H
.0227. If an individual permit is not required, the following performance criteria shall be met
as provided in 15A NCAC 2H .0227:
a. The sewer system shall be effectively maintained and operated at all times to prevent
discharge to surface waters, and any contravention of the groundwater standards in
15A NCAC 2L .0200 or the surface water standards in 15A NCAC 2B .0200.
b. A map of the sewer system shall be developed prior to January 1, 2004 and shall
be actively maintained.
C. An operation and maintenance plan shall be developed and implemented.
d. Pump stations that are not connected to a telemetry system shall be inspected daily,
as defined in 15A NCAC 2B .0503(5), until July 1, 2001; and thereafter, pump stations
shall be inspected every day. Pump stations that are connected to a telemetry system
shall be inspected at least once per week.
e. High-priority sewer lines shall be inspected at least once per every six-month period
of time.
f. A general observation of the entire sewer system shall be conducted at least once per
year.
g. Inspection and maintenance records shall be maintained for a period of at least three
years.
h. Overflows and bypasses shall be reported to the appropriate Division Regional Office
in accordance with 15A NCAC 2B .0506(a), and public notice shall be provided as
required by North Carolina General Statute 143-215.1 C.
4. This permit shall not be transferable. In the event there is a desire for the wastewater facilities
to change ownership, or there is a name change of the Permittee, a formal permit request
shall be submitted to the Division accompanied by documentation from the parties involved,
and other supporting materials as may be appropriate. The approval of this request shall be
considered on its merits and may or may not be approved.
5. Construction of the gravity sewers shall be scheduled so as not to interrupt service by the
existing utilities nor result in an overflow or bypass discharge of wastewater to the surface
waters of the State.
6. Upon completion of construction and prior to operation of these permitted facilities, a
certification, a copy of the construction record drawings, as well as supporting design
calculations for any pump stations permitted as part of this project shall be received from a
North Carolina-licensed Professional Engineer certifying that the facilities have been installed
in accordance with this permit; 15A NCAC 2H .0200; the Division's Gravity Sewer Design
Criteria adopted February 12, 1996 as applicable; the Division's Minimum Design Criteria for
the Fast-Track Permitting of Pump Station and Force Main adopted June 1, 2000 as
applicable; and other supporting materials. If this project is to be completed in phases and
partially certified, you shall retain the responsibility to track further construction approved
under the same permit, and shall provide a final certificate of completion once the entire
project has been completed. A copy of the construction record drawings, indicating the
facilities constructed in the phase being certified, shall be submitted with each partial
certification. Mail the Engineer's Certification, one copy of the "Construction Record
Drawings", and one copy of the supporting design calculations to the Non-Discharge
Permitting Unit, 1617 Mail Service Center, Raleigh, NC 27699-1617.
7. A copy of the construction record drawings shall be maintained on file by the Permittee for the
life of the wastewater collection facilities.
8. Failure to abide by the conditions and limitations contained in this permit; 15A NCAC 2H
.0200; the Division's Gravity Sewer Design Criteria adopted February 12, 1996 as applicable;
the Division's Minimum Design Criteria for the Fast-Track Permitting of Pump Station and
Force Mains adopted June 1, 2000 as applicable; and other supporting materials may subject
the Permittee to an enforcement action by the Division, in accordance with North Carolina
General Statutes 143-215.6A through 143-215.6C.
9. In the event that the wastewater collection facilities fail to perform satisfactorily, including the
creation of nuisance conditions, the Permittee shall take immediate corrective action,
including those as may be required by this Division, such as the construction of additional or
replacement facilities.
10. The issuance of this permit shall not exempt the Permittee from complying with any and all
statutes, rules, regulations, or ordinances that may be imposed by other government agencies
(local, state and federal) which have jurisdiction, including but not limited to applicable river
buffer rules in 15A NCAC 2B .0200, erosion and sedimentation control requirements in 15A
NCAC Ch. 4 and under the Division's General Permit NCG0100000, and any requirements
pertaining to wetlands under 15A NCAC 2B .0200 and 1.5A NCAC 2H .0500.
11. Non-compliance Notification:
The Permittee shall report by telephone to the Asheville Regional Office, telephone number
828/251-6208 as soon as possible, but in no case more than 24 hours or on the next working
day, following the occurrence or first knowledge of the occurrence of either of the following:
a. Any process unit failure, due to known or unknown reasons, that renders the facility
incapable of adequate wastewater transport, such as mechanical or electrical failures of
pumps, line blockage or breakage, etc.; or
b. Any failure of a pumping station or sewer line resulting in a by-pass directly to receiving
waters without treatment of all or any portion of the influent to such station or facility.
Persons reporting such occurrences by telephone shall also file a written report in letter form
within five days following first knowledge of the occurrence. This report must outline the actions
taken or proposed to be taken to ensure that the problem does not recur.

A. SCOPE
These specifications shall apply to the materials to be furnished to complete the installation of the sanitary
sewer lines shown on the contract drawings. All pipe and appurtenant materials shall be of the class and type
as indicated on the drawings and as designated herein.
B. GENERAL REQUIREMENTS
All materials shall be first quality with smooth interior and exterior surfaces, free from cracks, blisters,
honeycombs and other imperfections, and true to theoretical shapes and forms throughout. All materials shall
be subject to the inspection of the Engineer at the plant, trench, or other point of delivery, for the purpose of
culling and rejecting materials, which do not conform to the requirements of these specifications. Such
materials shall be marked by the Engineer and the Contractor shall remove it from the project site upon notice
being received of its rejection.
As particular specifications are cited, the designation shall be construed to refer to the latest revision under the
same specification number, or to superseding specifications under a new number except provisions in revised
specifications which are clearly inapplicable.
C. DUCTILE IRON PIPE(DIP)(Standard)
Ductile iron pipe shall be as manufactured in accordance with ASTM A-746,ANSI Specification A21.50 and
A21.51 and shall be Class 50 unless otherwise specified on the drawings.
The pipe interior shall be cement mortar lined and seal coated, standard thickness, in accordance with ANSI
Specification A21.4.
The exterior of all pipe shall be coated with either a coal or asphaltic base bituminous pipe coating in
accordance with ANT SI Specifications A21.8.
Pipe shall be furnished with Slip Joints, Mechanical Joints, or Flanged Joints as indicated on the drawings and
in accordance with the specifications described below:
(a) Slip Joints-This pipe joint shall be done by guiding the plain end of the pipe into the bell
end until contact is made with a gasket and by exerting a sufficient compressive force to
drive the plain end through the gasket until the plain end makes full contact with the base'of
the bell.
Bells of slip joint pipe shall be contoured to receive a circular rubber gasket and plain ends
shall have a slight taper to facilitate installation.
The circular gasket shall be furnished by the pipe manufacturer and shall be manufactured
in accordance with ANSI Specification A21.1 L
The pipe manufacturer shall also fitmish the lubricant used to assist in the pipe installation.
(b) Mechanical Joints - This pipe joint is essentially the same as the slip joint except it is
furnished with a cast 'iron clamp which acts as a retainer to hold the circular rubber gasket in
place. All mechanical type joints shall be furnished by the pipe manufacturer and
manufactured in accordance with ANSI Specification A21.11.
All bolts shall be tightened by means of torque wrenches in such a manner that the follower
shall be brought up toward the pipe evenly. If effective sealing is not obtained by

tightening the bolts to the specified, torques, the joint shall be disassembled and
reassembled after thorough cleaning.
Icl Flanged Joints - The flanged pipe joint is composed of flat steel plate shop fitted on the
[treaded end of the ductile iron pipe. The flanges shall be accurately faced at right angles to
the pipe axis and shall be drilled smooth and true.
Flanged joints shall be famished with 125 lb. flanges drilled in accordance with ANSI
Specification B 16.1.
In general, flangedjoints shall be made up with through bolts of the required size. Stud or
tap bolts shall be used only where shown or required.
Gaskets for flanged joints shall be the ring type of cloth inserted rubber or rubber with a
minimum thickness of 1/8 inch.
Connecting flanges shall be in proper alignment and no external force shall be used to bring
them together. Bolts and gaskets shall be furnished by the installer of piping for joints
connecting the piping with equipment, as well as for those between pipe and fittings,
whether such equipment and piping is furnished by the installer or not.
D. DUCTTi E IRON PIPE(Alumna Lind)
Ductile iron pipe shall be manufactured the same as standard specifications (ASTM A-746, ANSI A21.50 &
21.51)with the following coatings.
a. Outside Coating shall be a minimum of I mil red alkyd paint. The last 6 in. of spigot, including the
spigot face, shall be coated with 8 mils of epoxy to prevent corrosion in the joint. All pipe to be
stamped"NOT FOR POTABLE WATER"
b. Inside Lining shall be a high alumina cement mortar meeting all requirements of ANSI 21.4 and
Griffin Pipe Products Company or equal. The minimum lining thickness shall be 0.281 in. with
tapering permitted at the ends. Alumina cement shall be cement fondu as manufactured by LaFarge
Aluminous Cement Co.,Ltd.,or approved equal.
C. Inside Coating shall be standard seal coat over the high alumna cement lining. The entire inside of
the socket,including the gasket cavity shall be coated with a minimum of 8 mils of epoxy to prevent
corrosion in the joint.
d. Whenever a thickness class of 47 through 49 is stated on the contract drawings, the pipe shall be
subject to the manufacturees standards and the Engineer's written approval.
E. DUCTILE IRON PIPE(EPDXY LINED)
Ductile iron pipe shall be manufactured the same as standard specifications(ASTM A-746, ANSI A21.50 and
21.51)with the following coatings:
a. Outside Coating shall be a minimum of I mil red alkyd paint. The last 6 in. of spigot, including the
spigot face, shall be coated with 8 mils of epoxy to prevent corrosion in the joint. All pipe to be
stamped'NOT FOR POTABLE WATER."
b. Inside lining shall be a Koppers Coal Tar Epoxy 300 M applied in three (3) passes 8 mil dry
thickness to a total thickness of 24 mils. The lining shall cover the entire inside surface area of the
pipe socket with a minimum 8 mil dry thickness.

Polyvinyl chloride pipe shall be as manufactured in accordance with ASUM D-3034, latest edition, and shall
be suitable for use as a gravity sanitary sewer pipe. The standard dimension ratio (SDR) shall be 35 unless
otherwise specified on the contract.
All polyvinyl chloride pipe shall be of an integral bell and spigot of the same material as the pipe. It shall have
a solid cross section,with rubber"O" ring securely locked in place at the point of manufacture.
G. VITRIFIED CLAY PIPE(VCP)
Vitrified clay pipe shall be of the extra strength type as manufactured in accordance with ASTM C-700, latest
edition.
All vitrified clay pipe joints shall be of the integral bell and spigot type with "O"right gaskets provided by the
pipe manufacturer.
H. REINFORCED CONCRETE SEWER PIPE(RCPI
Reinforced concrete sewer pipe shall be furnished in accordance with ASTM C-76, latest edition, and shall be
suitable for use as a gravity sanitary sewer pipe.
The pipe shall be Class III,wall thickness B, with spigot grove joint. The rubber"O"ring gasket shall form a
flexible watertight seal at the assembled pipe joint and be manufactured in accordance with ASTM C-443.
The pipe manufacturer shall furnish to the Engineer certification from an independent laboratory that the
alkalinity of the pipe is a least 90%calcium carbonate equivalent. Certification shall also be furnished by the
pipe manufacturer stating the type of aggregate used in the pipe.
I. FLEMLECOUPERiGS
Whenever it becomes necessary to join sewer pipe lines of dissimilar materials or pipe sizes, it shall be
required to use a flexible coupling. The coupling shall be made of virgin polyvinyl chloride (PVC), shall not
harden, and shall be impervious to all known soil conditions. The coupling shall provide a permanent
leakproof seal approved by the Southern Building Code Congress and manufactured in accordance with
ASTM#C-594-70. The couplings shall be as manufactured by Femco Joint Sealer Co.or an approved equal.
J. COMPENSATION
Quantities of materials famished in accordance with this section shall be considered as included in the
payment for each type item being installed under the contract as described under Section 07260 of these
contract specifications and as shown on the drawings.

A. DESCRIPTION OF W RK
The work covered under this section shall consist of famishing all labor, equipment, and services for the
installation of sanitary sewer piping as shown on the drawings and specified herein and in agreement with the
General Conditions of these contract documents.
B. HANDLRNQ AND STORING MATERIALS
The Contractor shall unload material so as to avoid deformation or other injury thereto. Material shall not be
rolled or dragged over gravel or rock during handling. The Contractor shall store the appurtenances on sills
above storm drainage level and deliver installations after the trench is excavated. When any material is
damaged during transporting, unloading, handling or storing, the undamaged portions may be used or, if
damaged sufficiently, the Engineer will reject the material as being unfit for installation.
If any defective material is discovered after installation, it shall be removed and replaced with sound material
or shall be repaired by the Contractor in an approved manner and at his own expense.
C. PIPE ALIGNMENT AND GRAD
The layout of underground sanitary sewer lines and invert elevations at governing points are as shown on the
drawings.
The Contractor shall do all layout work for lines and grades from the information shown on the drawings or as
furnished by the Engineer.
When a laser beam instrument is used to set line and grade, the unit must be maintained in good working
order, and the calibration checked daily for both alignment and percent grade. In the event the required
accuracy of alignment and grade is not adhered to,the Engineer will prohibit the use of laser beams.
The Engineer shall field verify as-built invert elevations of all manholes as the work progresses
(approximately each 1,000 L.F.of line construction). The as-constructed elevations shall be within a tolerance
of 0.020 ft± of the design elevations or the Contractor may be required to remove and relay the non-
conforming work at his expense. Field survey work by the Engineer shall not relieve the Contractor of his
responsibility for field layout.
D. PREPARA-TION OF PIPE FOUNDATION
The pipe foundation shall be prepared to be uniformly firm and shall be true to the lines and grades as shown
on the plans. The preparation of the pipe bedding shall be in accordance with the typical trench cross sections
as shown on the drawings. Under no circumstances shall pipe be laid in water, on rock, or when trench
conditions or weather is unsuitable for such work.
Whenever the nature of the ground will permit, the excavations at the bottom of the trench shall have the
shape and dimensions of the outside lower third of the circumference of the pipe, care being taken to secure a
firm bearing support uniformly throughout the length of the pipe. A space shall be excavated under and
around each bell to sufficient depth to relieve it of any load and to allow ample space for filling and finishing
the joint. The pipe, when thus bedded firmly, shall be on the exact grade. In case the bed shaped in the
bottom of the trench is too low, the pipe shall be completely removed from position, and earth of suitable
quality shall be placed and thoroughly tamped to prepare a new foundation for the pipe. In no case shall the
pipe be of same, but a new and uniform support must be provided for the full length of the pipe. Where rock

or boulders are encountered in the bottom of the trench, the same shall be removed to such depth that no part
of the pipe, when laid to grade, will be closer to the rock or boulders than six (6) inches. A suitably tamped
and shaped foundation of approved material shall be placed to bring the bottom of the trench to proper
subgrade over rock or boulders.
Where the foundation material is found to be of poor supporting value, the Engineer may make minor
adjustment in the location of the pipe to provide a more suitable foundation. Where this is not practical, the
foundation shall be conditioned by removing the existing foundation material by undercutting to the depth as
directed by the Engineer, within the limits established on the plans, and backfilling with either an approved
material secured from unclassified excavation or borrow excavation at the nearest accessible location along the
project,or foundation conditioning material consisting of crushed stone or gravel or a combination o£sand and
crushed stone or gravel approved by the Engineer as being suitable for the purpose intended. The selection of
the type of backfill material to be used for foundation conditioning will be made by the Engineer.
The Contractor shall remove all water which may be encountered or which may accumulate in the trenches by
pumping or bailing and no pipes shall be laid until the water has been removed from the trench. Water so
removed from the trench must be disposed of in such a manner as not to cause injury to work completed or in
progress.
E. PIPE LAYING
Pipe shall be laid with bell ends facing in the direction of pipe laying, unless directed otherwise by the
Engineer. In all cases, pipe is to be installed in strict accordance with the manufacturers recommendations
and the contract material specifications. The Engineer may augment any manufacturer's installation
recommendations if,in his opinion,it will best serve the interest of the Owner.
Proper tools, implements, and facilities satisfactory to the Engineer shall be provided and used for the safe and
convenient prosecution of pipe laying. All pipe and other materials used in the laying of pipe will be lowered
into the trench piece by piece by means of suitable equipment in such a manner to prevent damage to the pipe,
materials, to the protective coating on the pipe materials, and to provide a safe working condition to all
personnel in the trench. Each piece of pipe being lowered into the trench shall be clean sound and free from
defects. It shall be laid on the prepared foundation as specified elsewhere to produce a straight line on a
uniform grade, each pipe being laid so as to form a smooth and straight inside flow line. Pipe shall be
removed at any time if broken,injured or displaced in the process of laying same,or of backfilling the trench.
When cutting short lengths of pipe, a pipe cutter as approved by the Engineer will be used and care will be
taken to make the cut at right angles to the centerline of the pipe or on the exact skew as shown on the plans.
In the case of push-on pipe, the cut ends shall be tapered with a portable grinder, or coarse file to match the
manufactured taper.
During times when pipe laying is not in progress, the open ends of pipe shall be closed and no trench water or
other material shall be permitted to enter the pipe.
Where the pipe is laid on a grade of 10 percent or greater, the laying shall start at the bottom of the slope and
proceed upward with the bell end of the new pipe upgrade. All pipe laid on a grade of 10 percent or greater
shall require thrust blocking and keying as shown on the drawings and standard details.
Where pipe lines of different materials are joined together,a standard sewer repair coupling shall be used.The
couplings shall be Eastern Standard Sewer Repair Couplings (Mission Rubber Company), the Femco Joint
Sealer Company,or an equal product approved by the Engineer.
F. BACKFILLING

All backtill shall be compacted so as not to damage the pipe and appurtenances and shall be compacted to 95
percent of the Standard Proctor Test for the various types of backtill materials.
Methods of backfilling shall be in strict accordance with the pipe manufacturer's recommendations. All
backtill materials shall have been approved by the Engineer. Select backfrll material shall be used when
requested by the Engineer. Select material shall be defined as a finely graded material free from stones over
1/2-inch in diameter, plastic clays,organic material, frozen lumps and various debris and shall be approved by
the Engineer prior to its installation.
Care shall be taken daring backfrll and compaction operations to maintain alignment and prevent damage to
the joints. The backfrll shall be kept free from stones, frozen lumps, chunks of highly plastic clay, or other
objectionable material. All pipe backfrll areas shall be graded and maintained in such a condition that erosion
or saturation will not damage the
pipe bed or backfill.
Heavy equipment shall not be operated over any pipe until it has been properly backfilled and has a minimum
cover as required by the plans. Where any part of the required cover is above the proposed finish grade, the
Contractor shall place, maintain, and finally remove such material at no cost to the Owner. Pipe which
becomes mis-aligned, shows excessive settlement, or has been otherwise damaged by the Contractoes
operations shall be removed and replaced by the Contractor at no cost to the Owner.
The Contractor shall maintain all pipes installed in a condition that they will function continuously from the
time the pipe is installed until the project is accepted.
O. COMPENSATION
Quantities of pipe installed in accordance with the requirements of this section shall be verified in the field by
the Engineer. Payment shall be made for total number of lineal feet of pipe installed at the unit prices bid in
each depth category. Price and payment shall constitute full compensation for famishing all equipment, tools,
labor,and materials to complete the work as specified.

WASTEWATER COLLECTION SYSTEM SANITARY SEWER(GRAVITY)
INSPECTION AND TESTING
A. DESCRIPTION OF WORK
The work covered in this section shall consist of furnishing all labor, equipment, and services for the
proper inspection and testing of the gravity sanitary sewer pipe lines installed in accordance with Section
07260.
B. LINE CLEANNG
Prior to inspection of any section(s) of gravity sanitary sewer pipe, the Contractor shall completely clean
the lines of all debris, silt, etc. The pipe line shall be ready for use by the Owner and shall be proved to be
in first class condition and constructed properly in accordance with the drawings and specifications.
The Contractor shall maintain the project insofar as his construction work is concerned, in first cla
condition for such ass
acceptable. time as it is necessary to satisfy the Engineer that all installations are correct and
C. INSPECTION AND INFILTRATION
Alignment and grade between manholes shall be tested by the Engineer by flashing a light between
manholes. A practically full circle of light shall be seen when reviewed from the adjoining end of the
line. All defects disclosed as a result of this test shall be corrected by the Contractor at his expense.
PVC pipe shall pass go-no-go mandrel sized to 95 percent of the pipe diameter with the pipe in place and
Properly backfilled. All pipe which will not pass the Mandrel shall be relayed or replaced by the
Contractor at no additional cost. The allowable deflection (less than 5 percent) shall be calculated using
the pipe stiffness formula in ASTM D2321.
When the sewers are completed, they shall be inspected by the Engineer for conformance with the
Provisions of the plans and specifications,particularly line and grade, and tested to determine the amount
of ground water infiltration into the sewer. All visible and audible leaks will be stopped and the
remaining infiltration will be measured using a V-notch weir and/or other devices, which shall be
furnished by the Contractor. The Contractor shall also furnish all required assistance for measuring the
infiltration.
If infiltration into the whole system or any segment thereof exceeds 100 gallons per 24 hours per inch of
pipe diameter per mile of sewer, necessary corrective measures shall be taken by the Contractor to limit
the infiltration to the maximum specified above. The Engineer shall decide the number and length of
segments of sewer line on which the testing shall be performed.
D. AIR TESTING
All gravity sanitary sewer lines may be subjected to a low pressure air test to determine the presence of
damaged pipe or faulty installation. If directed by the Engineer to conduct an air test, the Contractor will
furnish all facilities and personnel for conducting the test(s).
The acceptance air test shall be made after backfilling has been completed and compacted and in the
presence of the Engineer. The test shall be performed as described under ASTNIC-823, latest edition,
Standard Practice for Low Pressure Air Testing of V.C. Pipelines.

Failure of any section of the pipeline to meet the requirements of this test shall cause the Contractor to
determine, at his own expense, the source(s) of leakage, and repair or replace all defective materials or
workmanship. The repaired section(s) of line shall be re-tested to insure conformance with the
requirements of these contract specifications.
E. FINAL ACCEPTANCE
The Engineer will notify the Contractor, in writing, as to satisfactory n of the work in any or all completio
sections of gravity sanitary sewer pipe included in the project.
Upon such notification, the Contractor shall immediately remove all construction equipment, excess
materials, tools, debris, etc., from the site(s) and leave the same in a neat. orderly condition acceptable to
the Engineer.
Final landscaping requirements and restoration of surfaces shall then be completed by the Contractor in
accordance with their respective specifications and as shown on the drawings.
F. COMPENSATION
There will be no specific quantities measured for the Gravity Sanitary Sewer Pipe Inspection and Testing.
The cost of providing all equipment, labor, materials, etc., required to perform the work described under
this section shall be considered incidental to that work covered under other sections of the specifications.

SANITARY SEWER MANHOLE-NIATERIALS,
WASTEWATER COLLECTION SYSTEM
INSTALLATIONY. & TESTING
A. DESCRIPTION OF WORK
The work covered under this section shall consist of furnishing all materials. labor, equipment, and
services for the complete installation of sanitary sewer manholes-
B. MANHOLE CONSTRUCTION
Sanitary sewer manholes shall be installed at each break in line or grade in sanitan ;ewer line as shown
on the contract drawings.
The manholes shall consist of precast reinforced concrete sections including a base section and conical top
section. These precast concrete sections shall conform to ASTM C-178, latest revision.
The minimum inside diameter of the manhole shall be five (5) feet unless noted otherwise on the
drawings. Flat slab tops may be used in non-traffic areas, for very shallow manholes, or where shown on
the drawings or approved by the Engineer.
Maximum allowable absorption of concrete shall be 8.0 percent. Minimum compressive strength of
concrete for sections shall be 4000 psi. Each precast concrete section shall be joined together with an "O"
ring rubber gasket conforming to ASTM C-443 or an approved sealant conforming to Federal
Specifications 55500210.
Manhole inverts shall be constructed of concrete or concrete and brick of semicircular section conforming
to the inside diameter of the outlet sewer. Changes in size of pipe or grade shall be made gradually and
change in direction constructed by using true curves. Each manhole shall be provided with such channels
for all connecting sewer pipes.
C. MANHOLE INSTALLATION
The manhole foundation shall be prepared so as to provide a firm, level area on which to place the precast
concrete manhole base section. When poor foundation soil is encountered or excess groundwater exists,
the foundation shall be excavated 12 inches below the final subgrade elevation backfilled with washed
stone to provide a proper foundation.
The manhole sections shall be lifted from the side of the excavation to the bottom of the trench with
equipment and support slings capable of safely handling the heavy concrete pieces. The manhole shall be
set plumb and the rim and lid adjusted to the final finished surface grade with brick and mortar.
Pipe openings shall be exactly aligned to that of the pipe entering and leaving the manhole. The gravity
santitary sewer pipe lines shall be placed in the manhole openings, properly aligned and set to grade. Pipe
shall be firmly held in place, and opening around the exterior diameter of the pipe and the base opening
shall be filled with an expanding type or non-shrink type grout well rammed into place or with a flexible
pipe connector in accordance with ASTM C-923 to provide a watertight connection.
Manhole steps shall be properly spaced as shown in the standard detail drawing, but in no case more than
15 inches on center. The steps shall be made of heavy cast iron (1 inch thick), reinforced plastic (ASTM
2I46-Type II), or aluminum(min. load 100 lbs.).

D MANHOLE FRA,NfES AND COVERS
Frames and rovers shall be cast iron of superior quality, tough and even te.eture. Castings shall be gray
iron conforming to ASTM A-48, size as indicated, free from blow holes, porosity, hard spots. shrinkage
distortion, or other defects, well cleaned and coated with asphalt paint. This paint shall result in a smooth
coating, tough and tenacious when cold, not tacky and not brittle. The bearing surface between frame and
cover shall be machined to prevent rocking and rattling.
The Standard manhole casting shall be designed for heavy duty use with a 210 pound frame and l.it)
pound cover. Acceptable products include Sumter Machinery Company (4NfC-t8 & #MF-11). Dewev
Brothers Foundry (#MHRCR-2001) or an approved equal. '
The frame and cover shall be properly set in a bed of mortar and aligned to fit the top section of the
manhole. Concrete brick, set in mortar, shall be used to adjust the top of the frame and cover to finished
grade; however, no more than four(4)courses of brick will be used for adjustment.
E. MANHOLE INVERTS
Manhole inverts and benches shall be constructed in accordance with the standard details shown on the
drawings. Invert shall be a U-shaped channel with a height of 0.8 of the diameter and be a smooth
continuation of the pipe. The benches shall be constructed with a slope of one (1) inch per foot to the
channel. The channel and invert shall be constructed with a minimum of 2000 psi concrete or brick fill
with concrete finish minimum one (1) inch thick. Where sewer changes directions at the manhole,
channel shall be constructed with a smooth curve with as large a radius as the diameter of the manhole
will allow.
F. MANHOLE DROPS
Standard drop manholes will be constructed only at those locations shown on the drawings or as approved
by the Engineer. The design of the drop connection shall be in accordance with the standard detail
drawing.
The cost of the extra pipe, labor, etc., required to construct a drop manhole will be included in the unit
price for the drop manhole at the depths listed.
G. INSPECTION AND ACCEPTANCE
Manholes shall be constructed to provide a true circular inside diameter with properly corbeled tops,'
satisfactory inverts and properly placed steps and castings.- Any visible leaks in the manholes shall be
completely stopped to the satisfaction of the Engineer.
H. MANHOLE VENTS
Where designated on the contract drawings, a 4 inch-diameter vent pipe shall be installed as an integral
part of the manhole. The vent pipe is to be tapped into the upper most section of the manhole, anchored
in concrete and extended vertically to the elevation shown on the drawings. The pipe shall have a reverse
bend and screen to prohibit rain and foreign materials from entering pipe.
I. COMPENSATION
Quantities of manholes installed in accordance with the requirements of this section shall be verified in
the field by the Engineer. Payment shall be made for Standard Manholes or Standard Drop Manholes at
the prescribed depth measured from the top of the frame to the low pipe invert at the respective unit price

bid.
Payments for watertight manhole frame and covers and manhole vents will be considered to be included in
the unit price bids for the respective manholes.
Payment shall constitute full compensation for furnishing all equipment, tools, labor and incidentals and
performance of all work necessary to install the manholes as specified.

No. WQ0020676 for the construction and operation of the subject wastewater collection system
extension. This Permit shall be effective from the date of issuance until rescinded and shall be
subject to the conditions and limitations as specified therein. This cover letter shall be
considered a part of this permit and is therefore incorporated therein by reference.
Please pay particular attention to condition three (3) of the permit which requires that
the wastewater collection facilities be properly operated and maintained in accordance with
15A NCAC 2H .0227 or any individual system-wide collection system permit issued to the
Permittee.
Permitting of this project does not constitute acceptance of any part of the project that does
not meet 15A NCAC 2H .0200; the Division of Water Quality's (Division) Gravity Sewer Minimum
Design Criteria adopted February 12, 1996 as applicable; and the Division's Minimum Design Criteria
for the Fast-Track Permitting of Pump Stations and.Force Mains adopted June 1, 2000 as applicable,
unless specifically mentioned herein. Division approval is based on acceptance of the certification
provided by a North Carolina-licensed Professional Engineer in the application. It shall be the
Permittee's responsibility to ensure that the as-constructed project meets the appropriate design
criteria and rules. Failure to comply may result in penalties in accordance with North Carolina
General Statute 143-215.6A through 143-215.6C, construction of additional or replacement
wastewater collection facilities, and/or referral of the North.Carolina-Licensed Professional Engineer
to the licensing board.
In accordance with the provisions of Article 21 of Chapter 143, General Sta
Carolina as amended, and other applicable Laws; Rules, and Regulations, permissio
granted to the City of Hendersonville for the construction and operation of approximately 6
feet of eight (8) inch ductile iron pipe gravity sewer and 1,137 linearfeet of eight (8) inch PVC
sewer to serve Hillside Commons as part of the City of Hendersonville's wastewater collec
system and the discharge of 12,960 gallons per day of collected domestic wastewater into the Ci
of Hendersonville's existing sewerage system, pursuant to the application received on November 20,
2001 and in conformity with 15A NCAC 2H .0200; the Division's Gravity Minimum Design Criteria
adopted February 12, 1996 as applicable; the Division's Minimum Design Criteria for the Fast-Track
Permitting of Pump Stations and Force Mains adopted June 1 2000 as applicable; and other
supporting data subsequently filed and approved by the Department of Environment and Natural
Resources and considered a part of this permit.
The sewage and wastewater collected by this system shall be treated in the City of
Hendersonville's Wastewater Treatment facility (NPDES Permit No. NC0025534) prior to being
discharged into the receiving stream.
If any parts, requirements, or limitations contained in this permit are unacceptable, you have
the right to request an adjudicatory hearing upon written request within 30 days following receipt of
this permit. This request must be in the form of a written petition, conforming to Chapter 150B of
North Carolina General Statutes, and filed with the Office of Administrative Hearings, 6714 Mail
Service Center, Raleigh, NC 27699-6714. Unless such demands are made, this permit shall be
final and binding.

This permit shall be effective from the date of issuance until rescinded and shall be subject

1. This permit shall become voidable unless the wastewater collection facilities are constructed
in accordance with the conditions of this permit: 15A NCAC 2H .0200; the Division of Water
Quality's (Division) Gravity Sewer Minimum Design Criteria adopted February 12, 1996 as
applicable; the Division's Minimum Design Criteria for the Fast-Track Permitting of Pump
Stations and Force Mains adopted June 1, 2000 as applicable; and other supporting materials
unless specifically mentioned herein.
2. This permit shall be effective only with respect to the nature and volume of wastes described
in the application and other supporting data.
3. The wastewater collection facilities shall be properly maintained and operated at all times.
The Permittee shall maintain compliance with an individual system-wide collection system
permit for the operation and maintenance of these facilities as required by 15A NCAC 2H
.0227. If an individual permit is not required, the following performance criteria shall be met
as provided in 15A NCAC 2H .0227:
a. The sewer system shall be effectively maintained and operated at all times to prevent
discharge to surface waters, and any contravention of the groundwater standards in
15A NCAC 2L .0200 or the surface water standards in 15A NCAC 2B .0200.
b. A map of the sewer system shall be developed prior to January 1 , 2004 and shall
be actively maintained.
C. An operation and maintenance plan shall be developed and implemented_
d. Pump stations that are not connected to a telemetry system shall be inspected daily,
as defined in 15A NCAC 2B .0503(5), until July 1, 2001; and thereafter, pump stations
shall be inspected every day. Pump stations that are connected to a telemetry system
shall be inspected at least once per week.
e. High-priority sewer lines shall be inspected at least once per every six-month period
of time.
f. A general observation of the entire sewer system shall be conducted at least once per
year.
g. Inspection and maintenance records shall be maintained for a period o
years.
h Overflows and bypasses shall be reported to the appropriate Division Regional
in accordance with 15A NCAC 2B .0506(a), and public notice shall be provide
required by North Carolina General Statute 143-215.1 C.
4. This permit shall not be transferable. In the event there is a desire for the wastewater facilities
to change ownership, or there is a name change of the Permittee, a,formal permit request
shall be submitted to the Division accompanied by documentation from the parties involved,
and other supporting materials as may be appropriate. The approval of this request shall be
considered on its merits and may or may not be approved.
5. Construction of the gravity sewers shall be scheduled so as not to interrupt service by the
existing utilities nor result in an overflow or bypass discharge of wastewater to the surface
waters of the State.
6. Upon completion of construction and prior to operation of these permitted facilities, a
certification, a copy of the construction -record drawings, as well as supporting design
calculations for any pump stations permitted as part of this project shall be received from a
North Carolina-licensed Professional Engineer certifying that the facilities have been installed
in accordance with this permit; 15A NCAC 2H .0200; the Division's Gravity Sewer Design
Criteria adopted February 12, 1996 as applicable; the Division's Minimum Design Criteria for
the Fast-Track Permitting of Pump Station and Force Main adopted June 1, 2000 as
applicable; and other supporting materials. If this project is to be completed in phases and
partially certified, you shall retain the responsibility to track further construction approved
under the same permit, and shall provide a final certificate of completion once the entire
project has been completed. A copy of the construction record drawings, indicating the
facilities constrycted in the' phase being 'certified, shall be submitted with each partial
certification. Mail the Engineer's Certification, one copy of the "Construction Record
Drawings", and one copy of the supporting design calculations to the Non-Discharge '
Permitting Unit, 161.7 Mail Service Center, Raleigh, NC'27699-1617.
7. A copy of the construction record drawings shall be.maintained on file by the Permittee for the
life of the wastewater collection facilities.
8. Failure to abide by the conditions and limitations contained in this permit; 15A NCAC 2H
.0200; the Division's Gravity Sewer Design Criteria adopted February 12, 1996 as applicable;
the Division's Minimum Design Criteria for the Fast-Track Permitting of Pump Station and
Force Mains adopted June 1, 2000 as applicable; and other supporting materials may subject
the Permittee to an enforcement action by the Division„ in accordance with North Carolina
General Statutes 143-215.6A through 143-215.6C.
9. In the event that the wastewater collection facilities fail to perform satisfactorily, including the
creation of nuisance conditions, the Permittee shall take immediate corrective action,
including those as may be required by this Division, such as the construction of additional or
replacement facilities.
10. The issuance of this permit shall not exempt the Permittee from complying with any and all
statutes, rules, regulations, or ordinances that may be imposed by other government agencies
(local, state and federal) which have jurisdiction, including but not limited to applicable river
buffer rules in 15A NCAC 2B .0200, erosion and sedimentation control requirements in 15A
h. 4 and under the Division's General.Permit NCG0100000, and any requirements
fining to wetlands under 15A NCAC 213 .0200 and 15A NCAC 2H .0500.

The Permittee shall report by telephone to the Asheville Regional Office, telephone number
828/251-6208 as soon as possible, but in no case more than 24 hours or on the next working
day, following the occurrence or first knowledge of the occurrence of either of the following:
a. Any process unit failure, due to known or unknown reasons, that renders the facility
incapable of adequate wastewater transport, such as mechanical or electrical failures of
pumps, line blockage or breakage, etc.; or
b. Any failure of a pumping station or sewer line resulting in a by-pass directly to receiving
waters without treatment of all or any portion of the influent to such station or facility.
Persons reporting such occurrences by telephone shall also file a written report in letter form
within five days following first knowledge of the occurrence. This report must outline the actions
taken or proposed to be taken to ensure that the problem does not recur.

SECTION 07110 SEWER PIPE (GRAVITY) AND APPURTENANCE MATERIAL
07110 .1 SCOPE
These specifications shall apply to the materials to be
furnished and installed to complete the sanitary sewer line
installations in accordance with the plans . All pipe and
appurtenances shall be of the class and type as indicated on the
plans and designated herein.
07110 .2 GENERAL
All materials shall be first quality with smooth interior and
exterior surfaces, free from cracks, blisters, honeycombs and
other imperfections, and true to theoretical shapes and forms
throughout . All materials shall be subject to the inspection of
the Engineer at the plant, trench, or other point of delivery,
for the purpose of culling and rejecting materials, which does
not conform to the requirements of these specifications . Such
material shall be marked by the Engineer and the Contractor
shall remove it from the project site upon notice being received
of its rejection.
As particular specifications are cited, the designation shall be
construed to refer to the latest revision under the same
specification number, or to superseding specifications under a
new number except provisions in revised specifications which are
clearly inapplicable.
Polyvinyl Chloride Pipe
Polyvinyl Chloride (PVC) pipe shall meet the requirement of ASTM
D 3034, latest edition, and be suitable for use as a gravity
sewer conduit . The standard dimension ratio' (SDR) shall be
thirty-five (35) unless otherwise specified. The pipe shall be
supplied in standard laying lengths of twenty (20) feet and
twelve and one-half (12 .5) feet .
All PVC pipe joints shall be of an integral bell and spigot of
the same material as the pipe with a solid cross-section rubber
"O" ring securely locked in place at the point of manufacture.
Service saddles and other fittings shall be supplied by the pipe
manufacturer and shall be of the same material and type of
construction as the pipe material .

Ductile Iron Pipe
Ductile Iron Pipe shall be manufactured in accordance with ANSI
Specification A 21 . 51 . All Ductile Iron Pipe shall be Class 50
unless otherwise specified and shall be lined with cement mortar
not less than one sixteenth (1/16) inch thick conforming to ANSI
Specification A 21 . 4 .
Slip or "push-on" joints shall be manufactured in accordance
with ANSI Specification A 21 . 11 . Bells of "slip" joint pipe
shall be contoured to receive a bulbshaped, circular rubber
gasket, and plain ends shall have a slight taper to facilitate
installation. The lubricant used in making up the joints shall
be furnished by the pipe manufacturer. The jointing shall be
done by guiding the plain end into the bell until contact is
made with the gasket and by exerting a sufficient compressive
force to drive the joint home until plain end makes full contact
with the base of the bell .
Steel Casing Pipe
Steel casing shall conform to ASTM A-53 , Grade B or ASTM A-139
Grade B standard specifications . Casing pipe shall have a wall
thickness as follows, unless otherwise noted in the bid
documents :
12 " thru 24" casing - 0 .250 inches wall
thickness
30" casing - 0 .312 inches wall
thickness
36" casing - 0 .375 inches wall
thickness
07110.3 MANHOLES
Precast concrete manhole bases, risers and cones shall conform
to ASTM C-478, latest revision for Precast Reinforced Concrete
Manhole sections . Tapered sections and transition sections,
where required, shall be of eccentric cone design, having the
same wall thickness and reinforcement as the cylindrical ring
sections . Flat slab tops shall be required for very shallow
manholes and where shown or specified. Cast iron manhole covers
and assemblies shall be cast into slab tops for access into
manholes .
Minimum compressive strength of concrete shall be 4, 000 psi
and the maximum permissible absorption shall be six and one-half
(6 . 5) percent. Risers shall be reinforced with a single cage of
steel placed within the center one-third (1/3 ) of the wall . The
tongue or the groove of the joint shall contain one (1) line of
circumferential reinforcement equal in area to that in the
barrel of the manhole riser.

The minimum cross-secticnal area of steel per linear
foot shall be twelve one-hundredths (0 . 12) square
inches for larger sizes . ?recast manhole sections shall
fit together readily and shall have a self-contained
"O" ring rubber gasket conforming to ASTM C-443 .
The quality of materials, the process of manufacture, and the
finished manhole sections shall be subject to inspection and
approval by the Engineer and his inspector. The manhole sections
shall be perpendicular to their longitudinal axis within the
limits listed in ASTM C-478 .
07110 .4 MANHOLE FRAMES AND COVERS
Casting for manhole frames and covers shall be tough, even
grained, soft grey iron_, free from burnt on sand and other
injurious defects and conform to the requirements of ASTM A 48,
latest revision Class 30 .
07110 .5 BRICK
Brick for manholes and other structures shall conform to
applicable requirements of ASTM Specification C 62 , latest
revision, Grade SW.
07110. 6 CONCRETE
Concrete for sewer structures shall consist of a mix of Portland
cement, fine and coarse aggregate and water to produce concrete
with a minimum compressive strength at twenty-eight (28) days of
not less than 3 , 000 psi when tested according to ASTM
Designation C-39 or C-42 .
Air Content (by volume) of the mix shall be between four (4) and
eight (8) percent when placed. The consistency of the concrete
shall be such as to allow it to be worked into place without
segregation or excessive laitance. Slump shall be no more than
four (4) inches .
07110.7 MORTAR
Mortar for masonry in sewer structures shall be 1:2 cement sand
mix, except that hydrated lime may be substituted for not more
than ten (10%) percent by weight, of the cement.

07110 .8 FLEXIBLE COUPLINGS
Whenever it becomes necessary to join sewer pipe lines of
dissimilar materials or pipe sizes it shall be required to use
a flexible coupling. The coupling shall be made of virgin poly
vinyl chloride (PVC) shall not harden, and shall be impervious
to all known soil conditions . The coupling shall provide a
permanent leakproof seal approved by the Southern Building Code
Congress and manufactured in accordance by Fernco Joint Sealer
Company or an approved equal .

and specified herein and in agreement wit: the General
Conditions of these contract documents .
07120 .2 HANDLING AND STORING MATERIALS
The Contractor shall unload pipe so as to avoid deformation or
other injury thereto . Pipe shall not be placed within pipe of a
larger size and shall not be rolled or dragged over gravel or
rock during handling. The Contractor shall store the pipe on
sills above storm drainage level and deliver for laying after
the trench is excavated. When any joint or section of pipe is
damaged during transporting, unloading, 'handling, or storing,
the undamaged portions of the joint or section may be used where
partial lengths are needed, or, if damaged sufficiently, the
Engineer will reject the joint or section as being unfit for
installation.
If any defective pipe is discovered after installation,
it shall be removed and replaced with sound pipe or
shall be repaired by the Contractor in an approved
manner and at his own expense.
07120.3 PIPE ALIGNMENT AND GRADE
The layout of gravity sanitary sewer lines and invert elevations
at governing points are as shown on the drawings .
The Contractor shall do all layout work for lines and grades
from that information shown on the drawings or as furnished by
the Engineer.
When a laser beam instrument is used to set line and grade, the
unit must be maintained in good working order, and the
calibration checked daily for both alignment and percent grade.
In the event the r=_auired accuracy of alignment and grade is not
adhered to, the Engineer will prohibit the use of laser beams .
07120 .4 PREPARATION OF PIPE FOUNDATION
The pipe foundaticn shall be prepared to be uniformly firm and
shall be true to :he lines and grades as shown on the plans .
The preparation of the pipe bedding shall be in accordance with
the typical trench cross sections as shown on the drawings .

Under no circumstances s-all pipe be laid in water, on
rock, or when trench conditions or weather is
unsuitable for such work.
Whenever the nature of the ground will per-nit, the excavations
at the bottom of the trench shall have the shape and dimensions
of the outside lower third of the circumference of the pipe,
care being taken to secure a firm bearing support uniformly
throughout the length of the pipe. A space shall be excavated
under and around each bell to sufficient depth to relieve it of
any load and to allow ample space for filling and finishing the
joint. The pipe, when thus bedded firmly, shall be on the exact
grade. In case the bed shaped in the bottom of the trench is too
low, the pipe shall be completely removed from position, and
earth of suitable quality shall be placed and thoroughly tamped
to prepare a new foundation for the pipe. In no case shall the
pipe be brought to grade by blocking up under the barrel or bell
of same, but a new and uniform support must be provided for the
full length of the pipe. Where rock or boulders are encountered
in the bottom of the trench, the same shall be removed to such
depth that no part of the pipe, when laid to grade, will be
closer to the rock or boulders than six (6) inches . A suitably
tamped and shaped foundation of approved material shall be
placed to bring the bottom of the trench to proper subgrade over
rock or boulders .
Where the foundation material is found to be of poor supporting
value, the Engineer may make minor adjustment in the location of
the pipe to provide a more suitable foundation. Where this is
not practical, the foundation shall be conditioned by removing
the existing foundation material by undercutting to the depth as
directed by the Engineer, within the limits established on the
plans, and backfilling with either an approved material secured
from unclassified excavation or borrow excavation at the nearest
accessible location along the project, or foundation
conditioning material consisting of crushed stone or gravel or
a combination of sand and crushed stone or gravel approved by
the Engineer as being suitable for the purpose intended. The
selection of the type of backfill material to be used for
foundation conditioning will be made by the Engineer.
The Contractor shall remove all water which may be encountered
or which may accumulate in the trenches by pumping or bailing
and no pipes shall be laid until the water has been removed from
the trench. Water so removed from the trench must be disposed of
in such a manner as not to cause injury to work completed or in
progress .

07120 . 5 PIPE LAYING
Pine shall be laid with bell ends facing in the direction of
pipe laying, unless directed otherwise by the Engineer. In all
cases, pipe is to be installed in strict accordance with the
manufacturer ' s recommendations and the contract material
specifications . The Engineer may augme= any manufacturer' s
installation recc.-mendations if, in his coinion, it will best
serve the interest of the Owner.
Proper tools , implements, and facilities satisfactory to the
Engineer shall be provided and used for the safe and convenient
prosecution of pipe laying. All pipe and other materials used in
the laying of pipe will be lowered into the trench piece by
piece by means of suitable equipment in such a manner to prevent
damage to the pipe, materials, to the protective coating on the
pipe materials, and to provide a safe working condition to all
personnel in the trench. Each piece of pipe being lowered into
the trench shall be clean, sound and free from defects . It shall
be laid on the prepared foundation, as specified elsewhere to
produce a straight line on a uniform grade, each pipe being laid
so as to form a smooth and straight inside flow line. Pipe shall
be removed at any time if broken, injured or displaced in the
process of laying same, or of backfilling the trench.
When cutting short lengths of pipe, a pipe cutter as approved by
the Engineer will be used and care will be taken to make the cut
at right angles to the centerline of the pipe or on the exact
skew as shown on the plans. In the case of push-on pipe, the cut
ends shall be tapered with a portable grinder, or coarse file to
match the manufactured taper.
During times when pipe laying is not in progress, the open ends
of pipe shall be closed and no trench water or other material
shall be permitted to enter the pipe .
Where the pipe is laid on a grade of ten (10) percent or
greater, the laying shall start at the bottom of the slope and
proceed upward with the bell end of the new pipe upgrade. All
pipe laid on a grade of ten (10) percent or greater shall
require thrust blocking and keying as shown on the drawings and
standard details .
Where pipe lines of different materials are joined together a
standard sewer repair coupling shall be used. The couplings
shall be Eastern Standard Sewer Repair Couplings (Mission Rubber
Company) , the Fernco Joint Sealer Company or an equal product
approved by the Engineer.

07120 . 6 BACKFILLING
All backfill shall be compacted so as not =o damage the pipe and
appurtenances and shall be compacted to ninety-five (95) percent
of the Standard Proctor Test for the various types of backfill
materials .
Methods of backfilling shall be in stric� accordance with the
pipe manufacturer' s recommendations . All backfill materials
shall have been approved by the Engineer. Select backfill
material shall be used when requested by the Engineer. Select
material shall be defined as a finely graded material free from
stones over one-half (1/) 2 inch in diameter, plastic clays,
organic material, frozen lumps and various debris and shall be
approved by the Engineer prior to its installation.
Care shall be taken during backfill and compaction operations to
maintain alignment and prevent damage to the joints . The
backfill shall be kept free from stones, frozen lumps, chunks of
highly plastic clay, or other objectional material . All pipe
backfill areas shall be graded and maintained in such a
condition that erosion or saturation will not damage the pipe
bed or backfill .
Heavy equipment shall not be operated over any pipe until it has
been properly backfilled and has a minimum cover as required by
the plans . Where any part of the required cover is above the
proposed finish grade, the Contractor shall place, maintain, and
finally remove such material at no cost to the Owner. Pipe which
becomes mis-aligned, shows excessive settlement, or has been
otherwise damaged by the Contractor' s operations shall be
removed and replaced by the Contractor at no cost to the Owner.
The Contractor shall maintain all pipes installed in a condition
that they will function continuously from the time the pipe is
installed until the project is accepted.
07120.7 MEASUREMENT AND PAYMENT
Quantities of pipe installed in accordance with the requirements
of this section shall be verified in the field by the Engineer.
Payment shall be made for total number of lineal feet of pipe
installed at the unit prices bid in each depth category. Price
and payment shall constitute full compensation for furnishing
all equipment, tools, labor, and materials to complete the work
as specified.

SECTION 07290 SANITARY SEWER MANHOLE MATERIAL, INSTALLATION & TESTING
07290 . 1 DESCRIPTION OF WORK
The work covered under this section shall consist of furnishing
all materials , lahor, equipment, and services for the complete
installation of sanitary sewer manholes .
07290 .2 MANHOLE CONSTRUCTION
Sanitary sewer manholes shall be installed at each break in
line or grade in each sanitary sewer line as shown on the
contract drawings .
The manholes shall consist of precast reinforced concrete
sections including a base section and conical top section.
These precast concrete sections shall conform to ASTM C-478,
latest revision.
The minimum inside diameter of the manhole shall be five (5)
feet unless noted otherwise on the drawings . Flat slab tops may
be used in non-traffic areas, for very shallow manholes, or
where shown on the drawings or approved by the Engineer.
Maximum allowable absorption of concrete shall be eight (8) per
cent. Minimum compressive strength of concrete for all sections
shall be 4000 psi . Each precast concrete section shall be
joined together with a "0" ring rubber gasket conforming to
ASTM C-443 or an approved sealant conforming to Federal
Specification 55500210 .
Manhole inverts shall be constructed of concrete or concrete
and brick of . semicircular section conforming to the inside
diameter of the outlet sewer. Changes in size of pipe or grade
shall be made gradually and change in direction constructed by
using true curves. Each manhole shall be provided with such
channels for all connecting sewer pipes .
07290 .3 MANHOLE INSTALLATION
The manhole foundation shall be prepared so as to provide a
firm, level area on which to place the precast concrete manhole
base section. When poor foundation soil is encountered or
excess groundwater exists the foundation shall be excavated
twelve (12) inches below the final subgrade elevation
backfilled with washed stone to provide a proper foundation.

The manhole sections shall be lifted from the side of the
excavation to the bottom of the trench with equipment and
support slings capable o- safely handling the heavy concrete
pieces . The manhole shay_ be set plumb and adjusted to the
final finished surface grade with brick and mortar.
Pipe openings shall be exactly aligned to that of the pipe
entering and leaving the manhole . The gravity sanitary sewer
pipe lines shall be placed in the manhole openings, properly
aligned, and set to grade. Sanitary sewer shall be connected
to the manholes using lock joint flexible manhole sleeves or
equal . For large diameter pipe where a flexible rubber sleeve
is not available the pipe line shall be sealed into the manhole
using an expanding type or non-shrink type grout .
Manhole steps shall be properly spaced as shown in the standard
detail drawing, but in no case more than fifteen (15) inches on
center. The steps shall be made of heavy cast iron (10 inch
thick) , reinforced plastic (ASTM 2146-Type II) , or aluminum
(min. load 1000 lbs . ) .
07290 .4 MANHOLE FRAMES AND COVERS
Frames and covers shall be cast iron of superior quality, tough
and even texture. Castings shall be gray iron conforming to
ASTM A-48, size as indicated, free from blow holes, porosity,
hard spots, shrinkage distortion, or other defects, well
cleaned and coated with asphalt paint. This paint shall result
in a smooth coating, tough and tenacious when cold, not tacky
and not brittle. The bearing surface between frame and cover
shall be machined to prevent rocking and rattling.
The standard manhole casting shall be designed for heavy duty
use with a 210 pound frame and 130 pound cover. Acceptable
products include Sumter Machinery Company (#MC-18 & MF-11) ,
Dewey Brothers Foundry (#MHRCR-2001) or an approved equal .
Special waterproof manhole frame and covers shall be installed
only at those locations indicated on the contract drawings .
Acceptable products include Neenah Foundry, Sumter Machinery
Co. , Dewey Brothers Foundry, or an approved equal .
The frame and cover shall be properly set in a bed of mortar
and aligned to fit the top section of the manhole. Concrete
brick, set in mortar, shall be used to adjust the top of the
frame and cover to finished grade; however, no more than four
(4) courses of brick will be used for adjustment.

07290 . 5 MANHOLE INVERTS
Manhole inverts and benches shall be cons=ructed in accordance
with the standard details shown on the drawings . Invert shall
be a U-shaped channel with a height of eight-tenths (0 . 8) of
the diameter and be a smooth continuation of the pipe. The
benches shall be constructed with a slope of one (1) inch per
foot to the channel . The channel and invert shall be
constructed with a minimum of 2000 psi concrete or brick fill
with concrete finish minimum one (1) inch thick. Where sewer
changes directions at the manhole, channel shall be constructed
with a smooth curve with as large a radius as the diameter of
the manhole will allow.
07290 . 6 MANHOLE DROPS
Standard drop manholes will be constructed only at those
locations shown on the drawings or as approved by the engineer.
The design of the drop connection shall be in accordance with
the standard detail drawing.
The cost of the extra pipe, labor, etc . required to construct a
drop manhole will be included in the unit price for the drop
manhole at the depths listed.
07290 .7 INSPECTION AND ACCEPTANCE
Manholes shall be constructed to provide a true circular inside
diameter with properly corbeled tops, satisfactory inverts and
properly placed steps and castings . Any visible leaks in the
manholes shall be completely stopped to the satisfaction of the
Engineer.
07290.8 MANHOLE VENTS
Where designated on the contract drawings, a four (4) inch
diameter vent pipe shall be installed as an integral part of
the manhole . The vent pipe is to be tapped in to the upper most
section of the manhole, anchored in concrete and extended
vertically to the elevation shown on the drawings . The pipe .
shall have a reverse bend and screen to prohibit rain and
foreign materials from entering pipe .
The pipe material shall be Schedule 40 Galvanized Steel with
two coats of epoxy paint approved by the Engineer.

07290 .9 MEASUREMENT AND PAYMENT
Quantities of manholes installed in accordance with the
requirements of this section shall be verified in the field by
the Engineer. Payment shall be made for Standard Manholes or
Standard Drop Manholes at the prescribed depth measured from
the top of the frame to the low pipe invert at the respective
unit price bid.
Payments for watertight manhole frame and covers and manhole
vents will be considered to be included in the unit price bids
for the respective manholes .
Payment shall constitute full compensation for furnishing all
equipment, tools, labor and incidentals and performance of all
work necessary to install the manholes as specified.

SECTION 07295 SANITARY SEWER SERVICE CONNECTIONS
07295 .1 DESCRIPTION OF WORK
The work covered under this section shall consist of furnishing
all materials, labor, equipment, and services for the complete
installation of a sanitary sewer service connection from the
sanitary sewer (gravity) main line to the edge of the property
to be served as shown on the project drawings .
07295.2 CONNECTION TO MAIN
The standard sewer service connection shall be four (4) inches
in diameter unless shown otherwise on the drawings, and shall
connect to the main at a "wye" branch connection installed with
the pipe line as it is being laid. The "wye" branch shall be of
the same material as the main pipe line. Direct taps into the
sewer main will not be acceptable unless approved by the
Engineer prior to the laying of the main line.
07295.3 CONNECTION TO MANHOLE
When shown on the drawings or directed by the Engineer, a sewer
service connection shall be made into a manhole. The connection
shall be made at the manhole invert. The invert shall be rebuilt
so as to provide a smooth transition from service connection to
main line. Inverts and benches shall be constructed in such a
way as to prevent the deposition of solids in the manhole.
07295.4 SERVICE LINE
The service line shall be installed from the "wye" branch
connection to the edge of the public or utility right-of-way as
shown on the plans.
The sewer service line will be constructed of either PVC pipe
(PVC schedule 40) pipe, or Ductile Iron (CL 350) as shown on the
plans . When the service line pipe material is different from
that of the main line a flexible, water tight, rubber transition
coupling shall be used to join the service line to the "wye"
connection. The service line shall be laid at a minimum grade of
one (1 . 0) percent with a minimum cover of three (3 . 0) feet. Less
than three (3 ) feet of cover will require the use of ductile
iron pipe .

07295 .5 CLEANOUT AND PLUG
At the edge of the public or utility righ=-of-way a "cleanout"
shall be installed. The cleanout shall consist of a "wye" branch
connection, forty-five (45) degree bend, riser pipe, and
threaded plug installed flush with finished ground elevation.
The end of the utility owner' s sewer service connection shall
terminate at the end of the pipe which will normally extend five
(5) feet beyond the "wye" branch for the cleanout. A watertight
plug shall be installed at the end of this line until such time
as the property owner directs the utility owner to connect his
facilities to the sewer system.
07295. 6 MEASUREMENT AND PAYMENT
Payment for the furnishing and installation of all service
connections ( "wye" branches, service line, clean outs, plugs,
couplings, etc . ) as described in this section and as shown on
the plans, shall be covered under the contract unit price bid
for each Sewer Service Connection.

In accordance with your application received May 19, 1998, attached is Permit No. WQ0015370,
dated June 2, 1998, to the City of Hendersonville for the construction and operation of the subject
wastewater collection extension.
This permit shall be effective from the date of issuance until rescinded and you shall be subject to
the conditions and limitations as specified in the North Carolina Gravity Sewer Design Criteria adopted
February 12, 1996 and the attached permit.
In accordance with the provisions of Article 21 of Chapter 143, General Statutes of North Carolina
as amended, and other applicable Laws, Rules, and Regulations, permission is hereby granted for the
construction and operation of approximately 5,798 linear feet of 8 inch gravity sewer to serve 128-two
bedroom units and 71-three bedroom units at Wolfpen Improvements, Phase I through IV, and the
discharge of 56,280 GPD of collected domestic wastewater into the City of Hendersonville's existing
sewerage system, pursuant to the application received May 19, 1998, and in conformity with the North
Carolina Gravity Sewer Design Criteria.
The sewage and wastewater collected by this system shall be treated in the City of Hendersonville
Wastewater Treatment Facility (NPDES Permit No. NC0025534) prior to being discharged into the
receiving stream.
If any parts, requirements, or limitations contained in this permit are unacceptable, you have the
right to request an adjudicatory hearing upon written request within thirty (30) days following receipt of
this permit. This request must be in the form of a written petition,conforming to Chapter 150B of North
Carolina General Statutes, and filed with the Office of Administrative Hearings, P.O. Drawer 27447,
Raleigh,NC 27611-7447. Unless such demands are made this permit shall be final and binding.
The issuance of this permit does not preclude the Permittee from complying with any and all
statutes, rules, regulations, or ordinances which may be imposed by other government agencies (local,
state, and federal) which have jurisdiction.

GRAVITY SEWER COLLECTION SYSTEMS PERMIT
This permit shall be effective from the date of issuance until rescinded and shall be subject to the
following specified conditions and limitations:
1. This permit shall become voidable unless the facilities are constructed in accordance with the
conditions of the Division of Water Quality Gravity Sewer Design Criteria adopted February 12,
1996.
2. This permit is effective only with respect to the nature and volume of wastes described in the
application and other supporting data.
3. The facilities must be properly maintained and operated at all times.
4. This permit is not transferable. In the event there is a desire for the facilities to change ownership, or
there is a name change of the Permittee, a formal permit request must be submitted to the Division of
Water Quality (Division) accompanied by an application fee,documentation from the parties involved,
and other supporting materials as may be appropriate. The approval of this request will be considered
on its merits and may or may not be approved.
5. Construction of the gravity sewers shall be scheduled so as not to interrupt service by the existing
utilities nor result in an overflow or bypass discharge of wastewater to the surface waters of the State.
6. Upon completion of construction and prior to operation of this permitted facility, a certification
and a copy of the record drawings (i.e., as constructed plans) must be received from a professional
engineer certifying that the permitted facility has been installed in accordance with the Division of
Water Quality Gravity Sewer Design Criteria adopted February 12, 1996, and other supporting
materials. If this project is to be completed in phases and partially certified, you shall retain the
responsibility to track further construction approved under the same permit, and shall provide a final
certificate of completion once the entire project has been completed. A copy of the record drawings,
indicating the facilities constructed in the phase being certified, shall be submitted with each partial
certification. Mail the Certification and one (1) copy of the "Record Drawings"to the Non-Discharge
Permitting Unit, P.O. Box 29535, Raleigh, NC 27626-0535.
7. A copy of the Record Drawings shall be maintained on file by the Permittee for a minimum of five
years from the date of the completion of construction.
8. Failure to abide by the conditions and limitations contained in this general sewer permit or the North
Carolina Gravity Sewer Design Criteria may subject the Permittee to an enforcement action by the
Division, in accordance with North Carolina General Statute 143-215.6A to 143-215.6C.
9. In the event that the facilities fail to perform satisfactorily, including the creation of nuisance
conditions, the Permittee shall take immediate corrective action,including those as may be required by
this Division, such as the construction of additional or replacement wastewater collection facilities.

10. Noncompliance Notification:
The Permittee shall report by telephone to the Asheville Regional Office, telephone number
(704) 251-6208, as soon as possible, but in no case more than 24 hours or on the next working day,
following the occurrence or first knowledge of the occurrence of either of the following:
a. Any process unit failure, due to known or unknown reasons,that renders the facility incapable of
adequate wastewater transport, such as mechanical or electrical failures of pumps,line blockage or
breakage, etc.; or
b. Any failure of a sewer line resulting in a by-pass directly to receiving waters without treatment of
all or any portion of the influent to such station or facility.
Persons reporting such occurrences by telephone shall also file a written report in letter form within
five (5) days following first knowledge of the occurrence. This report must outline the actions taken
or proposed to be taken to ensure that the problem does not recur.

This form may be photocopied for use as an original. Pump Stations, force mains and low pressure sewers
(including S.T.E.P. systems) are not to be included as pan of this application package.
Any changes made to the content of this form shall not be allowed and will result in the
application being returned.
This application will not be accepted by DEM unless all of the applicable items are included
with the submittal.
1. REQUIRED ITEMS CHECKLIST:

I. One original and one copy of the completed and properly executed application form.
2. The appropriate permit processing fee,in accordance with 15A NCAC 2H.0205(c)(5). The fee for
sewer extensions is$400. The fee for sewer extensions by partially delegated municipalities
(applies only to those governmental jurisdictions that have specific delegation review authority,
as granted by the Division of Environmental Management) is 3200.
3. If the owner/authority of the wastewater treatment facility(VIWTF)and/or collection system that will be
accepting the wastewater flow from this project is different from the applicant of the project, then a letter(s)
must be provided from the owner/authority of the W WTF and/or collection system specifying the
volume of flow that will be accepted. The letter(s)should be recent(within one [I] year)and should
refer to the project by the same name as that identified on the application and the plans/specifications.
4. If the application is being submitted in the name of a privately owned public utility, written
documentation must be submitted from the Utilities Commission which demonstrates that the utility
is authorized to hold the franchise for the area to be served. In the case of contiguous service areas,
written documentation must be provided from the Utilities Commission acknowledging these areas
are covered under an existing franchise.
5. One original and two copies of a properly executed Operational Agreement must be submitted if the
sewer extension will be serving single family residences,condominiums,or town houses AND if the
subject sewer extension is owned by the individual residents, a homeowners association,or a developer.
